Obwohl die Degradation der extrazellulären Matrix durch das proteolytische System an Matrixmetalloproteinasen (MMP) eine entscheidende Rolle für das myokardiale Remodeling der Herzinsuffizienz spielt, ist bis heute die Regulation der myokardialen MMP-Expression nicht eindeutig geklärt. In der vorliegenden Studie wurde die Relevanz der chronischen Entzündung des Myokards für die Induktion der MMP-Expression bei Patienten mit inflammatorischer dilatativer Kardiomyopathie (infDCM) analysiert. 15 Patienten mit dilatativer Kardiomyopathie (EF 33+/-12%) wurden in die Studie eingeschlossen und mit einer Kontrollgruppe (KG) von 9 Patienten mit lokoregionaler linksventrikulärer Dysfunktion verglichen (EF 69+/-5%). Rechtsventrikuläre Myokardbiopsien aller 24 Patienten wurden durch histologische (Dallas- Kriterien) und immunhistologische (Infiltration an CD3 T-Lymphozyten) Techniken hinsichtlich der myokardialen Entzündung analysiert. Bei keinem der Patienten konnte histologisch eine aktive Myokarditis oder eine Borderline- Myokarditis diagnostiziert werden. 7/15 Patienten (47%) wurden aufgrund des immunhistologischen Befundes einer signifikant erhöhten Infiltration von CD3 T-Lymphozyten/High Power Field (3,40+/-2,92 versus 0,53+/-0,28) als infDCM klassifiziert. Mittels semiquantitativer RT-PCR wurde die Menge an mRNA von MMP-3 und seinem Inhibitor, Tissue Inhibitor of Metalloproteinases, TIMP-4, quantifiziert. Eine signifikante Erhöhung der mRNA Menge von MMP-3 (0,907+/-0,835 vs. 0,269+/-0,165) war mit einer signifikanten Erniedrigung von TIMP-4 (0,634+/-0,328 vs. 0,890+/-0,102) bei Patienten mit infDCM im Vergleich zur Kontrollgruppe assoziiert (MANOVA, ANOVA, LSD-Test, p<0.05). Dabei korrelierte der Grad der inflammatorischen Reaktion des Myokards positiv mit der mRNA von MMP-3 (r=0.463, p=0.013) und negativ mit der m-RNA Menge an TIMP-4 (r=-0.337, p=0.057). Es konnte jedoch kein signifikanter Unterschied der mRNA Menge von MMP-3 und TIMP-4 bei DCM ohne signifikante Infiltration mit T-Lymphozyten im Vergleich zur Kontrollgruppe festgestellt werden. Die chronische myokardiale Entzündung (infDCM) ist mit einer Erhöhung von MMP-3 ohne Gegenregulation der TIMP-4 Expression verbunden. Diese Imbalance der MMP-3 und TIMP-4 Expression kann eine zentrale Rolle für die Progression der linksventrikulären Funktion und somit große therapeutische Implikationen haben.
The disruption of the extracellular matrix by matrix metalloproteinases (MMPs) plays a key role in myocardial remodeling in heart failure. However, the mechanisms for MMP expression are still unknown. In this study we analyzed the relevance of chronic myocardial inflammation in patients with inflammatory dilated cardiomyopathy (infDCM) for the induction of MMP expression. 15 patients with the clinically suspected diagnosis of DCM (EF 33+/-12%) were enrolled in this study. 9 patients with locoregional left ventricular dysfunction (EF 69+/-5%) served as a control. Endomyocardial biopsies of the right ventricle of all 24 patients were analyzed by histological (Dallas criteria) and immunohistological techniques (CD3-T-lymphocyte infiltration) for the degree of myocardial inflammation. None of the patients were classified as active or borderline myocarditis. However, 7/15 (47%) were classified as infDCM with significantly increased CD3-T-lymphocytes/high power field (3,40+/-2,92 versus 0,53+/-0,28). Semi-quantitative RT-PCR was performed for the evaluation of mRNA abundance of MMP-3 and tissue inhibitor of MMP-4 (TIMP-4). The significant upregulated MMP-3 abundance (0,907+/-0,835 vs. 0,269+/-0,165) was associated with a significant downregulation of TIMP-4 (0,634+/-0,328 vs. 0,890+/-0,102) in patients with infDCM compared to controls (MANOVA, ANOVA, LSD-Test p<0.05). The degree of myocardial inflammation was positive correlated with the MMP3-mRNA abundance (r=0.463, p=0.013) and negative correlated with the TIMP4 mRNA abundance (r=-0.337, p=0.057). However, there was no significant difference in MMP-3 and TIMP-4 mRNA abundance in DCM patients without significant T-lymphocytic infiltrates compared with controls. Chronic myocardial inflammation in infDCM is associated with an increased MMP-3 mRNA abundance without a counterbalance of TIMP-4 expression. This imbalance of MMP-3 and TIMP-4 abundance might constitute a key factor for the progression of left ventricular function in these patients, which might have major therapeutic implications.
