Einführung: Die Axiale Spondyloarthritis ist eine chronisch-entzündliche Erkrankung, die hauptsächlich die Sakroiliakalgelenke und Wirbelsäule betrifft. Ein pathologischer Knochenum/-anbau führt zu typischen radiografischen Veränderungen, den Syndesmophyten. Die daraus resultierende spinale Versteifung verursacht bei betroffenen Patienten funktionelle Einschränkungen. Die radiografische Progression verläuft individuell und die Vorhersage ist mangels geeigneter Prädiktoren erschwert. Neben klinischen Prädiktoren werden zunehmend Biomarker des Knochen- und Inflammationsstoffwechsels hinsichtlich ihres möglichen prädiktiven Wertes untersucht. Ziel dieser Arbeit war es, ein prädiktives Modell der radiografischen Progression bei axSpA mithilfe von Biomarkern und klinischen Prädiktoren zu entwickeln. Methoden: In diese Arbeit wurden 117 Patienten mit Ankylosierender Spondylitis aus der Effects of NSAIDs on RAdiographic Damage in Ankylosing Spondylitis (ENRADAS)- Studie eingeschlossen. Die radiografische Progression über 2 Jahre wurde definiert als a) eine Erhöhung des modified Stoke Ankylosing Spondylitis Spine Score (mSASSS) ≥2 Punkte oder b) eine Syndesmophyten- /Ankylosebildung. Bekannte klinische Prädiktoren zur Baseline wie das Vorhandensein von Syndesmophyten, eine C-reaktive-Protein-Erhöhung ≥5mg/l im Serum, Raucherstatus und das männliche Geschlecht wurden als klinisches Modell herangezogen. Die Serumkonzentrationen folgender Biomarker wurde durch Enzyme-Linked Immunosorbent Assays zur Baseline und nach 2 Jahren erhoben: Adiponectin und die High-molecular-weight-Form, Calprotectin, Chemerin, Leptin, Lipocalin-2, Matrix-Metalloproteinasen-3, N-terminales Telopeptid, Omentin, Osteoprotegerin, Procollagen Typ-II N-terminales Propeptid, Resistin, Sclerostin, Serum Amyloid-A, Vascular-Endothelial-Growth-Factor, and Visfatin. Die statistische Analyse umfasst für die Biomarker/-kombinationen lineare und logistische univariable und multivariable Regressionsanalysen sowie eine Kreuzvalidierung. Ergebnisse: Eine signifikante Assoziation zur mSASSS-Progression ≥2 Punkte zeigte die logistische Regressionsanalyse für Leptin und Vascular-Endothelial-Growth-Factor, einen statistischen Trend für Lipocalin-2 und Visfatin. Die Biomarker High-molecular-weight-Adiponektin und Sclerostin waren signifikant mit der Syndesmophytenbildung/-progression assoziiert, ein Trend konnte für Leptin beobachtet werden. In Kombination mit dem klinischen Modell konnte für alle Biomarker in der multivariablen Regressionsanalyse eine Verbesserung des Vorhersagewertes gezeigt werden. In der Kreuzvalidierung ergab sich durch Hinzugeben einzelner Biomarker zum klinischen Modell kein Mehrwert. Die Kombination aus den Biomarkern Leptin, High-molecular-weight-Adiponektin und Vascular-Endothelial-Growth-Factor zeigte einen höheren Vorhersagewert als das klinische Modell (Area-Under-the-Curve(AUC)Biomarker= 0,731, 95%-Konfidenzintervall(KI)=0,614-0,848 vs. AUCklinisches Modell=0.665, 95%-KI=0.553-0.776). In der Kombination beider Modelle zeigte sich eine Verbesserung des Vorhersagewertes, ohne statistische Signifikanz (AUCklinisches Modell+ Biomarker= 0,768, 95%-KI=0,666-0,871). Bei Betrachtung der partiellen AUC(pAUC) bei einer Spezifität von ≥75% (p=0,01; pAUCklinisches Modell+ Biomarker= 0,119 vs. pAUCklinisches Modell= 0,053) ergab sich ein signifikant erhöhter Vorhersagewert. Schlussfolgerung: Die Verwendung von Biomarkern zusätzlich zu klinischen Parametern kann zu einer Verbesserung der Prädiktion der radiografischen Progression führen.
Introduction: Axial Spondyloarthritis is a chronic inflammatory disease affecting mainly the sacroiliac joints and spine. Pathological bone remodeling leads to typical radiographic manifestations such as syndesmophytes. Spinal stiffness as result of this mechanism, manifests in functional impairment of the spine. The radiographic progression rate varies and is difficult to predict in lack of reliable predictors. Next to clinical predictors, studies identified biomarkers with a predictive value. The objective of this study was to design a model to predict radiographic spinal progression by combining biomarkers with clinical parameters. Methods: This study includes a total of 117 patients with ankylosing spondylitis, who completed the Effects of NSAIDs on RAdiographic Damage in Ankylosing Spondylitis (ENRADAS) trial. Radiographic spinal progression over 2 years was defined as a) worsening of the modified Stoke Ankylosing Spondylitis Spine Score (mSASSS) by ≥2 points or b) new or progression of syndesmophyte formation. Present syndesmophytes at baseline, elevated C-reactive-Protein ≥5mg/l, cigarette smoking and male sex formed the clinical prediction model. Serum levels were measured using enzyme-linked immunosorbent assays at baseline and after 2 years for following biomarkers: adiponectin and its high-molecular-weight-form, calprotectin, chemerin, leptin, lipocalin-2, Matrix-Metalloproteinasen-3, N-terminal telopeptide, omentin, osteoprotegerin, procollagen type-II N-terminal propeptide, resistin, serum amyloid-A, sclerostin, vascular-endothelial-growth-factor and visfatin. Results: Logistic regression analysis revealed significant associations of leptin and vascular-endothelial-growth-factor to mSASSS-progression ≥2 points, lipocalin-2 and visfatin showed a statistical trend. The biomarkers high-molecular-weight-adiponectin and sclerostin were significantly associated with syndesmophyte-progression, a trend could be shown for leptin. Adding these biomarkers to the clinical model resulted in a numerically improvement of the predictive value, though results were not significant. No benefit could be found, when single biomarkers were added to the clinical model in repeated-cross validation analysis. Biomarker combination of leptin, high-molecular-weight-adiponectin and vascular-endothelial-growth-factor showed a higher predictive value in comparison to the clinical model with an Area-Under-the-Curve (AUC)Biomarkers=0.731 (95%confidence intervaI (CI)=0.614-0.848) vs. AUCclinical model =0.665, 95%CI=0.553-0.776). Adding the biomarker combination to the clinical model enhanced the predictive value (AUCclinical model+biomarkers=0.768, 95%CI=0.666-0.871); though not statistically significant (p=0.051). Considering a partial AUC (pAUC) with specifity of ≥75% in Receiver-operating-characteristic (ROC)-analysis a significant increase of the predictive value (pAUCclinical modell + biomarkers=0.119; pAUCclinical model=0.053; p=0.01) could be revealed. Conclusion: A combination of biomarkers with clinical predictors showed a potential improvement for the prediction of radiographic spinal progression in axSpA.
