Hintergrund und Ziele: In der Klinik für Unfall– und Orthopädische Chirurgie
des Klinikums der Stadt Nürnberg wurde im Jahr 2000 erstmalig die lange
Version des Targon PH – Nagels (Aesculap) zur antegraden Versorgung von
proximalen Humerusfrakturen verwendet. Operationsindikationen waren
Schaftfrakturen mit Ausläufern in den Kopf, Humeruskopffrakturen mit
Schaftkomponente in den proximalen Humerus und kombinierte Frakturen von Kopf-
und Schaft. Probleme und Komplikationen, die mit dem Implantat auftraten,
wurden erfasst. Patienten, die nach einem Jahr postoperativ zur Verfügung
standen, wurden mit Hilfe des Constant-Scores nachuntersucht. Methoden:
Insgesamt wurden 92 Patienten von 01.01.2000 bis 01.01.2005 am proximalen
Humerus mit einem langen Targon PH versorgt. 19 Patienten mit einer
pathologischen Fraktur und 6 Patienten mit reinen Kopffraktur sind nicht in
die Untersuchungsgruppe mit eingeflossen. Die klinische Nachuntersuchung
erfolgte bei 30 (45%) von 67 Patienten anhand des Constant–Scores. Die Daten
wurden mit dem Microsoft – Access Programm in einer Datenbank erfasst und mit
IBM SPSS Statistics Version 19 analysiert. Alle Patientengeschichten wurden
dokumentiert. Sämtliche Röntgenbilder und Fotographien wurden digital
archiviert. Ergebnisse und Beobachtungen: Unter den 67 Patienten des
Untersuchungskollektivs betrug das Durchschnittsalter 69,7 Jahre (20 – 93). Es
wurden 46 Frauen und 21 Männer behandelt. Das Geschlechtsverhältnis liegt
demnach bei 2:1. Die durchschnittliche Krankenhausaufenthaltdauer betrug 17
Tage. Durchschnittlich betrug die OP Zeit 90,6 (33 – 230) min. 10 Patienten
mussten sich aufgrund mechanischer Komplikationen einer Re-operation
unterziehen. Eine Patientin mit präoperativ bekannter Schultersteife und
Rotatorenmaschettenläsion erhielt therapeutisch zunächst zur Konsolidierung
der Fraktur nach einem Sturz einen langen Targon PH Nagel. Im zweiten
operativen Schritt nach Konsolidierung ein inverse Prothese. 30 Patienten
konnten nachuntersucht werden. In Anwendung des Constant–Score hatten nach
Punkten 11 Patienten ein exzellentes, 9 Patienten ein gutes und 6 Patienten
ein befriedigendes Ergebnis. Bei 4 Patienten fiel der Constant – Score jedoch
schlecht aus, 2 davon benötigten eine Reoperation. Bei keinen der
nachuntersuchten 30 Patienten trat radiologisch eine Humeruskopfnekrose oder
ein Nagelbruch auf. Bei allen 30 Patienten kam es zur knöchernen Durchbauung
der Fraktur. Praktische Schlussfolgerungen: Das Konzept der winkelstabilen,
antegraden Marknagelung mit dem Targon PH mit seinem winkel-, gleit- und
rotationsstabilen Stellschraubenprinzip verspricht eine hohe Primärstabilität.
Frühfunktionelle Mobilisation ermöglichen einen frühzeitigen postoperativen
Therapieerfolg. Der bewährte Deltasplitzugang verursacht keine Traumatisierung
des Frakturbereichs. Der kleine limitierte Zugangsweg ermöglicht unter
Verwendung des TMS – Verfahrens dennoch eine anatomische Reposition unter
Wahrung der vaskulären Versorgung des Humeruskopfes. Durch das Besetzen aller
proximalen Verriegelungsschrauben wird das Humeruskopffragment mit hoher
Stabilität abgestützt, selbst wenn bei osteoporotischen Knochen nur eine sehr
dünne subkortikale Spongiosa zur Verfügung steht. Mit der Langversion des
Targon PH ist erstmals eine intramedulläre Fixationsoption für langstreckige
Pathologien am Humerus gegeben, die Minimalinvasivität mit einer
winkelstabilen dreidimensionale Fixation des Humeruskopfes vereint. Daraus
ergeben sich die Indikationen für den langen Targon PH für pathologische
Frakturen des gesamten Humerusschaftes, Kombinationen von Humeruskopf- und
Humerusschaftfrakturen, Frakturen des proximalen Schaftdrittels bzw.
