Hintergrund: Die Amyotrophe Lateralsklerose (ALS) gehört zu den schwersten neurologischen Erkrankungen. Durch eine spezialisierte Versorgung, insbesondere durch eine Beatmungs- und Hilfsmittelversorgung sowie Pharmakotherapie, kann eine Symptomkontrolle und Lebensverlängerung erreicht werden. Strukturierte Daten zur Versorgungsrealität sowie zu Implikationen für das Überleben sind in der ALS-Versorgung in Deutschland bislang unzureichend vorhanden. Ziel: Durch Kohortenstudien sollten systematische Informationen zur Versorgungsrealität bei der ALS generiert werden, um aktuelle Standards der Versorgung und Behandlungspfade in den jeweiligen Versorgungsbereichen zu identifizieren und Potentiale für eine Versorgungsoptimierung abzuleiten. Methoden: Drei Beobachtungsstudien wurden im Interkohorten-Vergleich realisiert und nach der STROBE-Methode berichtet. In der monozentrischen Studie zur Beatmungsversorgung wurden Längsschnittdaten von 2702 Patienten mit ALS (2007-2019) ausgewertet. In der multizentrischen Studie zur Pharmakotherapie wurden Querschnittsdaten von 2392 Patienten mit ALS (2013-2019) und in der Studie zur Hilfsmittelversorgung, ebenso multizentrisch, Längsschnittdaten von 1494 Patienten ausgewertet und analysiert. Ergebnisse: 21 % der Patienten mit ALS wurden mit einer nicht-invasiven Beatmungstherapie (NIBT) und 9,5 % mit einer invasiven Beatmungstherapie (IBT) versorgt. Die NIBT (mediane Überlebenszeit 41 Monate) und die IBT (Überlebenszeit 82 Monate) sind mit einer signifikanten Lebensverlängerung assoziiert (ohne Beatmungstherapie 34 Monate). Die Analyse der Pharmakotherapie zeigte eine hohe Komplexität der Medikation (3,2 pro Patient) sowie eine beachtliche Anzahl der symptomatischen Medikamente (99 Arzneimittel). Die Studie zur Hilfsmittelversorgung identifizierte einen erheblichen Bedarf von Hilfsmitteln der Mobilität (z. B. Elektrorollstühle, 40,5 %) und Kommunikation (z. B. Augensteuerungssysteme 13,1 %). Zugleich wurde eine Nicht-Versorgung (u. a. Ablehnung durch Kostenträger oder Patient, Tod des Patienten vor Versorgung) festgestellt (29,8 %). Schlussfolgerungen: Die Beatmungsversorgung, die Pharmakotherapie und die Hilfsmittelversorgung stellen häufige, komplexe und relevante Versorgungen bei der ALS dar. Für die Beatmungsversorgung konnte ein Überlebensvorteil gezeigt werden, der mit den Ergebnissen einer kontrollierten Studie korrespondiert. Zugleich ist eine Unterversorgung zu diskutieren (Inanspruchnahme der Beatmungstherapie 28 %). Die Determinanten der Inanspruchnahme von NIBT/IBT erfordern weiterer Untersuchungen. Bei der Symptomkontrolle der ALS nehmen die Pharmakotherapie und die Hilfsmittelversorgung eine zentrale Rolle ein. Die Ergebnisse der Kohortenstudien wurden international publiziert und leisten einen Beitrag, um die Versorgungsstandards sowie die Behandlungsrichtlinien der nationalen und europäischen ALS Fachgesellschaften weiter zu entwickeln.
Background: Amyotrophic Lateral Sclerosis (ALS) is one of the most severe neurological disorders. Specialised care, most notably ventilation therapy and provision of assistive technology devices as well as pharmacotherapy, can effect symptom control and prolongation of life. Up to the present day, data for Germany on the reality of care provision and on implications for survival are still wanting. Objective: Cohort studies are designed to generate systematic information on real-world care provision in ALS so as to identify the current standards of care and treatment paths in the respective care areas and to derive the potential for care optimisation. Methods: Three observational studies conducted as an inter-cohort comparison were realised and reported according to STROBE criteria. In the single-centre study on ventilation care, longitudinal data for 2,702 patients with ALS (2007-2019) were evaluated. In the multi-centre study on pharmacotherapy cross-sectional data for 2,392 patients with ALS (2013-2019) were evaluated and analysed, and in the study on the provision of assistive technology devices, likewise a multi-centre study, longitudinal data for 1494 patients were assessed. Results: 21% of patients with ALS received non-invasive ventilation (NIV), and 9.5% of patients were administered tracheostomy invasive ventilation (TIV). Both NIV (median survival of 41 months) and TIV (survival of 82 months) are associated with significant prolongation of life (34 months without ventilation). The pharmacotherapy analysis shows great variability of medication (3.2 per patient) and a remarkable number of symptomatic medicines (99 drugs). The study on the provision of assistive technology devices identified a substantial need for mobility devices (e. g. electric wheelchairs, 40.5%) and communication devices (e. g. eye and head control systems, 13.1%). In parallel, provision failure (i. e. rejection by health insurance or patient, death of patient prior to provision) was identified (29.8%). Conclusion: Ventilation therapy, pharmacotherapy and assistive technology devices are common, complex and relevant provision categories in ALS. A survival benefit was demonstrated for the provision of ventilation therapy, corresponding to the results of a controlled study. Furthermore, deficient provision is a subject matter for discussion (ventilation therapy utilisation, 28%). The determinants for NIV/IV need investigating further. In ALS symptom control, pharmacotherapy and provision of assistive technology devices are key factors. The results of the cohort studies were published at international level and make a contribution to the further development of standards of care and treatment guidelines issued by national and European expert ALS associations.
