Zielstellung: Das Ziel meiner Studie war es, die Hypothese zu überprüfen, dass
Patienten mit Herzinsuffizienz mit normaler linksventrikulärer
Ejektionsfraktion (HFNEF) zu einem hohen Prozentsatz eine myokardiale
systolische und diastolische Dysfunktion des linken Ventrikels (LV), des
rechten Ventrikels (RV), und des linken Atriums (LA) aufweisen, welche in den
symptomatischen Status dieser Patienten verwickelt sein könnte. Methoden: Um
meine Hypothese zu validieren, habe ich bei 201 Patienten mit HFNEF und bei
364 asymptomatischen Kontrollpatienten ähnlichen Alters, Geschlechts und LVEF,
die in der Charité Campus Virchow im Zeitraum vom April 2009 bis zum April
2011 stationär behandelt wurden, eine Transthorakale Echokardiographie
durchgeführt. Ergebnisse: Die LV, RV, und LA longitudinale diastolische und
systolische Funktion war bei Patienten mit HFNEF signifikant eingeschränkter
als bei asymptomatischen Patienten (alle p < 0.0001). In diesem Zusammenhang
war die Prävalenz von LV, RV, und LA longitudinaler systolischer und
diastolischer Dysfunktion bei Patienten mit HFNEF sehr hoch (d.h., 80% und
60%; 75% und 48%; 65.5% und 28.5%; bzw.). Darüber hinaus wiesen Patienten mit
longitudinaler systolischer und diastolischer Dysfunktion des RV, LV, und LA
einen schlechteren Symptomatikstatus als Patienten mit normaler RV, LV, und LA
Funktion auf. Schlussfolgerungen: Dementsprechend habe ich darauf hingewiesen,
dass HFNEF nicht nur eine Störung mit isolierter LV diastolischer Dysfunktion,
sondern eine Krankheit mit komplexen systolischen Abnormitäten des LV, RV und
LA ist. Auf der Grundlage dieser Ergebnisse kann die Wiederherstellung einer
systolischen und diastolischen longitudinalen Dysfunktion des LV, RV, und LA
bei HFNEF die Symptome dieser Patienten verbessern. So sieht meine Forschung
neue pathophysiologische Erkenntnisse und damit potenzielle therapeutische
Wege für diese komplexe Krankheit, für die bisher keine wirksamen Therapien
existieren, vor.

Objectives: The aim of this study was to test the hypothesis that patients
with heart failure with normal left ventricular (LV) ejection fraction (HFNEF)
have a high rate of myocardial systolic and diastolic dysfunction of the left
ventricle (LV), right ventricle (RV), and left atrium (LA) which is associated
with the symptomatic status of these patients. Methods: In order to validate
this hypothesis, 201 patients with HFNEF and 364 asymptomatic subjects of
similar age, gender, and LV ejection fraction, who were admitted in der
Charité Campus Virchow between April 2009 to April 2011, were studied using
two-dimensional speckle-tracking echocardiography. Results: The longitudinal
systolic and diastolic function of the LV, RV, and LA was significantly more
impaired in patients with HFNEF than in asymptomatic patients (all p <
0.0001). In this regard, the rate of longitudinal systolic and diastolic
dysfunction of the LV, RV, and LA in patients with HFNEF was very high
(namely, 80% and 60%; 75% and 48%; 65.5% and 28.5%; respectively). In relation
to these findings, patients with both systolic and diastolic longitudinal
dysfunction of the LV, RV, and LA presented worse symptomatic status than
those with normal longitudinal function of the LV, RV, and LA. Conclusions:
This study has highlighted that HFNEF is not only a disease with isolated LV
diastolic dysfunction, but a disorder with complex systolic and diastolic
abnormalities of the LV, RV, and LA. Accordingly, on the basis of these
findings, the restoration of the longitudinal systolic and diastolic
dysfunction of the LV, RV, and LA in HFNEF could help to improve the
symptomatology of these patients. Thus, this research provides new
pathophysiological insights and thereby potential therapeutic pathways for
this complex disease for which, so far, no effective therapies exist.
