Zusammenfassung Hintergrund Die auf der Intensivstation erworbene Muskelschwäche (engl. intensive care unit-acquired weakness (ICUAW)) ist eine häufige neuromuskuläre Komplikation bei kritisch kranken Patienten auf Intensivstationen und ist durch eine generalisierte Muskelschwäche charakterisiert. Die damit verbundene verminderte Muskelkraft und Ausdauer resultieren häufig in langen Aufenthalten auf Intensivstationen und erhöhen die Gefahr von Sekundärkomplikationen. Das proinflammatorische Zytokin Interleukin 1β (IL-1β) spielt in Inflammationsreaktionen eine wichtige Rolle. Allerdings ist es unklar, inwiefern IL-1β an Muskelzellen wirkt und ob seine Inhibition eine Muskelatrophie verhindert. In dieser Arbeit soll der Einfluss der IL-1β-Aktivierung durch das NLRP3-Inflammasom in inflammationsvermittelter Muskelatrophie untersucht werden. Methoden Die Wirkung von IL-1β wurde in der Zellkultur an C2C12-Myotuben hinsichtlich einer Muskelatrophie auf Gen- und Proteinebene untersucht sowie die Atrophiereaktion morphologisch analysiert. Mittels Zökumligatur mit nachfolgender Punktion (CLP) wurde eine polymikrobielle Sepsis in Wildtyp- und Nlrp3-Knockout-Mäusen induziert. Anschließend wurde die Muskelatrophie anhand der Muskelmorphologie, der Genexpression und des Proteingehaltes der Atrophiemarker beurteilt. Ergebnisse Der IL-1β Signalweg ist in C2C12 Myotuben vorhanden und aktiv. Die Behandlung mit IL-1β führte zu einer gesteigerten Expression von muskelspezifischen Atrogenen und einer Atrophie von C2C12 Myotuben. Außerdem konnte gezeigt werden, dass Nlrp3-Knockout-Tiere, die eine verminderte Aktivierung von IL-1β aufweisen, gegenüber Wildtyptieren vor CLP-induzierter Muskelatrophie geschützt sind. Neben einem verminderten Entzündungsgeschehen konnte mittels Muskelmorphologie, Organ-gewichte, Genexpression und Proteinanalyse 96 Stunden nach CLP-Operation eine signifikant reduzierte Skelettmuskelatrophie in Nlrp3-Knockout-Tieren festgestellt werden. Die vorliegenden Daten weisen darauf hin, dass IL-1β direkt auf Muskelzellen einwirkt und in diesen Zellen Atrophie induziert. Die Hemmung der IL-1β-Aktivierung durch das NLRP3-Inflammasom könnte daher ein neues Ziel einer medikamentösen Therapie in der Behandlung der septischen Myopathie bei kritisch Kranken darstellen.
Background Intensive care unit-acquired weakness (ICUAW) is a common neuromuscular complication occurring in critically ill ICU-patients and is characterized by generalized muscle weakness. The associated decreased muscle strength and endurance often results in long stays in intensive care units and increases the risk of secondary complications. The proinflammatory cytokine interleukin 1β (IL-1β) plays an important role in inflammatory reactions. However, it remains unclear whether IL-1β acts directly on muscle cells and if its inhibition prevents muscle atrophy. In this work, the influence of IL-1β activation through the NLRP3 inflammasome is involved in inflammation- induced atrophy will be investigated. Methods The effect of IL-1β on C2C12 myotubes was examined in vitro for muscle atrophy at gene and protein level, and the atrophy response was analyzed morphologically. Polymicrobial sepsis was induced by celiac ligation followed by puncture (CLP) in wild-type and Nlrp3 knockout mice. Subsequently, muscle atrophy was assessed by muscle morphology and gene expression and protein content of atrophy markers. Results IL-1β signaling pathway is present and active in C2C12 myotubes. Treatment with IL-1β induces muscle atrophy with elevated gen expression of Il6 and muscle specific atrogens in C2C12 myotubes. Additionally Nlrp3 knockout mice with reduced IL-1β- activation are protected from CLP-induced muscle atrophy compared to their wild-type littermates. Besides reduced inflammation muscle morphology, organ weights, gene expression and protein levels revealed significantly reduced skeletal muscle atrophy in Nlrp3 knockout mice 96 hours after CLP surgery. These data indicate that IL-1β acts directly on muscle cells to cause atrophy. The inhibition of IL-1β activation by the NLRP3 inflammasome could therefore be a new drug target in the treatment of septic myopathy in critically ill patients.
