Auch im 21. Jahrhundert stellt die Tuberkulose eine ernsthafte Bedrohung der Gesundheit der Weltbevölkerung dar. Obwohl die Problematik der Tuberkulose in den Entwicklungsländern weitaus größer ist als in Westeuropa, bringt die zunehmende Globalisierung die Gefährdung durch die Tuberkulose auch vor unsere Türen. Zwar existieren neue, hochwertige Testverfahren zur Diagnostik der Tuberkulose, diese beruhen auf Messungen der T-Zell-Antwort auf die Exposition mit Antigenuntereinheiten des Tuberkuloseerregers wie z.B. ESAT-6 oder CFP-10. Weiterhin besteht jedoch die Problematik der unzureichenden Differenzierung zwischen latenter und akuter Tuberkulose im klinischen Alltag. Mit den in dieser Arbeit vorgestellten Methoden soll anhand spezifischer T-Zell- Charakterisierungen diese Problematik entscheidend verbessert werden. CD4-T-Zellen von Patienten und Kontrollprobanden wurden zum einen mit Tuberkulose Antigen (ESAT-6) und zum anderen mit PPD (Purified Protein Derivate), einem für den Mendel-Mantoux-Hauttest gereinigtem Proteingemisch, stimuliert und anschließend nach Vorhandensein spezifischer Oberflächenmarker für naive bzw. Gedächtnis-T-Zellen mittels Durchflusszytometrie charakterisiert. Die Immunantwort der T-Zellen auf die Tuberkulose Antigene wurde durch Nachweis von Interferon-Gamma überprüft. Der kostimulatorische Marker CD27 zeigte sich hier als entscheidend. CD27 wird im Rahmen der Reifung von T-Zellen nach Antigenexposition herabreguliert, wenn die T-Zelle in das Stadium der Gedächtnis-T-Zelle übergeht. Es konnte gezeigt werden, dass gesunde, BCG geimpfte Probanden signifikant mehr CD27 positive CD4 T-Helferzellen aufweisen als Patienten mit akuter Tuberkulose. Des Weiteren konnten Patienten mit latenter Tuberkulose ebenfalls anhand von CD27 von akut infizierten Tuberkulosepatienten unterschieden werden. Sie zeigten eine vermehrte Anzahl CD27 positiver CD4 T-Helferzellen im peripheren Blut. Die Anzahl CD27 positiver CD4 T-Helferzellen bei den latent infizierten Patienten lag unterhalb der Zellzahl der gesunden, BCG geimpften Probanden. Mit Hilfe des kostimulatorischen T-Zell Markers CD27 ist es somit möglich, eine akute Tuberkuloseinfektion nachzuweisen. Es gelang zudem erstmals, eine Differenzierung zwischen latent und akut infizierten Tuberkulosepatienten anhand von T-Zell-Oberflächenmarkern des peripheren Blutes zu erreichen. Diese neue Methode der Tuberkulosediagnostik ist einfach anwendbar und stellt somit eine Alternative zu anderen klinischen Testverfahren dar. Ergänzend wurden typische Chemokinmuster einer akuten pulmonalen Tuberkuloseinfektion ermittelt. Patienten mit einer akuten pulmonalen Tuberkulose zeigten nach der Stimulation mit PPD und ESAT-6 einen signifikant höheren Anteil des Chemokins IP-10 in den Überständen als die gesunden Kontrollen. Auch MIP-1α zeigte bei Patienten mit einer akuten pulmonalen Tuberkulose nach der Stimulation mit PPD einen höheren Anteil in den Überständen als die gesunden Kontrollen (nicht signifikant). Nach der Stimulation mit ESAT-6 konnten die Patienten mittels MIP-1α jedoch nicht von den Kontrollen differenziert werden. Die sichere Unterscheidung zwischen latent infizierten und akut an Tuberkulose Erkrankten ist für die Behandlung der Patienten und die Prävention von entscheidender Bedeutung und kann die Bekämpfung der Tuberkulose einen weiteren Schritt voranbringen.
Even in the 21st century, tuberculosis is a serious threat to public health. The problem of tuberculosis in developing countries is far greater than in Western Europe, but due to globalisation, tuberculosis has become an increasing danger to Western society. Although new, high-quality methods exist for the diagnosis of tuberculosis, they are based on measurements of T-cell responses following exposure to antigen subunits of the tuberculosis pathogen, for example ESAT-6 or CFP-10. However, the problem of insufficient differentiation between acute and latent tuberculosis in clinical practice continues. In this work, we present a specific T-cell characterisation method which could improve the problem of insufficient differentiation between acute and latent tuberculosis. CD4 T-cells from patients and control subjects were stimulated with the tuberculosis antigen ESAT-6 and with PPD (purified protein derivatives), which is a purified protein mixture used in the Mendel-Mantoux skin test. Then, CD4 T-cells were characterised by flow cytometry to evaluate the presence of specific surface markers for naïve or memory T-cells. The immune response of T-cells against tuberculosis antigens was assessed by the level of interferon- production. The costimulatory marker CD27 was shown to be crucial. CD27 is down-regulated during the process of T-cell maturation from naïve to a memory phenotype. It was shown that healthy, BCG-vaccinated subjects show significantly more CD27-positive CD4 helper T-cells than patients with acute tuberculosis. Furthermore, patients with latent tuberculosis infection could be differentiated from patients with acute infection by the CD27 surface marker. Patients with latent tuberculosis infection showed an increased number of CD27-positive CD4 T-helper cells in the peripheral blood. The number of CD27-positive CD4 T-helper cells in the latent infected patient group was lower than the number of CD27-positive CD4 T-helper cells in the healthy BCG vaccinated group. By assessing expression of the costimulatory T-cell marker CD27, it is possible to detect an acute tuberculosis infection. Therefore, it was demonstrated for the first time that the differentiation between latent and acute tuberculosis infection is possible using surface markers on T-cells in the peripheral blood. This new method of tuberculosis diagnostics is easy to perform and represents an alternative to other clinical tests. In addition, typical chemokine indices of acute pulmonary tuberculosis infection were identified. Patients with acute pulmonary tuberculosis showed, after stimulation with PPD and ESAT-6, a significantly higher amount of the chemokine IP-10 in cell culture supernatants compared with healthy controls. Patients with acute pulmonary tuberculosis also showed a higher amount of MIP-1α in the supernatants compared with healthy controls (although this difference was not significant) after stimulation with PPD. After stimulation with ESAT-6, patients could not be differentiated from the control group using MIP-1α. For the treatment and prevention of tuberculosis, an accurate distinction between acute and latent tuberculosis infection is of critical importance. Using the results of this study, the diagnosis of tuberculosis infection can be improved to lend support to the global fight against tuberculosis.
