EINLEITUNG UND FRAGESTELLUNG: Die DNA-Methyltransferasen DNMT1, DNMT3a und
DNMT3b katalysieren die Methylierung von DNA, welche über komplexe
epigenetische Prozesse zur Komprimierung und Inaktivierung des entsprechenden
DNA-Abschnitts führt. Im physiologischen Zustand findet sich im Genom eine
globale Hypermethylierung bei Hypomethylierung der Promotor-Regionen von
Genen. In der Tumorgenese hingegen werden diese Promotoren vor allem bei
Tumorsuppressorgenen häufig hypermethyliert und damit inaktiviert, während das
übrige Genom aberrant hypomethyliert ist. Bei einigen Tumoren korreliert nach
heutiger Studienlage die verstärkte Expression von DNMTs in Tumorzellen mit
dieser Hypermethylierung. DNMT-Inhibitoren konnten teilweise in Phase-I- bis
III-Studien Hypermethylierungen aufheben. Die DNMT-Expression bei AML und ALL-
Rezidiv im Kindesalter wurde bislang nicht analysiert. In dieser Arbeit wurde
die Expression der für die aberrante Methylierung in Tumorzellen
hauptverantwortlichen Enzyme DNMT1 und DNMT3b inklusive ausgesuchter
Spleißvarianten bei ALL-Erstrezidiv und AML-Ersterkrankung im Kindesalter im
Vergleich zu gesunden Probanden analysiert und untersucht, inwieweit diese
eine prognostische Bedeutung hat. MATERIAL UND METHODEN: Es wurden Tumorzellen
von 26 Patienten mit ALL-Erstrezidiv (davon 7 in CCR und 19 mit Folgeereignis)
sowie 47 Patienten mit AML-Ersterkrankung (davon 34 in CCR und 13 mit
Folgeereignis) im Vergleich mit Leukozyten von 22 gesunden Probanden
hinsichtlich der DNMT-Expression untersucht. Nach Aufreinigung der Zellen
wurde die RNA isoliert und mittels reverser Transkription in cDNA
umgeschrieben; diese wurde durch eine Real-Time-PCR nach Etablierung einer
Standardreihe gegen 2 Referenzgene (RpL13a als konstant exprimiertes Gen und
PCNA als Proliferationsmarker) quantifiziert. ERGEBNISSE: Die Expression von
DNMT1 ist sowohl im ALL- als auch im AML-Kollektiv im Vergleich zum
Kontrollkollektiv normalisiert zu RpL13a nicht verändert und normalisiert zu
PCNA vermindert. DNMT3b ist im ALL-Kollektiv in Normalisierung gegen RpL13a
verstärkt und in Normalisierung gegen PCNA nicht verändert. Im AML-Kollektiv
ist DNMT3b gegenüber beiden Referenzgenen verstärkt exprimiert. Der Anteil der
Spleißvariante DNMT1’ an der DNMT1-Gesamtexpression liegt in allen Kollektiven
konstant bei rund 90%. Die übrigen Spleißvarianten von DNMT1 und DNMT3b wurden
nicht in die Analyse einbezogen, da sie nicht in allen Proben nachweisbar
waren. DNMT3b4 konnte in rund 70% der ALL- und AML-Proben nachgewiesen werden,
jedoch nicht im Kontrollkollektiv. Prognostisch relevante
Expressionsunterschiede wurden nicht gefunden. DISKUSSION: Die
DNMT1-Expression in ALL- und AML-Zellen ist im Vergleich zum Kontrollkollektiv
trotz der verstärkten Proliferationsaktivität nicht verändert. Diese somit
vergleichsweise niedrige Expression in leukämischen Zellen ist ein möglicher
Faktor für die Hypomethylierung neu synthetisierter DNA. DNMT3b ist in AML-
Zellen gegenüber beiden Referenzgenen verstärkt exprimiert. Dies könnte
einerseits zur Hypermethylierung von Promotor-Regionen führen, andererseits
über Konkurrenz mit DNMT1 ebenfalls zur Hypomethylierung beitragen,
beispielsweise durch die Spleißvariante DNMT3b4, der diese Eigenschaft bei
anderen Tumoren zugeordnet werden konnte. Möglicherweise stellt die verstärkte
Expression ein relevantes Entscheidungskriterium zur Therapie beispielsweise
des AML-Erstrezidivs mit DNMT-Inhibitoren sein, deren Wirkung vor allem über
Enzyme der DNMT3-Familie entfaltet wird. In dieser Arbeit konnte somit eine
gegenüber physiologischen Verhältnissen deutlich aberrante Expression der DNA-
Methyltransferasen bei ALL und AML im Kindesalter festgestellt werden.

