Zu den häufigsten Formen akuter und chronischer Schmerzen gehört der
Entzündungsschmerz (z.B. postoperativer oder arthritischer Schmerz). Sowohl
für die Gesundheit des Einzelnen als auch für die Ökonomie sind Schmerzen von
großer Bedeutung. Opioide bilden zwar die Haupttherapie für starken akuten und
chronischen Schmerz, sie verursachen jedoch eine Reihe von Nebeneffekten wie
z.B. körperliche und psychische Abhängigkeit, Sedierung und Atemdepression,
die von dem Einfluss der Opiate auf das zentrale Nervensystem herrühren. Daher
besteht ein Bedarf für Wirkmittel und Applikationsformen, die bei chronischen
Schmerzen Erleichterung ohne Nebeneffekte schaffen. Opiate üben eine Analgesie
durch Aktivierung verschiedener Typen von Opioidrezeptoren (µ, δ, κ) aus.
Endomorphine (EMs) sind endogene selektive Agonisten des µ-Opioidrezeptors.
Ihr Stellenwert in der Behandlung des entzündlichen Schmerzes ist bisher nicht
vollständig geklärt. In meiner Arbeit habe ich anhand von Ratten den
antinozizeptiven Effekt der EMs im peripheren entzündeten und nicht
entzündeten Gewebe untersucht. Den Ratten wurde eine einseitige Entzündung der
Hinterpfote mit Hilfe abgetöteter Mykobakterien induziert. 4 - 6 Tage nach der
Entzündungsinduktion wurde ihre Schmerzschwelle anhand eines mechanischen
Hyperalgesietests bestimmt. Die EMs wurden direkt in beide Hinterpfoten
appliziert, es zeigten sich jedoch ausschließlich in der entzündeten Pfote
antinozizeptive Effekte. Diese Effekte konnten durch die lokale Applikation
eines selektiven µ-Rezeptor-Antagonisten (D-Phe -Cys-Tyr-D-Try-Orn-Thr-Pen-
Thr-NH2), nicht jedoch durch einen κ-Rezeptor-Antagonisten
(norBinaltorphimindihydrochlorid) geblockt werden. Die δ-Rezeptor-Antagonisten
(N,N-diallyl-Tyr-Aib-Aib-Phe-Leu und Naltrindolhydrochlorid) konnten die
antinozizeptiven Effekte von EM-1 nicht beeinflussen. EM-2 hingegen konnte
durch die beiden δ-Rezeptor-Antagonisten dosisabhängig gemindert werden.
Antikörper gegen β-Endorphin, Methionin-Enkephalin oder gegen Leucin-
Enkephalin konnten die EM-2 induzierte Antinozizeption nicht signifikant
beeinflussen. Somit ist nicht von einer indirekten Vermittlung durch andere
Opioidpeptide auszugehen. Schließlich haben wir das Bindungsverhalten von EM-2
am δ-Opioidrezeptor untersucht. Es ließ sich keine verlässliche Bindung von
radioaktiv markierten δ-Liganden in den Hinterwurzelganglien nachweisen.
Dennoch kann nicht ausgeschlossen werden, dass EM-2 direkt an den δ-Rezeptoren
der peripheren primär afferenten Neuronenenden bindet, zum einen aufgrund des
verstärkten axonalen Transports von Opioidrezeptoren zu peripheren
Nervenendigungen im Rahmen einer subkutanen Entzündung, zum anderen aufgrund
vom mehrfachen immunhistochemischen Nachweisen von δ-Rezeptoren am peripheren
Ende sensorischer Neurone. Dieses muss jedoch in nachfolgenden Studien weiter
untersucht werden.

Inflammatory pain (e.g. postsurgical pain, arthritis) is among the most
prevalent forms of acute and chronic pain. Pain is a problem of major
prevalence for both patients and for the economy. Opioids remain the major
therapy for persistent pain. However, systemically administered opioids
produce a number of side effects, such as physical and psychological
addiction, sedation or respiratory depression which results from the effects
of opioids on the central nervous system. Hence, there is a need for agents
and their ways of administration which provide relief of chronic pain without
producing any side effects. Opioids exert analgesia through activation of
different types of opioid receptors ( µ, δ, κ). Endomorphins (EMs) are
endogenous selective µ-opioid receptor agonists. Their role in inflammatory
pain, however, has not been fully elucidated yet. In my study I used rats to
examine the antinociceptive effects of EMs in peripheral both inflamed and
noninflamed tissue. In order to induce an inflammation the rats received
killed mycobacteria into the right hindpaw. 4 - 6 days after induction of
inflammation the pawpressure test (a mechanical hyperalgesia) was used to
assess the nociceptive threshold. EMs injected directly into both hindpaws,
however, produced antinociception exclusively in the inflamed paws. These
effects were blocked by locally applied µ-receptor-selective (DPhe-Cys-Tyr-D
-Try-Orn-Thr-Pen-Thr-NH2), but not by κ-receptor-selective
(norbinaltorphimine) antagonists. δ-Receptor antagonists (N,N-diallyl-Tyr-Aib-
Aib-Phe-Leu and naltrindole) did not influence the effects of EM-1. EM-2,
however, could be dosedependently decreased by both δ-receptor antagonists.
Antibodies against β-endorphin, methionine-enkephalin or leucine-enkephalin
did not significantly change EM-2 induced antinociception. Therefore, an
indirect binding through other opioid peptides does not appear very likely.
Finally we tested the way EM-2 binds directly to δ-opioid receptors. We could
not, however, detect any reliable binding of radioactive δ-ligands in the
dorsal root ganglion. Nevertheless, it cannot be excluded that EM-2 binds
directly to δ-receptors on peripheral primarily afferent neurons. On the one
hand there is an increased intra-axonal transport of opioidreceptors to the
peripheral nerve terminal within the inflamed subcutaneous tissue. On the
other hand, the immunohistochemistry has shown many times that δ-receptors are
located at the peripheral sensory nerve terminal. This, however, is to be
analysed in further studies.
