Methodik: 22 Berliner Frauenkliniken waren Gegenstand der epidemiologischen Untersuchungen. Es erfolgte eine retrospektive Analyse der OP-Bücher, OP- Berichte und Krankenakten. Insgesamt wurden 36808 Patientinnen in Berlin im Jahr 2001 gynäkologisch operiert. 11057 Patientinnen von Ihnen erhielten eine Laparoskopie und/oder Laparotomie. Bei 908 dieser Patientinnen wurde Endometriose diagnostiziert. Ergebnisse: Die Prävalenz, unabhängig von der Operationsindikation, betrug in Berlin im Jahr 2001 8,2%. Bei 6,7% aller in Berlin im Jahr 2001 per laparoskopiam bzw. per laparotomiam operierten Patientinnen wurde die Erkrankung erstmalig diagnostiziert. 41,8% der Patientinnen hatten Blutungsstörungen. Hypermenorrhoen (19,9%), Menorrhagien (17,9%), Zwischenblutungen (6,5%) und Polymenorrhoe (5,4%) wurden am häufigsten angegeben. Die häufigsten Schmerzsymptome waren Dysmenorrhoen (91,2%), chronische Unterbauchschmerzen (Chronic Pelvic Pain) (44,3%) und Dyspareunien (13,4%). 17,5% der untersuchten Patientinnen litten unter einer Sterilität. 67,3% der operierten Endometriosepatientinnen hatten oberflächliche peritoneale Endometrioseherde. 10,9% aller Patientinnen litten unter ausgedehnten, überwiegend tief infiltrierenden Endometrioseherden. 3% aller Patientinnen hatten eine rektovaginale Endometriose. Bei 55,2% der untersuchten Patientinnen wurden ovarielle Endometriosezysten beschrieben. Die häufigsten Lokalisationen der Endometriose waren die Ovarien (63,5%), der Douglas (33,8%), das vesicouterine Peritoneum (32,7%) und die Sakrouterinligamente (19,3%). 27,1% der oberflächlichen Endometrioseläsionen wurden hinsichtlich farblicher Ausprägung bzw. Wachstumsmuster beschrieben. Bei nur 89% aller Patientinnen wurde eine Histologie aus endometrioseverdächtigen Läsionen entnommen. Insgesamt hatten 81,7% aller Patientinnen eine histologisch verifizierte Endometriose. Die durchschnittliche Gesamtpunktzahl nach der rASRM-Klassifikation betrug 20,5. 35,8% waren im Stadium I, 9,5% im Stadium II, 42,2% im Stadium III und 12,5% im Stadium IV erkrankt. Schlussfolgerung: Die Endometriose ist eine der häufigsten gynäkologischen Erkrankungen der Frauen im reproduktionsfähigen Alter. Frauen mit einem höheren Bildungsgrad scheinen häufig an Endometriose zu erkranken. Der Anteil an Patientinnen mit Nikotinabusus ist unter den Endometriosepatientinnen höher als in der „Normalbevölkerung“. Allergikerinnen scheinen kein erhöhtes Risiko zu haben, an Endometriose zu erkranken. Das häufigste Symptom der Endometriose ist die Dysmenorrhoe. Endometriose zeigt ein bevorzugtes linksseitiges Vorkommen. Um einheitliche Therapiestandards im Rahmen der evidenzbasierten Medizin zu erarbeiten, müssen die Anamnese sowie das diagnostische und therapeutische Procedere besser dokumentiert werden. Dass wichtige Bereiche der Anamnese in vielen Fällen nur mangelhaft eruiert wurden und dass das diagnostische und therapeutische Vorgehen zum Teil nur eingeschränkt zu reproduzieren war bzw. mangelhaft durchgeführt wurde, ist eine beunruhigende Erkenntnis. Um jedoch optimal diagnostizieren und behandeln zu können, sind eine umfangreiche und auf Vollständigkeit bedachte Anamnese sowie eine möglichst leitliniengerechte Diagnostik und Therapie mit anschließender gründlicher Dokumentation Grundvoraussetzung. Hier gilt es in der Zukunft bessere Qualitätsstandards für das Bundesland Berlin zu erarbeiten.
Methods: 22 Berlin departments of gynecology (excluding MEO-Clinic, outpatients’ departments) were subject of the study. It was a retrospective analysis of the operation-books, operation-reports and patient-files. All in all 36808 patients had a gynecologic operation in Berlin in 2001. 11057 women of them were operated by laparoscopy and/or laparatomy. 908 patients had a laparoscopically or laparotomically confirmed endometriosis. Results: Independet from indication of the surgery the prevalence in Berlin in 2001 was 8,2%. 6,7% of all patients who had a laparoscopy and/or a laparotomy in Berlin in 2001 had an endometriosis diagnosed for the first time. The prevalence of postmenopausal endometriosis was 0,24%. On average the patients were 36 years old. On average the Body-Mass-Index was 22,9. 33,8% were academics. 49,6% had allergies. 18,5% had a pollen-allergy. 42,4% had nicotine abuse. Only 79,5% of all patients had a complete statement about their menstruation cycle. 41,8% had menstruation disorder. Heavy menstrual flow (19,9%), long duration of flow (17,9%), intermenstrual bleeding (6,5%) and a shorter cycle length (5,4%) were most frequent. A complete statement about pain had only 94,6%. Most frequent were dysmenorrhea (91,2%), chronic pelvic pain (44,3%) and dyspareunia (13,4%). The infertility rate was 17,5% (primary infertile 12,5% and secundary infertile 5%). 82,4% of the patients were operated by laparoscopy. The most frequent cause for surgery were adnexal masses (61,5%) and pelvic pain (57%). 67,3% had peritoneal endometriosis. 10.9% had deep infiltrating endometriosis. 3% had rectovaginal endometriosis. 55,2% had ovarian endometriomas. They were most frequent on the left side (58,1%) than on the right side. The most frequent localisations were ovaries (63,5%), douglas (33,8), vesicouterine peritoneum (32,7%) and sacrouterinal ligaments (19,3%). Only 27,1% of the peritoneal implants were described by color and/or shape of growth. Only in 89% of the cases a biopsy of the lesions were made. 81,7% had a histologic verification of endometriosis. On average the rASRM-Score was 20,5. 35,8% had stage I, 9,5% stage II, 42,2% stage III and 12,5% stage IV. On average the women were 6,26 days in hospital. Conclusions: Endometriosis is one of the most frequent gynecologic diseases of women in the reproductive age. The postmenopausal appearance of endometriosis is rare. Women with a higher education level more frequently fall ill with endometriosis. It seems to be that women with a nicotine abuse had a higher risk to fall ill with endometriosis. Patients with allergies have no higher risk to fall ill with endometriosis. The most frequent symptom of endometriosis is dysmenorrhea. The ’gold standard’ to diagnose an endometriosis is the laparoscopy. Endometriosis is more frequent on the left side. To create uniform therapy standards within the scope of evidence based medicine, the anamnesis, the diagnostic and therapeutic procedure have to be documented more carefully. A bad knowledge is that important parts of the anamnesis were found out just insufficient in many cases and the diagnostic and therapeutic acting were reproduceable only partially. To diagnose and treat optimal, a sufficient anamnesis, a guideline orientated diagnosis and treatment with thorough documentation is irreplaceable. In the future we have to acquire better standards of quality for the state Berlin.
