Abstract: Research on wound healing and skin anti-inflammatory agents traditionally relies on in vitro or in vivo studies, each with inherent limitations in replicating the complex, dynamic interactions of living systems. In vitro models fail to capture critical immune responses, while in vivo studies face ethical concerns, high costs, and variability that challenge data reproducibility and interpretation. In contrast, ex vivo skin models based on cultured human skin explants have emerged as effective platforms for realistically evaluating the therapeutic potentials of various wound healing and anti-inflammatory agents due to their closer mimicry of human biological responses. The presented studies focused on the efficacy of nanoparticle formulations and adipose-derived stem cells (ASCs) or their conditioned medium (ASC-CM) in modulating inflammatory responses and enhancing tissue regeneration. The first study assessed ASCs and ASC-CM application in an ex vivo wound model. Skin viability, histology, β-catenin expression, and levels of inflammatory mediators and growth factors were monitored over a 12-day culture period. Despite a slight decline in metabolic activity, skin morphology and epithelial regeneration at wound edges were maintained. β-catenin expression increased, particularly in ASC-CM treated smaples. Growth factors (HGF, PDGF-AA) and inflammatory mediators (IL-1α, IL-7, TNF-α, and IL-10) were upregulated over time, with ASC-CM significantly upregulated inflammatory cytokines and PDGF-AA, whereas ASCs in DMEM with 10% FCS elevated FGF-basic and certain immunomodulatory cytokines. The results indicate that ASCs and ASC-CM modulate wound microenvironment by regulating cytokines and growth factors, enhancing re-epithelialization and vascularization to promote wound healing. The second study employed an inflammatory ex vivo skin model to evaluate the anti-inflammatory drug sirolimus formulated in environment-sensitive nanocarriers. These oxidative-sensitive CMS nanocarriers improved drug localization and suppressed IL-2 release in co-cultured Jurkat T cells. The third study compared the anti-inflammatory efficacy of different tofacitinib (TFB) delivery systems using the inflammatory ex vivo model. After 24 h treatment, the activation of the signal transducer and activator of transcription (STAT3 and STAT5) pathways decreased, though IL-6 and IL-8 suppression remained unchanged. These studies effectively demonstrate the ex vivo human skin model as a robust platform for evaluating the complex interactive effects and overall efficacy of various therapeutic interventions.
Zusammenfassung: Die Forschung zur Wundheilung und entzündungshemmenden Wirkstoffen für die Haut stützt sich traditionell auf in vitro- und in vivo-Studien, die jedoch Einschränkungen bei der Nachbildung komplexer biologischer Interaktionen aufweisen. In-vitro-Modelle erfassen kritische Immunreaktionen nur unzureichend, während in-vivo-Studien ethische Bedenken, hohe Kosten und Variabilität mit sich bringen, die die Datenreproduzierbarkeit erschweren. Ex vivo-Hautmodelle mit kultivierten humanen Hautexplantaten bieten eine realistischere Plattform zur Bewertung therapeutischer Ansätze. Diese Arbeit untersucht die Wirksamkeit von Nanopartikeln sowie aus Fettgewebe gewonnenen Stammzellen (ASCs) und deren konditioniertem Medium (ASC-CM) in der Modulation von Entzündungsreaktionen und Geweberegeneration. In der ersten Studie wurde die Anwendung von ASCs und ASC-CM in einem ex vivo-Wundmodell analysiert. Über 12 Tage wurden Hautviabilität, Histologie, β-Catenin-Expression sowie Entzündungsmediatoren und Wachstumsfaktoren untersucht. Trotz eines leichten Rückgangs der Stoffwechselaktivität blieben Hautmorphologie und Epithelregeneration erhalten. Die β-Catenin-Expression war besonders in ASC-CM-behandelten Proben erhöht. Zudem stiegen Wachstumsfaktoren (HGF, PDGF-AA) und Entzündungsmediatoren (IL-1α, IL-7, TNF-α, IL-10) über die Zeit. ASC-CM förderte die Hochregulation von Zytokinen und PDGF-AA, während ASCs in DMEM mit 10% FCS vor allem FGF-basic und immunmodulatorische Zytokine erhöhten. Dies zeigt, dass ASCs und ASC-CM die Wundmikroumgebung beeinflussen, Zytokine und Wachstumsfaktoren regulieren und so Reepithelialisierung und Vaskularisierung fördern. In der zweiten Studie wurde ein entzündliches ex vivo-Hautmodell verwendet, um Sirolimus in umgebungssensitiven Nanoträgern zu untersuchen. Die oxidationssensitiven CMS-Nanoträger verbesserten die Medikamentenlokalisierung und unterdrückten die IL-2-Freisetzung in ko-kultivierten Jurkat-T-Zellen. Die dritte Studie verglich die entzündungshemmende Wirksamkeit verschiedener Tofacitinib-(TFB)-Verabreichungssysteme im entzündlichen ex vivo-Modell. Nach 24 Stunden Behandlung zeigte sich eine verminderte Aktivierung der Signalwege STAT3 und STAT5, während die Expression von IL-6 und IL-8 unverändert blieb. Diese Studien unterstreichen das Potenzial des ex vivo-Hautmodells als robuste Plattform zur Evaluierung therapeutischer Ansätze in der Wundheilung und Entzündungshemmung.
