Hintergrund: Nekrotisierende Fasziitis (NF) ist eine seltene aber potenziell
tödliche Erkrankung der Weichteile. In den letzten Jahren war ein Wandel in
Bezug auf die Patientencharakteristika und das Keimspektrum zu verzeichnen.
Das Ziel der vorliegenden Studie war die Identifizierung der Charakteristika
und Risikofaktoren der NF Patienten, die Bestimmung des Keimspektrums und die
Bestimmung von unterschiedlichen Prognosefaktoren bei überlebenden und nicht
überlebenden Patienten. Materialien und Methodik: Die Daten von allen
Patienten mit NF, die ab 1. Januar 2005 bis 31. Dezember 2010 in dem Centrum
für Muskuloskeletale Chirurgie, Campus Virchow Klinikum, Charité
Universitätsmedizin behandelt wurden, wurden retrospektiv analysiert.
Ergebnisse: Die Mortalität von den 24 behandelten Patienten betrug 20,8%. Der
Mittelwert von Komorbiditäten war 3,6±3,5 pro Patient. Diabetes mellitus,
kardiovaskuläre Erkrankung (KVE) und Immunsuppression waren die am häufigsten
einhergehenden Begleiterkrankungen. Die Mehrheit der Infektionen (54,2%;
13/24) waren Monoinfektionen. Hämolytischer Streptokokkus der Gruppe A (25%)
und Methicillin - resistent Staphylococcus aureus (MRSA) (20,8%) waren die am
häufigsten vorkommenden Keime. Durchschnittlich wurden 8,1± 4,7 Operationen
pro Patient durchgeführt. In 8 Patienten (33,3%) wurde ein systemisches
inflammatorisches Response – Syndrom (SIRS) diagnostiziert, in 5 Patienten
(20,8%) wurde der klinische Verlauf durch eine disseminierte intravasale
Gerinnung (DIC) kompliziert. Alle Patienten, die eine DIC entwickelten,
starben. Im Vergleich zu den überlebenden bestand bei allen nicht überlebenden
Patienten initial eine septisch induzierte Koagulopathie, die sich im Verlauf
zu einer DIC entwickelte (p<0,001). Ferner wurde bei den nicht überlebenden
Patienten initial nur eine empirische antibiotische Monotherapie mit
Ampicillin und Sulbaktam eingesetzt (p<0,001). Schlussfolgerung: Die
aggressive chirurgische Versorgung mit einhergehender optimierter
antibiotischer Therapie mittels Doppelschema (Breitspektrum-Beta-Laktam-
Antibiotikum und Clindamycin) sowie das Fehlen einer initialen Koagulopathie /
DIC scheinen die entscheidenden Prädiktoren für reduzierte Mortalität zu sein.
Die Berücksichtigung anderer in dieser Studie identifizierter Prognosefaktoren
wie die initiale antibiotische Monotherapie, die Beteiligung von Stamm,
Thoraxwand und den proximalen Segmenten der Extremitäten und die zunehmende
Zahl von Komorbiditäten sowie eine bestehende Immunsuppression könnte in
aktuellen Therapieschemata der NF eine mehr auf das individuelle
Patientenprofil zugeschnittene Therapieoptimierung und Outcomeverbesserung
ermöglichen und damit einer weiteren Eskalation der ansonsten schwerwiegenden
Erkrankung aktiv entgegenwirken.

Introduction: Necrotising Fasciitis (NF) is a rare but potentially lethal
disease of the soft tissues. In the last years a change in terms of patient
characteristics and bacterial spectrum has been noted. The aim of the present
study was the identification of the characteristics and risk factors of
patients with NF, the definition of the bacterial spectrum and the
identification of different prognostic factors between survivors and non-
survivors. Materials and Methods: Data from all patients with NF that were
treated in the Center for Musculoskeletal Surgery, Campus Virchow Klinikum,
Charité – Universitätsmedizin Berlin between 1st January 2005 and 31st
December 2010 were included in the study and analyzed. Results: The overall
mortality of the 24 patients was 20.8%. Mean number of comorbidities/ patient
was 3.6±3.5. Diabetes mellitus, cardiovascular disease and immunosuppression
were the most common ones. The majority of the infections (54.2%; 13/24) were
mono-infections. Haemolytic Streptococcus of the group A (25%) and
methicillin-resistant Staphylococcus aureus (MRSA) (20.8%) were the most
common bacteria. The mean number of operations/ patient was 8.2±4.7. In 8
patients (33.3%) a systematic inflammatory response syndrome (SIRS) was
diagnosed; in 5 patients the situation was complicated by a disseminated
intravasal coagulation (DIC). Compared to the survivor group, coagulopathy was
present initially in all non-survivor patients (p<0.001). The patients in the
non-survivor group received more often an antibiotic monotherapy initially
compared to the survivor group (p<0.001). Conclusions: The aggressive surgical
treatment with optimization of the antibiotic therapy with a double-schema
(broad-spectrum beta-lactame-antibiotics and clindamycin), as wells as the
absence of initial coagulopathy seem to be the most important factors for a
reduced mortality. The initial antibiotic monotherapy, the infectious insult
of the trunk, thorax and proximal segments of extremities, as well as, the
increasing number of comorbidities and the presence of immunsuppression should
be taken into account in order to achieve better results in the treatment of
the aggressive infection.
