Einleitung: Der Einfluss der p16 Expression auf das biologische Verhalten und die Prognose des oralen Plattenepithelkarzinoms stellt ein kontrovers diskutiertes Thema dar. Wenngleich insbesondere für oropharyngeale Tumoren eine Verbesserung des Überlebens bei erhöhter p16-Expression als gesichert gilt, differieren die Ergebnisse in Bezug auf das orale Plattenepithelkarzinom stark. Aufgrund der elementaren Rolle als Tumorsuppressor gilt die Beeinflussung des p16 als integraler Bestandteil einer HPV induzierten Karzinogenese u.a. mit Einfluss auf die Teilungsrate. Primäre Zielstellung dieser Arbeit ist die Untersuchung des Einflusses von p16- und Mib-Status auf das orale Plattenepithelkarzinom in einer großen Kohorte. Material und Methodik: Formalin-fixierte Paraffin-eingebettete Präparate von 339 Tumoren wurden gegen p16 und Mib/Ki67 mittels Immunhistochemie gefärbt. Die Auswertung der Präparate erfolgte als manuelle Zählung mit Bestimmung des relativen Anteils positiver Tumorzellen. P16- positiven Präparate wurden auf das Vorhandensein von HPV-DNA mittels PCR-Testung untersucht. Die Bestimmung der primären Endpunkte erfolgte als Korrelation der erzielten Ergebnisse mit den klinischen Charakteristika. Ergebnisse: 23 der 339 Tumoren (6,8%) erfüllten die Kriterien einer p16-Positivität, wobei in lediglich 6 der auszuwertenden Tumoren ein HPV-DNA Nachweis gelang (positiv-prädiktiver Wert: 0,4). Kombiniert p16-/HPV-positive Tumoren zeigen sich vermehrt bei jüngeren Patient:innen (p=0,036) und häufiger eine extrakapsuläre Ausbreitung (p=0,015). Der durchschnittliche Anteil Mib-positiver Tumorzellen lag bei 28,4% ohne signifikante Unterschiede zwischen p16-positiven und -negativen Tumoren. Es zeigte sich ein signifikanter Einfluss des p16- und Mib-Status auf das Grading mit schlechter differenzierten Tumoren bei jeweils hoher Positivität. In der Überlebensanalyse ergaben sich lediglich in der kombinierten Betrachtung der p16- und Mib-Positivität signifikante Unterschiede, wobei p16-positive Tumoren mit niedrigen Teilungsraten allen anderen Subgruppen überlegen sind (p=0,048, p=0,048, p=0,034). Es zeigte sich eine Überlegenheit von p16-positiven Tumoren mit niedrigen Teilungsraten gegenüber allen anderen Subgruppen bei Patient:innen, die einer Operation ohne Adjuvanz zugeführt wurden (p=0,005, p=0,014, p=0,006). Diese Unterschiede finden sich nicht bei Patient:innen mit einer Operation und anschließender Adjuvanz. Während Tumoren mit einer Teilungsrate >21% von einer systemischen Therapie profitieren (p=0,034), zeigten Tumoren mit einer p16-Positivität >25% eine bessere Prognose unter einer Adjuvanz (p=0,042). Zusammenfassung: In dem hier untersuchten Kollektiv fand sich keine Korrelation zwischen p16-/HPV-Status und Mib-Positivität. Es zeigte sich ein entscheidender Einfluss der kombinierten Betrachtung von p16- und Mib-Positivität sowohl auf das Overall Survival im Gesamtkollektiv als auch in Bezug auf die Therapieform.
Introduction: The influence of p16 expression on the biological behavior and prognosis of oral squamous cell carcinoma is a subject of controversial discussion. While an improvement in survival is well-established for the entirety of head and neck tumors, especially for oropharyngeal tumors, the results vary significantly concerning oral squamous cell carcinoma. Due to its fundamental role as a tumor suppressor, the influence of p16 is considered an integral part of HPV-induced carcinogenesis, including an impact on the rate of cell division. Therefore, the increased expression of p16 is discussed as a surrogate marker for HPV infection. The primary objective of this study is to investigate the influence of p16 and Mib status on oral squamous cell carcinoma in a large cohort. Material and Methods: Formalin fixed paraffin-embedded tissue samples of 339 tumors were stained for p16 and Mib/Ki67. Evaluation of the preparations was carried out through manual counting, determining the relative proportion of positive tumor cells. In a further step, p16-positive preparations were tested for the presence of HPV DNA using PCR testing. The primary endpoints were determined by correlating the obtained results with clinical characteristics. Results: Among the 339 tumors, 23 (6.8%) met the criteria for p16 positivity, with only 6 of these tumors showing HPV DNA detection (positive predictive value: 0.4). Combined p16/HPV-positive tumors were observed more frequently in younger patients (p=0.036) and exhibited extracapsular spread more frequently (p=0.015). The average proportion of Mib-positive tumor cells was 28.4%, with no significant differences between p16-positive and negative tumors. There was a significant influence of p16 and Mib status on grading, with poorly differentiated tumors showing high positivity for both. In survival analysis, significant differences were observed only in the combined evaluation of p16 and Mib positivity, with p16-positive tumors with low proliferation rates being superior to all other subgroups (p=0.048, p=0.048, p=0.034). When divided by treatment modality, p16-positive tumors with low proliferation rates were superior to all other subgroups in patients who underwent surgery without adjuvant therapy (p=0.005, p=0.014, p=0.006). These differences were not observed in patients who underwent surgery followed by adjuvant therapy. While tumors with a proliferation rate >21% benefited from systemic therapy (p=0.034), tumors with p16 positivity >25% showed an improved response to adjuvant therapy (p=0.042). Conclusion: In thisstudy, no correlation was found between p16/HPV status and Mib positivity. There is a significant influence of the combined evaluation of p16 and Mib positivity on overall survival in the overall cohort as well as on overall survival related to the treatment modality.