langstreckige metaphysären Frakturen im Sinne einer „intermuskulären“ Fraktur
sowie auch der distale Randfrakturen nach proximaler Standardnagelung. Somit
wurde in dieser Arbeit, die klinische Relevanz eines möglichen
Indikationsspektrums überprüft. Im Vergleich zu anderen Implantatsystemen
zeigt sich, dass es sich hierbei um eine schonende und sichere Methode
handelt, mit einem geringen Risiko bezüglich der Entstehung von
Humeruskopfnekrosen oder Pseudarthrosen im Schaftbereich. Somit konnte die
Nullhypothese widerlegt und die Alternativhypothese bestätigt werden. Der
lange Targon PH Nagel ist bei speziellen Indikationsstellungen, aufgrund
seiner Eigenschaften eine Therapiealternative zu anderen
Osteosyntheseverfahren.

Background and Aims of the study: The long version of Targon PH (Aesculap) has
been used in the department of trauma and orthopaedic surgery in the general
hospital of Nuremberg for antegrade fixation for proximal fractures of the
humerus since the year 2000. Operation indications were fractures of the shaft
with splices into the humeral head, humeral head fractures with shaft
components, combination fractures of the humeral head and the humeral shaft.
Problems and complications with the implant were documented. Available
patients had a clinical postoperative follow up examination after one year by
using the constant-score. Methods: Altogether 92 patients from 01.01.2000 to
01.01.2005 got the long version of Targon PH at the proximal humerus. 19
patients with pathological fractures and 6 patients with isolated humeral head
fractures were dropped out of the study group. 30 (45%) patients from 67 could
be followed up by constant score. All data were registered with Microsoft –
Access program and analysed by IBM SPSS Statistic Version 19. All histories of
the patients were documented. All x-rays and photos were digital archived.
Results and Observations: The average age of the 67 patients in the study
group was 69.7 years (20 - 93). 46 were female and 21 male. The sex ratio was
2:1. The average duration of hospitation was 17 days. The average operation
time was 90.6 (33 – 230) minutes. 10 patients with mechanical complication of
the implant had to undergo a re-operation. One patient with a preoperative
known adhesiolysis of the shoulder and a lesion of the rotator cuff was
implanted the long version of Targon PH to fix the humeral fracture. After
consolidation of the fracture in a second step, the patient was treated with
implant removal and secondary arthroplasty of the shoulder. By using the
constant - score, the result was scored as excellent in 11 patients, good in 9
patients and moderate in 6 patients. At less, 4 patients had an unsatisfied
constant – score, 2 of these had to be operated again. There was no humeral
head necrosis and no broken implants in 30 followed up patients. All fractures
were consolidated in 30 patients. Practical Conclusion: The concept of an
angle-stabled, intramedullary nailing with Targon PH with angle-, glide-, and
rotation-stable fixation screws promised a very high primary stability of the
fracture. Early functional mobilisations of the patients make early
postoperative success possible. The proved delta split approach doesn’t make
damages in the fracture area. The small approach with transmedullary support
of the TMS scews makes anatomical reduction possible, without disturbing
vascular support of the humeral head. All proximal locking options can be
exploited for stabilisation fragments of the humeral head, even by
osteoporotic bones with small subcortical spongiosa. The long version of
Targon PH is the first intramedullary implant for stabilisation all
pathological diseases at the humerus in association with minimal invasive
action and three-dimensional angle-stable fixation of the humeral head. The
indication are proved for all pathological fractures at the humerus,
combination of fractures at the humeral head an the humeral shaft, unstable
proximal humeral fracture and fractures at the metaphyseal junction called
“intermuscular“ fracture and also for distal fractures after nailing with the
standard short version of Targon PH. The clinical relevance of an indication
spectrum is proved in this study. Compared to other implant systems, this is a
soft and safe method, with small risk for humeral head necrosis and
pseudarthrosis at humeral shaft. The null-hypothesis is disproved and the
alternative-hypothesis is proved. In special indications the long version of
Targon PH with it properties can be an alterative implant compared to other
implant systems.
