EINLEITUNG: Die vorliegende Arbeit stellt eine unizentrisch prospektive Studie, die mit dem NEO-FFI Test das Persönlichkeitsprofil der Patienten vor Therapiebeginn evaluiert und nach erfolgter Zahnimplantation mit Hilfe des OHIP-G 14 Fragebogens die Beeinträchtigung der mundgesundheitsbezogenen Lebensqualität erfasst hat dar. Hauptzielstellung der Arbeit ist die Frage, ob durch die Ausprägung bestimmter Persönlichkeitsmerkmale sich die mundgesundheitsbezogene Lebensqualität hinsichtlich der Zufriedenheit mit der Zahnimplantation vorhersagen lässt. METHODE: Zwischen dem 24.03.2008 und 14.12.2009 wurden bei insgesamt 50 Patienten (25 Männer und 25 Frauen, mittleres Alter 52,1 Jahre) bei denen eine Zahnimplantat Therapie mit Xive Implantaten geplant war, auf freiwilliger Basis eine Woche vor Implantat Therapie, die Persönlichkeitsprofile und 3 Monate nach Implantat Therapie, die mundgesundheitsbezogene Lebensqualität mittels Fragebögen ermittelt. Verwendet wurden dafür das NEO-Fünf Faktoren Inventar (NEO-FFI) und der Oral Health Impact Profile Germany 14 (OHIP-G 14) in ihrer deutschen Übersetzung. Durch die gewählten Ein- und Ausschlusskriterien wurde gewährleistet, dass die Patienten in beiden Gruppen nach 3-6 Monaten ihre prothetische Versorgung erhielten. Alle Patienten erhielten implantatgetragene Einzelkronen. Alle Daten wurden prospektiv analysiert und statistisch mittels der Software Statistica 7.1 ausgewertet. Häufigkeitstabellen wurden mittels Pearsons Chi- Quadrat-Test analysiert. Es wurde der Pearson Korrelationstest benutzt, um den Zusammenhang zwischen den Variablen Alter und NEO-FFI zu analysieren. Die Daten des OHIP-G 14 und die Anzahl der Implantate wurden mittels Mann- Whitney-U-Test untersucht. Der Spearman Korrelationstest wurde angewendet, um den Zusammenhang zwischen der Anzahl der Implantate und den Scores der Fragebögen sowie zwischen den Scores der Fragebögen zu untersuchen. Es wurde überprüft, ob und inwieweit die Testwertausprägungen des OHIP-G 14 und des NEO-FFI vom Geschlecht, Alter und Implantatanzahl der Studienteilnehmer abhängig waren. Der Zusammenhang zwischen den Testwerten und den potentiellen Einflussfaktoren wurde mit dem Korrelationskoeffizienten (r) nach Spearman dargestellt. Als signifikant wurden p-Werte von p<0,05 angesehen. ERGEBNISSE: Männer zeigten einen statistisch signifikanten Zusammenhang zwischen den Persönlichkeitsdimensionen: Neurotizismus (p = 0,001), Extraversion (p = 0,044), Verträglichkeit (p = 0,035) und Gewissenhaftigkeit (p = 0,011) und der mundgesundheitsbezogenen Lebensqualität. Bei unseren Gesamtpatienten zeigte sich eine Tendenz, dass die Scores für Neurotizismus bis zum ca. 50. Lebensjahr ansteigen, während die für Verträglichkeit, Gewissenhaftigkeit und Offenheit abfallen. SCHLUSSFOLGERUNG: Basierend auf den Ergebnissen unserer Studie und in Übereinstimmung mit den Ergebnissen anderer Studien, könnte die Ermittlung von Persönlichkeitsmerkmalen, insbesondere von Neurotizismus, vor Therapiebeginn wertvolle Informationen für die Vorhersage der mundgesundheitsbezogenen Lebensqualität hinsichtlich der Zufriedenheit mit einer Zahnimplantat Therapie geben.
INTRODUCTION: This study is an unicenter, prospective trial which investigates the association between personality profiles among patients seeking dental implant therapy with the NEO-FFI Test and its impairment on the oral health related quality of life after dental implantation with the OHIP-G 14 questionnaire. Our primary aim was to assess whether the markedness of certain personality traits can predict the oral health related quality of life regarding the satisfaction with dental implantation. METHOD: From March 24th 2008 through December 14th 2009 50 patients (25 men and 25 women, mean age 52,1 years) with a planed dental implantation with Xive implants completed questionnaires about their personality profiles one week before implant therapy and about their oral health related quality of life 3 months after implant therapy on a voluntary basis. The NEO-Five Factor Inventory (NEO-FFI) and the Oral Health Impact Profile Germany 14 (OHIP-G 14) in their german translations were used. The selected inclusion and exclusion criteria ensured that patients in both groups received their prosthetics after 3-6 months. All patients received implant supported single crowns. All data were prospectively analyzed and statistically evaluated with the Software Statistica 7.1. Frequency tables were analyzed with the pearsons chi-square-test. The pearson correlation test was used to investigate the correlation between the variables age and NEO-FFI. The Mann-Whitney-U-test was used to examine the data of OHIP-G 14 and the number of implants. The spearman correlation test was used to show the correlation between the number of implants and the scores of the questionnaires as well as between the questionnaires. It was examined whether and to what extend the test markedness of the OHIP-G 14 and the NEO-FFI were depending from gender, age and number of implants of the study participants. The correlation between the test results and the potential influential factors was shown with the spearman correlation coefficient (r). P<0,05 was considered statistically significant. RESULTS: Men showed a statistic significant correlation between the personality dimensions: neuroticism (p=0,001), extraversion (p=0,044), agreeableness (p=0,035) and conscientiousness (p=0,011) and the oral health related quality of life. All patients showed a tendency of a progressive neuroticism score till the age of 50, while the ones for agreeableness, conscientiousness and openness were decreasing. CONCLUSION: Considering our results and the correspondence with the results of other studies, the investigation of personality traits, especially neuroticism prior to treatment could give important information for the prediction of the oral health related quality of life regarding the satisfaction with dental implant therapy.
