Hintergrund: Trotz internationaler Verbreitung und Anerkennung als moderne und zukunftsweisende Basis für eine gute Versorgung von schwer psychisch Erkrankten, finden teambasierte, ambulante, flexible und integrative Versorgungsmodelle zur Akutbehandlung in Deutschland selten Anwendung. Zur Förderung dieser Versorgungsformen hat der Gesetzgeber verschiedene Maßnahmen ergriffen, darunter im Jahr 2013 auch die Einführung des § 64b Sozialgesetzbuch V (SGB V). Dieser Paragraph erlaubt eine Kostenabrechnung auf Basis der Kopfpauschale und damit eine flexiblere Steuerung der therapeutischen Maßnahmen. Infolgedessen entstanden bundesweit Modellprojekte, die flexiblere, kontinuierliche, teambasierte und stationsersetzende Versorgungsleistungen anbieten. Ziel: Gemäß den gesetzlichen Vorgaben werden die im Rahmen des § 64b SGB V entstandenen neuen Versorgungsmodelle begleitend evaluiert. Teil der Evaluationsstrategie ist die EvaMod64b Studie, deren V orstudie darauf abzielt, praktisch anwendbare Merkmale zu entwickeln, operationalisieren, quantifizieren und validieren, welche die spezifischen Strukturen und Prozesse der Modellprojekte nach § 64b SGB V abbilden. Methoden: Im Rahmen der EvaMod64b Vorstudie wurde ein mehrschrittiger, iterativer Prozess gemäß der Grounded Theory Methodology genutzt, um spezifische Merkmale der Modellprojekte zu identifizieren und operationalisieren. Die Inhaltsvalidität dieser Merkmale wurde mittels Berechnung der Content Validity Ratio ermittelt und die Reliabilität anhand des internen Reabilitätskoeffizienten Cronbachs α berechnet, ebenfalls wurde die Augenscheinvalidität unter Einbezug aller Stakeholder ermittelt. In einem weiteren Schritt wurden anhand eines eigens entwickelten Algorithmus und basierend auf einer standardisierten Expert*innenbefragung die Merkmale quantifiziert, das relative Gewicht eines jeden Merkmales berechnet und ein Gesamtwert entwickelt. Ergebnisse: Zehn von elf entwickelten Merkmalen konnten operationalisiert, quantifiziert, gewichtet und in einen Gesamtscore integriert werden. Die Augenscheinvalidität aller Merkmale war gut und die Inhaltsvalididät ausreichend. Insgesamt acht Merkmale wiesen zudem eine gute, und der Gesamtscore aller Merkmale eine exzellente Reliabilität auf. Schlussfolgerungen: Die entwickelten Merkmale der EvaMod64b Vorstudie dienen der verlässlichen Einstufung und Evaluation der Modellprojekte nach §64b SGB V. Sie zeigen sich anschlussfähig an bereits existierende internationale Literatur und können der Entwicklung von Prozess- und Ergebnisevaluationsinstrumenten zu Projekten der Modellversorgung nach § 64b SGB V dienen.
Background: Despite wide international use and perception as modern and progressive foundation for good care of severely mentally ill patients, team-based, outpatient, flexible and integrative care models are still rarely applied in acute psychiatric care in Germany. The legislator has taken various measures to promote novel forms of care, including the introduction of § 64b German Social Security Code (SGB V) 2013. The newly installed law allowed a capitation-based reward system of health care services and by that ensured a more flexible and autonomous steering of psychiatric care. Consequently, new flexible, team- and home-based care models were implemented all over Germany in the past decade. Objective: As part of a nationwide evaluation strategy, the EvaMod64b pre-study was designed to validate and operationalize the newly installed care models, implemented on the basis of the §64b SGB V legal framework. Validation included developing, operationalizing, quantifying and interpreting program components that describe specific structures and processes of model projects. Methods: The EvaMod64b pre-study is a multi-step iterative process based on the Grounded Theory Methodology, which was used to identify and operationalize model project specific components. The content validity of these components was determined by calculating the content validity ratio and the reliability was calculated using the internal reliability coefficient Cronbach's α. Furthermore, the face validity was examined involving all stakeholders. In a second step, using a formalized mathematical process and based on a standardized expert survey, components were quantified, the relative weight of each component was calculated and the components where integrated into an overall score. Results: Ten of eleven developed components could be operationalized, quantified and integrated into an overall score. The face validity of all operationalized components was good and the content validity sufficient, eight of ten components showed a good reliability and the overall score an excellent reliability. Conclusions: The newly developed components of the EvaMod64b pre-study serve as a first, relevant hallmark for the classification and evaluation of novel model-projects based on the § 64b SGB V. They proved to be compatible to the existing international literature on critical components of innovative psychiatric care. The herein described components may serve as future evaluation instruments for model projects of psychiatric care based on the § 64b SGB V.
