Einleitung Diese Promotionsarbeit umfasst vier in internationalen Fachzeitschriften publizierte Arbeiten, die sich mit der Therapie, den Nebenwirkungen und der Pathogenese der Immunthrombozytopenie befassen. Die Immunthrombozytopenie ist eine Autoimmunerkrankung mit vorrübergehender oder persistierender isolierter Thrombozytopenie mit Thrombozytenzahlen < 100 x 109/l. Die Erkrankung wird in eine primäre Form ohne ursächliche Auslöser und eine sekundäre Form unterteilt. Zu den Auslösern der sekundären Immunthrombozytopenie zählen z.B. Infektionen, Malignome, Medikamente und systemische Autoimmunerkrankungen. Es wird angenommen, dass etwa 80% der Patienten an der primären Form leiden. Autoantikörper vermittelter Thrombozytenabbau und eine verringerte Thrombozytopoese führen bei den betroffenen Patienten zur Thromobzytopenie. Klinisch kann sich die Erkrankung in Form einer kutanen bzw. mukokutanen Blutungsneigung (z.B. Petechien, Ecchymosis, Hämatome, Epistaxis, Hypermenorrhoe) und selten im Rahmen von inneren Blutung (z.B. intrakranielle Blutung, gastrointestinale Blutung) manifestieren. Ob und mit welcher Therapie ein Patient mit Immunthrombozytopenie behandelt werden muss, ist abhängig von vielen Faktoren, z.B. Blutungsneigung, Thrombozytenzahl, Alter, Beruf und Komorbiditäten. Die Auswahl einer geeigneten Therapie bleibt trotz neuer Therapieoptionen in vielen Fällen fraglich. Methodik Für die hier präsentierten Studien wurden die Daten von 400 Patienten (143 Männer und 257 Frauen) mit einem mittleren Alter von 50,5 Jahren (Spannweite 3 – 101 Jahre), die die aktuellen Diagnosekriterien der Immunthrombozytopenie erfüllten retrospektiv untersucht. Alle Patienten wurden an der Charité – Campus Virchow Klinikum am Institut für Transfusionsmedizin von einem Arzt zwischen 1996 und 2016 ambulant behandelt. Zur Patientenerfassung erfolgte die Auswertung der Krankenakten aller der im o.g. Zeitraum behandelten Patienten. Die betreffenden Patientendaten wurden mittels Microsoft Excel (Windows) tabellarisch erfasst und analysiert. Schlussfolgerung Die Anzahl der Patienten mit primärer Immunthrombozytopenie ist geringer als bisher angenommen. Zudem muss man annehmen, dass die Immunthrombozytopenie selbst, ursächlich für die Entwicklung sekundärer Erkrankungen sein kann
This thesis includes four papers published in international scientific journals. It is dealing with therapy, adverse reactions and pathogenesis of immune thrombocytopenia. Immune thrombocytopenia is an autoimmune disease characterized by isolated transient or persisting thrombocytopenia with platelet counts < 100 x 109/l. The disease can be classified into a primary and a secondary form. The triggers of secondary immune thrombocytopenia may include infections, malignancies, drugs and systemic autoimmune diseases. It is believed that approximately 80% of the patients suffer from primary immune thrombocytopenia. Antibody mediated platelet destruction as well as impaired platelet production are the mechanisms leading to thrombocytopenia. Low platelet counts may result in cutaneous or mucocutaneous bleeding (eg. petechiae, ecchymosis, hematoma, epistaxis, hypermenorrhea) or in rare cases also in internal bleeding (eg. intracranial bleeding, gastrointestinal bleeding). Whether a patient requires therapy and which therapy is preferable depends on many aspects, e.g. bleeding propensity, platelet count, age, profession/lifestyle, comorbidities and many more. Despite the availability of new treatment options it remains frequently questionable in many cases how to treat patients. Methods: In the presented studies the data of 400 patients (143 men and 257 women) with an average age of 50.5 years (range 3–101 years), which fulfilled the current diagnostic criteria of immune thrombocytopenia, were analyzed. All patients were treated at the Charité - Campus Virchow Klinikum at the Institute for Transfusion Medicine by one physician between 1996 and 2016 on an outpatient basis. All medical charts, of the patients treated in the aforementioned period were analyzed and tabulated using Microsoft Excel (Windows). Conclusion: The incidence of primary immune thrombocytopenia is less common than has been suggested. Furthermore, there is evidence that immune thrombocytopenia itself might be responsible for the development of secondary diseases. The number of treatment options for ITP is still growing and various reviews and guidelines on the management of immune thrombocytopenia are available. Nevertheless, treatment decisions remain challenging in many cases. In addition, the majority of available therapies may result in intolerable or even life-threatening adverse reactions. It is also not predicable which therapy is effective for each individual patient. Similar to other autoimmune diseases, there is still no specific or invariably curative therapy for immune thrombocytopenia.
