EINLEITUNG Im Rahmen von inflammatorischen Prozessen im Anschluss an einen
akuten Myokardinfarkt kommt dem C-reaktiven Protein (CRP) als Modulator der
initialen Immunreaktion über das Komplementsystem eine wichtige Bedeutung zu.
Studien zeigten, dass nicht nur die Prognose der Patienten sondern auch die
Größe der Infarktnarbe, signifikant mit der peripheren CRP-Konzentration
assoziiert ist. Tierexperimentelle Arbeiten konnten bestätigen, dass eine
Blockierung von CRP geeignet ist, die Infarktnarbe signifikant zu reduzieren.
METHODIK UND ERGEBNISSE In Teilprojekt 1 der vorgelegten Arbeit wurde ein
porcines Infarktmodell zur spezifischen Depletion von CRP mittels
Plasmapherese etabliert (Publikation 1) und die Auswirkung der Behandlung auf
die mittels MRT ermittelte Infarktgröße, Funktionsparameter des Herzens und
die Narbenmorphologie untersucht (Publikation 2). Es konnte gezeigt werden,
dass zirkulierendes CRP mittels Bindung an sepharosegekoppeltes
Phosphorylcholin erfolgreich gesenkt werden konnte und dass diese Reduktion
sich im Vergleich zu einer mit Scheinapherese behandelten Gruppe vorteilhaft
auf die Größe der resultierenden Infarktnarbe auswirkt (Proof of Principle).
In Teilprojekt 2 der vorliegenden Arbeit wurde die CRP-Kinetik bei Patienten
mit hochgradigem Verdacht auf akuten Myokardinfarkt auf dem Notarztwagen und
in der Notaufnahme untersucht. Es zeigten sich diagnoseunabhängig erhöhte
Werte 12-36 Stunden nach Aufnahme der Patienten und das prognostische
Potential von CRP zur Vorhersage von kardiovaskulären Ereignissen konnte
bestätigt werden. SCHLUSSFOGERUNGEN Durch die erfolgten Experimente konnten
eine erfolgreiche CRP-Depletion, sowie eine vorteilhafte Auswirkung dieser auf
die Größe der resultierenden Infarktnarbe gezeigt werden. In weiteren Studien
sollte die demonstrierte Assoziation zwischen spezifischer CRP-Abreicherung
und der Größe der Infarktnarbe sowie der kardialen Funktion beim Menschen
bestätigt und an größeren Fallzahlen, insbesondere auch im Vergleich zu einer
nur nach Standardtherapie behandelten Gruppe, untersucht werden. Da ein CRP-
Anstieg erst spät nach Ischämiebeginn nachweisbar ist, sollten in klinischen
Studien geeignete Parameter für die frühzeitige Identifikation einer
vulnerablen Patientenpopulation, welche von einer Anwendung des entwickelten
Verfahrens profitieren könnten, evaluiert werden.

INTRODUCTION C-reactive protein (CRP), as a complement dependent mediator of
the initial immune response, has an important role in inflammatory processes
following acute myocardial infarction. Several studies have shown that CRP
does not only provide prognostic information but is also associated with
infarct size and cardiac function. In animal experiments in rats and rabbits
it was shown, that therapeutic reduction of CRP-levels seems to influence the
size of a developing scar after myocardial infarction as well as functional
cardiac parameters. METHODS AND RESULTS In the first part of this work a
porcine infarction model for the specific depletion of CRP was developed. CRP
was depleted via plasmapheresis in a specific adsorber containing sepharose-
coupled phosphorylcholine (publication 1). It was shown that the procedure was
eligible to significantly reduce circulating CRP-levels and that it had a
favorable effect on infarct size, as assessed by cardio-MRI, and cardiac
function as compared to a sham-treated control group (publication 2). In the
second part, the temporal release pattern of CRP was investigated in patients
with a high suspicion of acute myocardial infarction on the ambulance and in
the Emergency Department. CRP-values were elevated after 12-36 hours and the
prognostic potential of CRP for the prediction of cardiovascular events was
confirmed. CONCLUSIONS We were able to provide the proof of principle that CRP
was successfully reduced by selective plasmapheresis. The developed
therapeutic measure was also shown to favorably influence infarct size and
cardiac function. Following studies should investigate the therapeutic
potential of CRP to confirm the association between specific CRP-depletion and
infarct size as well as cardiac function in humans and should involve more
individuals and also a comparison to a standard-care group without sham
procedure. As CRP rises late after onset of ischemia it might be helpful to
evaluate other parameters for the preselection of a vulnerable patient
population that could profit from specific CRP-reduction.
