Bei der vorliegenden Studie handelt es sich um eine Einpunkterhebung zur
Untersuchung des Zusammenhangs zwischen Neuroimmunologie und Psychologie bei
Atopischer Dermatitis im Vergleich zu einer gesunden Kontrollgruppe. Es wurden
Cortisol, SP, BDNF, NGF und die Zytokine IL-4, IL-5, IL-10, IFN-γ und TNF-α
untersucht. Auf psychologischer Ebene wurden mittels Fragebögen die Parameter
chronischer Stress mittels der „Impact of Event Scale“, der subakute Stress
mittels des „Percieved Stress Questionnaires“ und der akute Stress des
täglichen Lebens mittels der „Daily Hassel and Uplift Scale“ ermittelt. Das
Copingverhalten der Probanden wurde mit dem COPE-Fragebogen und die
Lebensqualität der Atopiker mittels des dermatologischen
Lebensqualitätsindexes gemessen. Angst und Depression wurden mittels der
„Hospital Anxiety and Depression Scale“ erfasst. Ferner wurden zur genaueren
Beurteilung der Ergebnisse die Atopiker nach Schweregrad der Krankheit in
leicht, mittelschwer und schwer betroffene Patienten gegliedert. Es kann
festgestellt werden, dass Atopiker im Vergleich zu Hautgesunden signifikant
höhere BDNF-Werte aufweisen, wobei sich signifikante Unterschiede zwischen
vormittags und nachmittags zeigen, die bisher noch nie beschrieben worden
sind. BDNF steigt proportional zur Schwere der AD. Im Gegensatz dazu zeigt
sich mit Zunahme der Krankheitsschwere eine Abnahme des Cortisolspiegels.
Besonders bei akuter AD sind die Werte erniedrigt. Es kann von einer gestörten
HPA-Achse ausgegangen werden. Es zeigen sich signifikant höhere IFN-γ-Werte
bei Atopikern, wobei festgestellt werden muss, dass der Großteil der Patienten
an mittelschwerer AD leidet, bei welcher die Werte am höchsten nachgewiesen
worden sind. Ferner wird festgestellt, dass die Hypothese eines möglichen
Einflusses von SP und NGF auf den TH1/TH2 Switch bestätigt werden kann, da
besonders bei mittelschwerer, chronischer AD die beiden Parameter in engem
Zusammenhang mit den Zytokinen und den psychologischen Parametern stehen.
Zusammenfassend kann festgestellt werden, dass bei Atopikern eine enge
Verbindung zwischen den verschieden Stressformen und der psychischen
Gesundheit besteht und dass daher in der Praxis nicht nur auf die klinischen
Symptome, sondern auch auf das psychologische Ungleichgewicht der Patienten
eingegangen werden muss. Interessanterweise zeigt sich, dass die Parameter
BDNF und Cortisol bei Atopikern in negativem Zusammenhang zu täglichem Stress
stehen, die gestörte HPA-Achse scheint mit einer gestörten Stressreaktion der
Patienten einherzugehen. Dies wird besonders durch die Tatsache unterstützt,
dass sie sich durch tageszeitliche Schwankungen morgens mit erhöhten und
nachmittags mit erniedrigten Werten darstellen. SP scheint trotz seiner
Stabilität während des Tages ebenfalls mit dem akuten Stress in enger
Verbindung zu stehen, wobei festgestellt werden muss, dass diese drei
Parameter bei positiven Ereignissen am Tag ansteigen und bei Distress sinken.
Im Falle von Cortisol geht das mit der Schwere der AD einher. NGF zeigt sich
als Parameter, der bei psychischer Labilität besonders niedrig ist, auch hier
hätte man erwartet, dass eine positive Korrelation zwischen Angst und NGF
nachweisbar ist und keine negative. Die Zytokine zeigen besonders bei
mittelschwerer AD eine enge Verbindung zu Depression, welche den engen
Zusammenhang zwischen psychischem Gleichgewicht und Hautzustand bei
chronischer Krankheit unterstützen. Bezüglich der psychologischen Parameter
ergibt sich, dass bei Atopikern eine Störung der internen Stresswahrnehmung
festgestellt werden kann, die in engem Zusammenhang mit Angst, Depression und
eingeschränkter Lebensqualität der Patienten steht. Die traumatischen
Lebensereignisse sind bei beiden Gruppen nicht unterschiedlich. Werden die
täglichen Stressoren untersucht, zeigt sich, dass Atopiker, die sich von
täglichen Stressoren beeinflussbar sind, vermehrte interne Labilität zeigen,
hohe Werte an BDNF sowie geringe an SP, NGF und Cortisol aufweisen. Dieses
Bild kann schlimmstenfalls mit einer schweren akuten Neurodermitis
einhergehen. Auf den ersten Blick zeigen sich keine wesentlichen Unterschiede
in der Stresswahrnehmung zwischen Kranken und Gesunden. Werden jedoch die
Patienten nach Schweregraden eingeteilt, ergibt sich, dass mittelschwer
betroffene Atopiker ein unterschiedliches psychologisches sowie
neuroimmunologisches Profil aufweisen: Die untersuchten Patienten mit
mittelschwerer AD zeigen sich in einer chronischen Phase der Krankheit, wobei
IFN-γ als zentrales Zytokin der TH1-Antwort erhöht vorliegt. Sie konzentrieren
sich sehr stark psychisch auf ihre Krankheit. Sie zeichnen sich als besorgt
aus, sie versuchen aktiv und passiv gegen ihre Krankheit vorzugehen. Die
Konzentration auf die Krankheit steht im Zentrum ihres Lebens, sie versuchen,
sie mit Vermeidung und Leugnen zu bekämpfen. Bei schwerer Atopie hingegen
zeigen die Patienten ein neuroimmunologisch akutes Profil, welches sich
psychisch durch eine Entfernung von der Krankheit zeigt, indem sie trotz
signifikant eingeschränkter Lebensqualität und Depression die aktive
Konzentration nicht auf die Krankheit richten, sondern sich in einer Art
Resignation befinden. Es sollte also bei der Therapie der Patienten je nach
Schweregrad eine unterschiedliche psychologische Betreuung zusätzlich zur
Parmakotherapie angewandt werden, wobei das neuroiummunologische Profil der
Patienten mit in die Therapie, nicht nur zur Durchbrechung des Itch-Scratch
cycle, miteinbezogen werden muss und in weiteren Studien daraufhin untersucht
werden muss.

