Einleitung: „In dieser Beobachtungsstudie wurde ein alternatives Verfahren zur Aufrechterhaltung der Allgemeinanästhesie bezüglich Sicherheit und Praktikabilität getestet. Die Hypothese lautet, dass eine Kombinationsnarkose - bestehend aus Sevofluran-Inhalation, low-dose-Propofolinfusion (definierte Zielkonzentration 1-1,5 µg·ml-1 im Target-controlled-Infusion-Modus) und Remifentanilgabe - im Vergleich zur totalen intravenösen Anästhesie (TIVA) mit Propofol und Remifentanil weniger unerwünschte Begleiteffekte zeigt. [1]“
Methoden: „Die Kombinationsnarkose wurde bei 270 konsekutiven Patienten, die sich einer elektiven Operation unterzogen haben, durchgeführt. Um eine möglichst strukturgleiche Kontrollgruppe zu kreieren, wurden nach dem statistischen Verfahren der Matched-Pair-Analyse 270 prospektiv untersuchte Patienten ausgewählt, bei denen die Aufrechterhaltung der Narkose mit einer TIVA erfolgte. Matching-Kriterien waren ASA-Klassifikation, Geschlecht, Body-Mass-Index, Alter, Operation und PONV (postoperative Übelkeit und Erbrechen)-Risiko. Die Hypnotikasubstitution wurde in beiden Studiengruppen mittels EEG-Signal gesteuert. [1]“ Als primärer Endpunkt wurde die Häufigkeit des Auftretens von PONV gewertet. Eingeschlossen wurden Patienten, die sich zwischen Januar 2015 - Februar 2016 einer elektiven Operation mit einer zu erwartenden Narkosedauer > 30 Minuten unterzogen haben. Es wurden perioperative (prä-, intra- und postoperative) Daten gesammelt.
Ergebnisse: Es konnten 270 Patienten pro Gruppe (540 insgesamt) in die Studie eingeschlossen werden. „Alle Kombinationsnarkosen konnten problemlos (praktikabel) und ohne anästhesiebedingte Zwischenfälle (sicher) durchgeführt werden.“ Bei gleicher Narkosetiefe (Narcotrend-Index: TIVA = 41, Kombinations-Gruppe = 43) betrug die Propofol-Dosierung in der TIVA-Gruppe im Median 8 (IQR 7-9, range 5-14) mg·kg-1·h-1, während in der Kombinations-Gruppe die Propofol-Dosierung im Median bei 2 (IQR 2-2, range 1-3) mg·kg-1 ·h-1 lag. „Die minimale alveoläre Konzentration von Sevofluran (Luft-Sauerstoff-Gemisch) in der Kombinationsnarkose-Gruppe betrug 0,4 (0,3-0,6 [0,2-1,0]) Vol %. In der Kombinationsnarkose-Gruppe wurden 0,22 (0,16-0,28 [0,05-0,48]) μg·kg-1·min -1 Remifentanil und in der TIVA-Gruppe 0,25 (0,19-0,32 [0,05-0,62]) μg·kg-1 ·min-1 Remifentanil verabreicht (n.s.). Signifikante Gruppenunterschiede zeigten sich hinsichtlich der PONV-Rate (Kombinationsnarkose: 59/270, TIVA: 84/270, p = 0,015), bezüglich des Auftretens unerwünschter Spontanbewegungen der Patienten während der Operation (Kombinationsnarkose: 8/270, TIVA: 38/270, p < 0,001), beim Vasopressorenbedarf (Kombinationsnarkose: 121/270, TIVA: 184/270, p < 0,001) und bei der Extubationszeit (Kombinationsnarkose: 8 (6-11 [2-15]) min, TIVA: 10 (8-12 [2-22]) min, p = 0,012). [1]“ 21 % der TIVA-Gruppe (n = 57/270) und 13% der Kombinations-Gruppe (n = 38/270) mussten antiemetisch behandelt werden (p = 0,012).
Schlussfolgerungen: Die Kombinationsnarkose ist ein gutes und sicheres Anästhesieverfahren. „Die Verminderung von PONV-Rate und Spontanbewegungen sowie die größere hämodynamische Stabilität zeigen, dass es eine Rationale für die kombinierte Anwendung von volatilen Anästhetika und Propofol zur Aufrechterhaltung der Narkose gibt. [1]“
Introduction: “In this observational study the feasibility of an alternative technique to maintain general anaesthesia was clinically evaluated. We tested the hypothesis that the combined use of inhaled sevoflurane (oxygen/air mixture), low-dose propofol and remifentanil infusion is associated with a reduced number of side effects compared to a total intravenous anaesthesia (TIVA) using propofol and remifentanil.“
Methods: “In 270 consecutive patients anaesthesia was performed using sevoflurane, low-dose propofol (constant infusion rate: 1 μg·ml-1 in the target controlled infusion mode) and remifentanil (combined anaesthesia). A group of 270 prospectively examined patients undergoing TIVA served as the control (matched-pair design). Matching criteria were ASA classification, gender, body-mass-index, age, surgical procedure and PONV (post operative nausea and vomiting) risk. The depth of anaesthesia was controlled in both groups using EEG (Narcotrend monitor). [1]“ Primary endpoint was the prevalence of PONV. Patients who underwent elective surgery between January 2015 - February 2016 with an expected narcosis > 30 minutes were included. Perioperative (pre-, intra- and postoperative) data was collected.
Results: 270 patients were included per group (overall 540). “All combination anaesthesias were carried out (safely) without complications or occurrence of anaesthesia-related instances. The minimum alveolar sevoflurane concentration in the combined anaesthesia group was 0.4 (0.3-0.6 [0.2-1.0]) vol%. The median propofol dose in the TIVA group was 8 (7-9 [5-14]) mg·kg-1·h-1. The remifentanil dose was 0.22 (0.16-0.28 [0.05-0.48]) μg·kg-1 ·min-1 in the combined anaesthesia group and 0.25 (0.19-0.32 [0.05-0.62]) μg·kg-1·min-1 in the TIVA group (n.s.). Significant differences between groups were seen regarding PONV rate (combined Anaesthesia: 59/270, TIVA: 84/270, p = 0.015), patient’s movements during anaesthesia (combined anaesthesia: 8/270, TIVA: 38/270, p < 0.001), need for vasopressors (combined Anaesthesia: 121/270, TIVA: 184/270, p < 0.001), and extubation time (combined anaesthesia: 8 (6-11 [2-15]) min., TIVA: 10 (8-12 [2-22]) min., p = 0.012).“ Antiemetic treatment was required more often in the TIVA group 21 % (n = 57/270) vs. 13 % (n = 38/270) in the combined anaesthesia group (p = 0.012).
Conclusion: The combined anaesthesia is a good and safe anaesthetic procedure. “The combined use of sevoflurane, low-dose propofol and remifentanil for the maintenance of anaesthesia seems to be a feasible alternative to TIVA, with additive effects of both hypnotics possibly reducing the number of side effects such as PONV rate, patient’s movements during anaesthesia or haemodynamic instability.[1]“
