Der Coxsackievirus- und Adenovirus-Rezeptor (CAR) ist ein multifunktionelles
Protein, das in den tight junctions von Epithelzellen und den Glanzstreifen
des Herzens lokalisiert ist. Über seine extrazelluläre Domäne vermittelt es
die Bindung und Internalisierung von Coxsackie- und Adenoviren, aber auch die
Ausbildung von Zellkontakten durch homophile und heterophile Interaktionen.
Durch den zytoplasmatischen Teil steht CAR mit intrazellulären Signalkaskaden
und dem Zytoskelett in Verbindung. Die Expression von CAR ist im embryonalen
Herzen und bei kardialen Erkrankungen stark erhöht. Dies lässt auf eine Rolle
von CAR in Umstrukturierungsprozessen des Herzens schließen. Die Funktion von
CAR in der Embryonalentwicklung und in Erkrankungen des Herzens sollten in
dieser Arbeit untersucht werden. Die Expression von CAR allein im Herzen
reicht aus, um den embryonal letalen CAR KO Phänotyp zu umgehen. Dies belegt,
dass CAR im Herzen, im Gegensatz zu anderen Organen, essentiell ist. Die
zelluläre Funktion von CAR im Herzen bleibt über Speziesgrenzen hinweg
erhalten, da CAR aus dem Huhn das der Maus ersetzen kann. In der
Pathophysiologie viraler Erkrankungen vermittelt CAR aus dem Huhn jedoch keine
Coxsackievirus Infektion. Bei einem experimentell induzierten Myokardinfarkt
erhöht die Deletion von CAR signifikant die Überlebenschancen. Es kommt zu
kleineren Infarkten - vermutlich durch eine veränderte Expression von
Connexinen und eine daduch reduzierte Ausbreitung der zellulären Letalität. Im
Gegensatz zum Wildtyp bleibt im KO Herzen die Pumpfunktion erhalten und es
entwickelt sich keine kompensatorische Hypertrophie. Die Induktion der CAR
Expression bei kardialen Erkrankungen ist demnach Teil des Pathomechanismus
und nicht des Heilungsprozesses. Die Ergebnisse dieser Arbeit belegen, dass
die Expression von CAR für die Entwicklung und Funktion des Herzens von großer
Bedeutung ist. Auf den Verlauf von Herzerkrankungen wirkt CAR dagegen negativ.
In anderen Geweben scheint CAR eine untergeordnete Rolle zu spielen.
Entsprechend bietet CAR einen Angriffspunkt für mögliche Therapieansätze zur
Behandlung von inflammatorischen, ischämischen und dilatativen
Herzerkrankungen, die es in Zukunft zu entwickeln gilt.

dc.description.abstract
The Coxsackievirus- and Adenovirus-Receptor (CAR) is a multifunctional protein
that localizes to the tight junctions of epithelial cells and to the
intercalated discs between cardiomyocytes. Via its extracellular domain CAR
mediates the binding and internalization of Coxsackie- and Adenoviruses as
well as homophilic and heterophilic interactions with other cells. The
cytoplasmic tail links CAR to intracellular signaling cascades and the
cytoskeleton. CAR is highly expressed in the developing and diseased heart.
This suggests a role of CAR in cardiac remodeling. This study adresses the
function of CAR in embryonic development and cardiac diseases. Expression of
CAR exclusively in the heart is sufficient to rescue the otherwise embryonic
lethal CAR-KO phenotype. This indicates an essential role for CAR in the
developing heart, but not in other organs. There were no species-specific
differences in the cellular function of mouse versus chicken CAR with normal
cardiac function of the rescue animals. Nevertheless, chicken CAR does not
mediate virus entry so that rescue hearts are protected from myocarditis. In
an experimental model of myocardial infarction, deletion of CAR improves
survival. This likely relates to the reduced spread of injury secondary to a
differential expression of connexins. Unlike WT animals, the KO maintains
proper cardiac function and does not become hypertrophic. Hence, the up-
regulation of CAR in the diseased heart contributes to the pathomechanism and
does not support the healing process. The data presented here suggests an
important role of CAR in cardiac development. This is contrary to the negative
impact of CAR on the progression of cardiac diseases. In other tissues CAR
seems to be less relevant. Thus, CAR is a suitable therapeutic target towards
the improved future treatment of cardiac inflammation, ischemia and dilated
cardiomyopathy.
