Die Kombination klinisch angewendeter Dosierungen von Omeprazol (Ome) und
Amoxicillin (Amox) wurde zur Therapie der Helicobacter pylori-Infektion
verwendet und wies in unkontrollierten Studien hohe Eradikationsraten auf. Es
wurde angenommen, daß die antibakterielle Wirkung des Amox durch das von Ome
induzierte erhöhte intragrale pH gesteigert wird. Ziele dieser Studie waren,
die Bioverfügbarkeit von Amox sowie die pharmakokinetische und
pharmakodynamische Wechselwirkung von Ome und Amox zu untersuchen.
Die hier vorliegende Studie wurde als "three-way-cross-over"-Doppelblindstudie
konzipiert. Nach einer Basis-Langzeit-pH-Metrie unter Placebobehandlung wurden
24 H. pylori-positive gesunde Probanden in drei randomisierten Zyklen
behandelt: 2 x tgl. 750 mg Amox + Placebo, 2 x tgl. 750 mg Amox + 2 x tgl. 40
mg Ome und 2 x tgl. 40 mg Ome + Placebo, und zwar im Abstand von jeweils vier
Wochen ("wash-out"). Am fünften Tag jedes Therapiezyklus wurde eine
standardisierte intragastrale Langzeit-pH-Metrie und zeitlich definierte
Blutabnahmen zur Erstellung von Serumprofilen beider Medikamente synchron
durchgeführt.
Mit Hilfe des C13-Harnstoffatemtests und der H. pylori-Serologie konnte
nachgewiesen werden, daß der H. pylori-Status vor Beginn der Studie und am
Ende der drei "wash-out"-Phasen unverändert war. Die intragastralen pH-Profile
unter Monotherapie mit Placebo und mit Amox waren nahezu identisch, d. h. daß
Amox keinen säureinhibitorischen Effekt hatte. Der pH-Median während der
kombinierten Behandlung mit Ome und Amoxi (pH 5,0) war signifikant niedriger
gegenüber der Monotherapie mit Ome (pH 5,8; p< 0,01). Der Vergleich der
pharmakokinetischen Parameter Cmax, tmax und AUC aller Behandlungsgruppen
ergab keine signifikanten Unterschiede - abgesehen von der AUC von Ome am Tag
und in der Nacht (allein und in Kombination). Dies bedeutet u.a., daß die
Pharmakokinetik des Amox nicht abhängig war vom Grad der Säureinhibition.
Eine fünftägige Behandlung mit 2 x tgl. 40 mg Ome und 2 x tgl. 750 mg Amox
hatte keinen Eradikationseffekt auf H. pylori. Hohe Dosen von Ome veränderten
die Pharmakokinetik des Amox nicht, das selbst keinen Einfluss auf das
intragastrale pH hatte. Das im Vergleich zur Ome-Monotherapie signifikant
niedrigere intragastrale pH während der kombinierten Therapie könnte mit der
Suppression der Ureaseaktivität bzw. der Keimzahl von H. pylori
zusammenhängen. Insgesamt sind die Ergebnisse unserer Untersuchungen vereinbar
mit der Annahme, daß es keine pharmakokinetische und pharmakodynamische
Interaktion zwischen Ome und Amox in klinisch empfohlenen Dosierungen gibt.
Die signifikant verschiedene AUC von Ome unter Mono- und kombinierter Therapie
am Tag und in der Nacht bedarf weiterer Untersuchungen.

## Abstract
Background: Omeprazole (Ome) with amoxicillin (Amox) has been used to treat
Helicobacter pylori infection. It was speculated that omeprazole induced
hypoacidity enhances the antibacterial activity of Amox. Limited information
exists about intragatric pH and bioavailability of Amox during combination
therapy. No data are available about possible effects of the antibiotic on the
pharmacokinetics and pharmacodynamics of Ome. This study investigated the
pharmacokinetic and pharmacodynamic interaction of Amox and Ome.
Methods: The study was performed in a three-way cross-over double-blind
design. After a run-in-period on placebo with a baseline intragastric pH-
metry. 2 4 H. pylori-positive healthy subjects were randomly dosed with Amox
750 mg b.d. + placebo, Amox 750 mg b.d. + Ome 40 mg b.d. and Ome 40 mg b.d. +
placebo for 5 days. On the last day of each treatment period intragastric pH-
metrys were performed and blood samples taken for Ome and Amox serum profiles.
During the pH-metry-monitoring the subjects remained at the hospital and
received standardized meals. To proof the H. pylori-status during the intire
study C13-urea breath tests were performed before treatment and after the
wash-out periods of at least four weeks.
Results: Intragastric pH and area under the serum concentration time cure
(AUC) are expressed as estimated mean with corresponding s. d. and 95 % CL.

Amox monotherapy had no acid-inhibition effect. Median pH during combined
dosing was significantly lower, compared to Ome monotherapy p< 0,01. Mean
serum concentrations of Ome and Amox given alone or in combination were not
significantly altered.
Conclusion:
* A 5-days treatment with Ome plus Amox does not eradicate H. pylori under the conditions tested.
* High-dose omeprazole does not alter the pharmacokinetic of amoxicillin.
* The significantly lower intragastric pH during combination therapy might be due to the H. pylori-suppessive effect of this treatment.
* Our results do not support the assumption of pharmacokinetic or pharmacodynamic interaction between Amox and Ome at clinically recommended doses.