INTRODUCTION AND AIM OF STUDY: The DNA methyltransferases DNMT1, DNMT3a, and
DNMT3b catalyze DNA methylation. This leads to compression and inactivation of
the methylated DNA region via complex epigenetic mechanisms. Under
physiological conditions, the genome is globally hypermethylated, whereas gene
promoters tend to be hypomethylated. In tumorigenesis these promoters,
especially those of tumor-suppressor-genes, often undergo methylation and
consecutive inactivation, while the rest of the genome is aberrantly
hypomethylated. Promoter hypermethylation is shown to correlate with increased
DNMT expression in some tumors. Promoter hypermethylation could be partially
reversed by DNMT inhibitors in Phase I - III studies. DNMT expression has not
yet been analyzed in pediatric AML and relapse of ALL. In this study, the
expression of DNMT1 and DNMT3b (the DNMTs which are considered to be mainly
responsible for aberrant methylation in tumor cells) and the expression of
selected isoforms was analyzed in newly diagnosed pediatric AML and relapse of
pediatric ALL. The expression was compared to that in healthy subjects.
Furthermore, the prognostic role of DNMT expression was examined. MATERIAL AND
METHODS: The DNMT expression in tumor cells of 26 patients with first relapse
of pediatric ALL (7patients in CCR and 19 patients with subsequent event) and
47 patients with newly diagnosed pediatric AML (34 patients in CCR and 13
patients with subsequent event) was examined and compared to the expression in
leukocytes of 22 healthy subjects. After cell filtration RNA was isolated and
reversely transcribed into cDNA, which was quantified by Real-Time-PCR after
establishment of standard curves and normalized with 2 reference genes (RpL13a
served as a stably expressed gene and PCNA as a proliferation marker).
RESULTS: Expression of DNMT1 is not altered in leukemic cells compared to the
control group if normalized with RpL13a, whereas it is decreased if normalized
with PCNA. Expression of DNMT3b is increased in ALL if normalized with RpL13a
but not altered if normalized with PCNA. In AML cells, DNMT3b expression is
increased after normalization with both reference genes. The isoform DNMT1’
accounts for 90% of the entire DNMT1 expression in all groups. All other
isoforms were not included into the statistical analysis since they were not
detectable in all of the samples. DNMT3b4 was detected in 70% of the samples
of the ALL- and AML-group, respectively, whereas it was not detected in the
control group. No differences were found between the subgroups with different
outcome. DISCUSSION: Expression of DNMT1 is not altered in the examined
leukemic cells compared to the control group despite the elevated level of
proliferation. This relatively low level of expression in leukemic cells might
contribute to the hypomethylation of newly synthesized DNA. DNMT3b expression
is elevated in AML-cells after normalization with both reference genes. This
might lead to aberrant hypermethylation of promoter regions as well as to
hypomethylation by DNMT3b4 via competition with DNMT1, which has been shown in
other entities. The elevated expression might be a relevant parameter to
identify AML patients who could profit from DNMT-inhibitor-therapy, since the
effects of DNMT inhibitors are mainly developed via DNMT3-enzymes. In
conclusion, a distinct aberration of DNMT expression in pediatric AML and ALL
was found in this study.
