Die extrakorporale Zirkulation (EKZ) ist wichtiger Bestandteil der modernen Herzchirurgie. Gleichwohl führt sie zu erhöhter Inflammation und Hämolyse sowie zu Störungen im Gerinnungs- und Fibrinolysesystem. In der vorliegenden Arbeit wurden zwei Richtungen der modernen Herzchirurgie mit dem Standard der herkömmlichen Herz-Lungen-Maschine (HLM) verglichen. Hierbei wurden vier minimal-invasive Extrakorporalsysteme und ein off-pump-coronary-artery-bypass- System (OPCAB) (d.h. das völlige Fehlen einer EKZ) evaluiert. Die Bewertung erfolgte anhand ausgewählter Parameter der Hämolyse, Inflammation und Gerinnung/Fibrinolyse. Grundlegende Innovationen minimal-invasiver Systeme sind: reduzierte Fremdoberfläche; biokompatiblere Oberflächen durch Beschichtungen verschiedener Art; fehlender Blut-Luft-Kontakt; Vermeidung der Reinfusion von Zellen, zerstörter Zellbestandteile und Mediatoren. Ziel der vorliegenden Arbeit war es den Effekt der verschiedenen Einzelkomponenten näher zu beleuchten und zu prüfen, ob es eine einzelne technische Innovation oder der Komplex von Verbesserungen ist, der zu geringeren Komplikationen führt. Insgesamt konnte übereinstimmend mit der Literatur ein generell schädigender Einfluss einer EKZ bestätigt werden. Von besonderem Vorteil zeigten sich alle fünf Vergleichsgruppen gegenüber dem Kollektiv der Standard- HLM beim Hämolyseparameter des freien Hb (p<0.001). Von den Neuerungen der minimierten Systeme stellten sich hierbei eine geringe Fremdoberfläche und eine Beschichtung mit Poly-2-methoxyethylacrylat (PMEA) als vorteilhaft heraus. Bei Betrachtung der Inflammationsparameter konnte sich kein minimiertes System deutlich gegenüber der Standard-HLM durchsetzen, auch wenn diese quantitativ gesehen über den gesamten Verlauf erhöhte Werte aufwies. Die Gruppe des OPCAB-Verfahrens zeigte sich dabei über den Gesamtverlauf als sehr stabil. Einzelne technische Verbesserungen bedingten für sich alleine keinen Vorteil bei den minimierten Systemen. Allerdings konnten für einzelne Komponenten wie geringe Fremdoberfläche oder auch Heparinbeschichtung Trends hinsichtlich einer Reduktion der inflammatorischen Antwort gesehen werden. Auch in Bezug auf die Gerinnungswerte wiesen einzelne minimierte Systeme keinen besonderen Vorteil auf, präsentierten sich aber über den gesamten Verlauf generell besser als die Standard-HLM. Auch die OPCAB-Gruppe zeigte Gerinnungsalterationen. Bei Betrachtung des Aufbaus der Systeme schienen erneut eine Beschichtung mit PMEA sowie eine reduzierte Fremdoberfläche positive Effekte generieren zu können. Zusammenfassend zeigte sich, dass durch eine geringe Fremdoberfläche sowie einer biokompatiblen Beschichtung die positiven Effekte am deutlichsten zu Tage traten. Die Art der Beschichtung scheint dabei eher zweitrangig zu sein. Allerdings müssen auch die anderen Innovationen der minimierten Systeme berücksichtigt werden, da die positiven Effekte der minimal-invasiven Systeme letztlich als Summe der Einzelfaktoren gewertet werden können.
The extracorporeal circulation (ECC) is an important element of the modern cardiac surgery. Also it leads to increased inflammation and hemolysis as well as to disorder in the coagulation and fibrinolysis system. In this paper two ways of modern cardiac surgery were compared with the conventional standard- ECC. Therefore four minimized ECCs and one off-pump-coronary-artery-bypass- System (OPCAB) were evaluated. The effect of extracorporeal circulation was measured by parameters of hemolysis, inflammation and coagulation/fibrinolysis. The innovations of the minimized ECCs are: reduced artificial surface, biocompatible surfaces by coating, no blood-air-contact, no reinfusion of cells, parts of cells and mediators. The aim of the study was to evaluate the effect of the different innovations and to determine, if it is one single innovation or the complex of them which is leading to decreased complications. All in all a general disturbed influence of ECC could be confirmed in accordance to the literature. With regard to the hemolysis (free Hb) all minimized ECCs and the OPCAB group showed an important advantage versus the standard-ECC (p<0.001). The innovations with the most important influence at this juncture were: reduced artificial surface; coating with Poly-2-methoxyethylacrylat (PMEA). The evaluation of the inflammation showed no important advantage of the minimized ECCs versus the standard-ECC, although the standard-ECC had higher values in progress. The OPCAB group had a constant course again. In this case no single innovation demonstrated an important advantage, but a reduced artificial surface and a Heparin-coating showed a reduction of the inflammation. The evaluation of the coagulation and fibrinolysis showed no important advantage of the minimized ECCs versus the standard-ECC, but about the time they were better than the standard-ECC. The OPCAB-Method had alterations, too. At this juncture a coating with PMEA and a reduced artificial surface showed positive effects in terms of reduced complications. In summary, a reduced artificial surface and different kind of coating showed the best effects. But other technical innovations are important too, because the positive effects of the minimized ECCs can be evaluated as an accumulation of all single innovations.