The hereshown study examines the relationship between neuroimmunology and
psychology of patients with atopic dermatits and compares the results with
those of a group of healthy controls by using a cross-sectional study.
Cortisol, SP, NGF, BDNF and the cytokines IL-4, IL-5, IL-10, IFN-γ and TNF-α
have been analysed. On the psychological level various questionnaires were
applied to measure the stress, e.g. chronic stress via the „Impact of Event
Scale“, the subacute stress via the „Percieved Stress Questionnaires“ and the
acut stress of daily life via the „Daily Hassel and Uplift Scale“. The
probands’ coping behavior was studied using the COPE- questionnaire and the
quality of life using the „The Dermatology Life Quality Index“. Aniety and
depression were taken into account using the „Hospital Anxiety and Depression
Scale.“ In order to evaluate the results more precisely, the patients with
atopic dermatitis were subdivided, according to their disease’s severity into
the groups of mildly, moderately or severely affected patients. It can be
observed that the AD patients show significantly higher levels of BDNF
compared with the healthy probands. The results show significant differences
beetween am and pm, which have never been evidenced before. BDNF rises
proportionally to the disease’s severity. By contrast, the Cortisol levels
diminish with the increase of the illness’s severity. Especially regarding the
acut AD these scales are low. Thus an abnormal Hypothalamus- Pituary- Adrenal
Axis (HPA-axis) can be assumed. Significantly higher levels of IFN-γ can be
shown for AD patiens, whereas it is remarkable that the majority of the
patients suffer from moderate AD with its highest levels show. Furthermore the
hypothesis of a possible influence of SP and NGF on the TH1/TH2-switch can be
confirmed, for especially in the case of moderate, chronic AD both parameters
are closely linked with the cytokines and the psycological parameters. In
summary it can be stated that concerning AD patients a very close connection
between the various forms of stress and the psychic health exists and that in
practice not only the clinical symptoms but also the psychic imbalance of the
patients has to be taken into account. Interestingly it can be found that the
parameters BDNF and Cortisol concerning AD patients correlate negatively with
daily stress. The abnormal HPA-axis seems to go hand in hand with the abnormal
stress reaction of the patients. This is especially supported by the fact that
they show variations during the day with high levels in the morning and low
levels in the afternoon. Despite its stability during the day, SP seems to be
closely connected with acute stress, whereas it is remarkable that all these
parameters increase in the same way as positive daily uplifts and decrease in
the same way as daily hassels. In case of Cortisol this is a concomitant of
the disease`s severity. NGF is a parameter which is notably low regarding
patients liable to psychic illness, although one might have expected a
positive (and not a negative) correlation between anxiety and NGF. The
cytokines display a close link to depression, especially regarding moderate AD
which display the close connection between psychic equilibrium and the skin`s
condition in the case of chronic disease. Concerning the psychological
parameters it is concluded that patients with AD show abnormal internal stress
perception, which is closely related to anxiety, depression and restricted
quality of life. The traumatic life events do not differ amognst the groups.
Regarding the daily stressors, AD patients, who are affected by these daily
hassels, demonstrate augmented liability to psychic illness, high BDNF levels
as well as low SP, NGF, Cortisol levels. In the worst case this pattern can be
accompanied by a severe acute AD. At first sight there seem to be no evident
differences between patients and healthy subjects concerning the stress
perception. However subdividing the patients according to their disease`s
severity demonstrates that moderately affected patients show a different
psychological and neurological profil: The analysed patients with moderate AD
show a chronic phase of their disease, whith increased IFN-γ as the central
cytokin of the TH1-response. They focus very much on their disease in a
psychic way. They appear worried, trying to fight their disease actively and
passively. The concentration on their disease is placed in the centre of their
lives, they attempt to fight it with avoidance and denial. Severe AD patients
show a neuroimmunologically acute profil, which seems to be a retreat from the
disease. Despite their significantly restricted quality of life and depression
they do not focus their active concentration on the disease, but rather appear
to be in a state of resignation. To conclude it must be said that regarding
the patients’ therapy a different psychological approach of care should be
applied, depending on the disease’s severity, in addition to the
pharmacological therapy; this means including the neuroimmunological profil
into therapy not only using it to breake through the Itch-Scratch cycle. This
has to be investigated in further studies.
