Hämatopoetische Stammzellen (HSCs) lokalisieren in einer komplex-
zusammengesetzten Mikroumgebung, die essentiell für die HSC-Regulation ist.
Bei einer akuten myeloischen Leukämie (AML) resultiert aus einer klonalen
Transformation eine Expansion maligner Progenitorzellen, die das Knochenmark
infiltrieren und mit mesenchymalen Stromazellen (BM-MSC) der Mikroumgebung
interagieren. Zahlreiche Studien untersuchten Aberrationen in AML-Zellen,
während molekulare Alterationen in BM-MSC von AML-Patienten (AML BM-MSC) und
deren putativ pathophysiologische Rolle weitestgehend unergründet blieben. Um
molekulare Alteration der AML-Mikroumgebung zu untersuchen, wurden AML BM-MSC
zu verschiedenen Zeitpunkten des AML-Krankheitsverlaufs isoliert und in vitro
expandiert. AML BM-MSC wurden auf genetischer, transkriptioneller und
epigenetischer (DNA-Methylierung) Ebene mittels Hochdurchsatzplattformen
(Illumina und Affymetrix) untersucht und mit Kontrollen gesunder Spender (GS
BM-MSC) verglichen. Die Gesamtexomsequenzierung der AML BM-MSC von 21
Patienten zeigte ein unspezifisches Muster genetischer Alterationen. In AML
BM-MSC wurden, verglichen mit GS BM-MSC, mehr Mutationen identifiziert.
Hingegen trugen AML BM-MSC eine geringere Mutationslast als die
korrespondierenden hämatopoetischen Zellen. Die aberranten Genexpressions- und
DNA-Methylierungsmuster der AML BM-MSC zeigten eine signifikante Anreicherung
zellulärer Komponenten mit einer Implikation in der Stroma-Leukämie-
Interaktion: Chemokine, Zelladhäsionsmoleküle und Proteoglycane. Beide
molekulare Ebenen, die globale Genexpression und die DNA-Methylierung der AML
BM-MSC, spiegelten eine Deregulierung zellulärer Prozesse der Endozytose und
des Metabolismus wider. Die generierten Genexpressions- und DNA-
Methylierungssignaturen zeigten zudem deutlichere Aberrationen im Kontrast zu
GS BM-MSC als im Vergleich zwischen Verlaufsproben von Patienten in der AML-
Progression. Eine überexprimierte Komponente der AML-Mikroumgebung, welche aus
den transkriptionellen Profilen hervorging, war das Proteoglycan Lumican. Eine
stabile Überexpression von Lumican alterierte die Proliferation und
Morphologie der Stromazelllinie HS-5, hatte hingegen keinen Einfluss auf die
Apoptoserate in HS-5 Zellen. Weiterhin wurden der Immunophänotyp
(CD33-CD45-CD73+CD105+CD271+/-) sowie die Proliferationskapazitäten der frühen
Passagen der AML BM-MSC in vitro Kultur untersucht. Eine CD271+ Population war
hierbei hochvariabel (0-81 % der Gesamtpopulationen). AML BM-MSC zeigten
gegenüber GS BM-MSC keine signifikant verschiedenen in vitro-Expansionszeiten.
Die vorliegende Arbeit präsentiert eine umfassende Analyse genetischer,
transkriptioneller und epigenetischer Alterationen in AML BM-MSC. Die
aberranten molekularen Signaturen der AML BM-MSC zeichnen das Bild einer
global alterierten AML-Mikroumgebung und bieten eine Plattform für die
Identifizierung neuer putativer Zielmoleküle für nischenspezifische
Therapieansätze.

Hematopoeitic stem cells (HSCs) are sheltered in the bone marrow (BM)
microenvironment, a multicellular compartment containing mesenchymal stromal
cells (BM-MSC), which are essential for HSC repopulation and differentiation.
In acute myeloid leukemia (AML), a clonal transformation event gives rise to
the accumulation of malignant myeloid progenitors that infiltrate the bone
marrow cavity and interact with BM-MSC. Numerous studies have focused on
molecular aberrations of AML cells, while molecular alterations in BM-MSC
derived from AML patients (AML BM-MSC) and their putative pathophysiological
role in leukemogenesis remain elusive. In order to unravel molecular
alterations in the AML microenvironment, AML BM-MSC were isolated and expanded
in vitro from different time points of disease progression. In addition,
healthy donor controls (HD BM-MSC) were obtained. Both AML BM-MSC and HD BM-
MSC were molecularly profiled on genetic, transcriptional, and epigenetic (DNA
methylation) levels using Illumina and Affymetrix high throughput platforms.
Whole exome sequencing of AML BM-MSC of 21 patients revealed a nonspecific
mutational pattern and a higher frequency of genetic lesions in AML BM-MSC
than in HD BM-MSC. Moreover, AML BM-MSC exhibited less genetic lesions than
the hematopoietic counterparts (AML cells). Aberrant transcriptional patterns
of AML BM-MSC were enriched in putative stroma-leukemia interaction molecules:
chemokines, adhesion molecules and proteoglycans. Both molecular levels,
aberrant gene expression and DNA methylation, were enriched in genes
associated with endocytosis and metabolic pathways indicating an imbalance of
these processes in the AML microenvironment. Compared to patient-matched
serial AML BM-MSC samples in the course of the disease, AML BM-MSC showed the
most significant changes in contrast to HD BM-MSC. Proteoglycan Lumican, a
putative target with significant overexpression in AML BM-MSC transcriptional
profiles, was chosen for further investigations. Lumican overexpression
impaired the proliferation and altered the morphology of stromal cell line
HS-5 but did not influence the apoptotic rate in HS-5 cells. Furthermore, AML
BM-MSC were characterized with regard to immunophenotype
(CD33-CD45-CD73+CD105+CD271+/-) and to expansion capacities of the early in
vitro culture. A CD271+ population was highly variable (0-81 % of total cell
population). AML BM-MSC showed similar expansion times compared to HD BM-MSC.
The thesis provides a comprehensive analysis of genetic, transcriptional, and
DNA methylation alterations in AML BM-MSC. Furthermore, this approach provides
a platform for the identification of putative targets for future niche
directed therapies.
