BACKGROUND The therapeutic blockade of tumor necrosis factor-α (TNF-α) is used with great success in various chronic inflammatory diseases. In ankylosing spondylitis (AS) or Bechterew's disease, only about 50-60% of patients show a good to very good response, about 30% are classified as non-responders. Since there are now various therapeutic options for TNF-α blockade in AS patients available, great interest exists in the discovery of new biomarkers, which allow therapy stratification of patients already before the start of treatment. Therefore, the aim of this explorative study was to identify cell-based biomarkers in peripheral blood that can be used for the pretherapeutic stratification of AS patients using cell-based multiparametric cytometry. METHODS A multiparametric flow cytometric approach was established to identify baseline predictors in 31 AS patients prior to treatment with the TNF blockers adalimumab (TNF neutralization) and etanercept (soluble TNF receptor) at the single cell level. RESULTS The high-resolution immunophenotyping approach allows the identification of a subpopulation of CD8-expressing natural killer (NK) cells whose increased frequency correlates with good response to TNF blockade. While an inverse correlation of classical CD56+/CD16+NK cells with a reduction in disease activity could be observed, it behaved the other way around with the CD8+ NK cell subpopulation. Thus, prior to treatment, responders showed a significantly increased frequency of CD8+ NK cells compared to non-responders. CONCLUSIONS This work demonstrated for the first time that detailed immunophenotyping, especially of the NK cell compartment, allowed the identification of a baseline predictor for predicting the success of treatment in anti-TNF-α-treated AS patients. Pathomechanically, these data indicate an active involvement of NK cells in general and particularly of CD8+ expressing subsets in the chronic inflammatory process in AS.
Die therapeutische Blockade des Tumornekrosefaktor-α (TNF-α) wird mit großem Erfolg bei verschiedenen chronisch-entzündlichen Erkrankungen angewendet. Im Fall der ankylosierenden Spondylitis (AS) oder Morbus Bechterew zeigen nur ca. 50-60% der Patienten ein gutes bis sehr gutes Ansprechen, ca. 30% werden als Therapieversager eingestuft. Da es mittlerweile verschiedene therapeutische Optionen zur TNF-α- Blockade bei AS-Patienten gibt, besteht ein großes Interesse an neuen Biomarkern, die eine Therapiestratifizierung von Patienten bereits vor Behandlungsbeginn ermöglichen. Ziel dieser explorativen Arbeit war es daher, mit Hilfe der multiparametrischen Zytometrie zellbasierte Biomarker im peripheren Blut zu identifizieren, die zur prätherapeutischen Stratifizierung von AS-Patienten eingesetzt werden können. METHODEN Ein multiparametrischer, durchflusszytometrischer Ansatz wurde etabliert, um Baseline- Prädiktoren in 31 AS-Patienten vor der Behandlung mit den TNF-Blockern Adalimumab (TNF-Neutralisation) und Etanercept (löslicher TNF-Rezeptor) auf Einzelzell-Niveau zu identifizieren. ERGEBNISSE Der hochauflösende Immunphänotypisierungsansatz erlaubte es, eine Subpopulation von CD8-exprimierenden natürlichen Killer (NK)-Zellen zu identifizieren, deren erhöhte Frequenz im Blut mit einem guten, klinischen Ansprechverhalten auf eine TNF- Blockade korreliert. Während eine inverse Korrelation von klassischen CD56+/CD16+ NK-Zellen mit einer Reduktion der Krankheitsaktivität beobachtet werden konnte, zeigte die CD8+ NK-Zell-Subpopulation eine positive Korrelation, d.h. vor der Behandlung zeigten Responder eine signifikant erhöhte Frequenz von CD8+ NK-Zellen verglichen mit Non-Respondern. FAZIT Mit dieser Arbeit konnte zum ersten Mal gezeigt werden, dass eine detaillierte Immunphänotypisierung, insbesondere des NK-Zell-Kompartimentes, die Identifizierung eines sogenannten Baseline-Prädiktors zur Vorhersage des Therapieerfolgs bei anti- TNF-α-behandelten AS-Patienten erlaubte. Pathomechanistisch deuten diese Daten auf eine aktive Beteiligung von NK-Zellen im Allgemeinen und von CD8+ exprimierenden Subsets im Speziellen am chronischen Entzündungsgeschehen bei der AS hin.
