Ziel dieser Arbeit war die systematische Untersuchung der kontinuierlichen
Kälteanwendung mittels Kryopumpe nach verschiedenen Vorderfuss - Osteotomien
und ihr Einfluss auf das Schmerzempfinden, die Schwellneigung und auf
verschiedene Laborparameter (CRP, Hb usw.). Das Kältesystem basiert auf einer
Geräteeinheit, an die eine, der Topographie der zu kühlenden Region angepasste
Manschette, über ein Schlauchsystem angeschlossen wird. Im Gerät wird die
Temperierflüssigkeit gekühlt, um anschließend den Patienten im Kreislauf
-Gerät und Manschette- zu durchströmen und ihn mit seiner entsprechenden
Kryotherapie zu versorgen. Mit dieser Behandlungsoption können tiefer gelegene
Gewebeschichten beeinflusst werden. Der Hauptangriffspunkt der
Kryolangzeittherapie liegt derzeit in der Reduktion des postoperativen
Schmerzes. Neben der reaktiven Hyperthermie der Muskulatur und der Hemmung
verschiedener enzymatischer Prozesse, werden durch die Verlängerung der
Refraktärperiode und der Repolarisationsvorgänge die
Nervenleitgeschwindigkeiten von sensiblen - und motorischen Nervenfasern
herabgesetzt. Zusätzlich kann über eine Verkleinerung der Endothelspalten und
eine Tonuserhöhung der Gefäße im Mikrozirkulationsbereich ( cappilary oozing
) eine postoperative Ödembildung verhindert werden. Nach Aufbau des Systems,
postoperativer Lagerung der Patienten und vorausgegangener Randomisierung in
drei Gruppen (Gruppe 1: Kontrolle, Gruppe 2: Kryolangzeit und Gruppe3:
Celecoxib), erhielten die Patienten der Gruppe 2 die entsprechende Manschette
am Vorfußbereich. Die operierten Bereiche der Studienteilnehmer der Gruppe 1
und der Gruppe 3 wurden intermittierend (bei Bedarf) mit Kryolongbeuteln
gekühlt. Die Kryopumpe wurde in einem 18-monatigen Untersuchungszeitraum in
einer prospektiven, randomisierten Studie bei 28 von 81 Patienten angewendet.
Vorausgegangen waren bei 48 Patienten eine Chevron- und bei 33 Patienten eine
Scarf - Osteotomie. Als Studienergebnis war festzustellen, dass die Patienten
der Kryogruppe 2 sowohl das geringste subjektive Schmerzempfinden mit dem
entsprechend niedrigsten Analgetikaverbrauch, als auch die geringste
Schwellneigung postoperativ aufwiesen. Daraus resultierend konnte diese
Patientengruppe im Mittel die Klinik bis zu 1,5 Tagen eher verlassen, als die
Patienten der Gruppen 1 und 3. Mit dem Einsatz der Kryopumpe assoziierte
Komplikationen wie Kälteurtikaria oder das Vegetativum negativ beeinflussende
Faktoren, konnten im gesamten Untersuchungszeitraum nicht beobachtet werden.

dc.description.abstract
The aim of this study was the clinical evaluation of continued coldness-
application using a cryopump after different forefoot-osteotomics and its
stake in sensation of pain, the addiction of turgor and different laboratory
parameter (CRP, Hb etc.). The system of coldness utilises an unit of
equipment. It includes an adapted tollar which is connected at a flexible
tube-system. The unit cools down the fluid for accommodating the patients with
their congruent therapy. The main issue of cryotherapy currently runs in the
reduction of postoperative pain. Other than that reactive hyperthermics of the
muscles is the inhibition of different enzymatics processes. Additional the
capillary oozing avoids the addiction of turgor postoperative. After the
randomisation into three groups ( 1st group: control, 2nd group: cryotherapy
and 3rd group: celecoxib) the patients of the 2nd group achieved the adapted
tollar at their forfeet. The patients of the 1st and the 3rd group achieved
intermittendly coolpacks. The cryopump was utilised in a period of 18 months
in a prospective, randomized study on 28 of 81 patients. Time before 48
patiens got a chevron-osteotomie the rest of 33 patients got a scarf
osteotomie. The mean registrations were the lowest sensation of pain according
to the lowest consumption of analgetics and the lowest addiction of turgor
regarding the patients of the 2nd group. In the result these patients could
leave the hospital 1,5 days earlier in average. No system associated
complications such as urticaria of coolness or any side-effects in terms of
cardiovascular diseases could be observed.
