Nach einer Tibiafraktur ist eine intensive Kontrolle des
Frakturheilungsverlaufs wichtig; da es sich oft um Frakturen mit
Knochenverlust handelt und eine schnelle Rehabilitation dadurch gefährdet sein
kann. Fakt ist; dass trotz aller Fortschritte in der Frakturversorgung bei bis
zu zwanzig Prozent der Frakturtherapien immer noch Heilungsstörungen
auftreten; was die fortdauernde Forschung auf diesem Gebiet rechtfertigt
(Haas; 2000). Die Analyse von Serumparametern wird bei metabolischen
Erkrankungen schon lange mit guten Ergebnissen eingesetzt. Die Analyse von
Frakturheilungsprozessen anhand von verschiedenen Serumparametern führte aber
zu kontroversen Ergebnissen (Delmas et al.; 1997; Eriksen et al.; 1995; Kurdy
et al.; 1998; Yu-Yahiro et al.; 2001). Deshalb wurden in dieser Studie sieben
Serumparameter in einem standardisierten Modell verglichen. Die sieben
beschriebenen Serumparameter wurden am Schaf während der Heilung einer
Tibia¬fraktur simuliert durch eine Tibiaosteotomie getestet; wobei die Enden
der osteotomierten Tibia drei Millimeter distrahiert wurden. Die Fraktur wurde
mit einem Fixateur externe stabilisiert. Der Versuchszeitraum betrug neun
Wochen und in diesem heilte die Tibiaosteo-tomie klinisch aus. Den Schafen
wurde einmalig vor der Osteotomie und wöchentlich nach dieser Blut entnommen.
Die ermittelten Serumwerte wurden mit den Ergebnissen der histologischen
Untersuchung der heilenden Osteotomie zu vier Zeitpunkten nach der Operation
verglichen. Alle Serumparameter zeigten große inter-individuelle Schwankungen
und einen Abfall der Werte über den Untersuchungszeitraum von neun Wochen;
auch bei den Tieren der Kontrollgruppe. Der systemische Knochenstoffwechsel
ist auch bei gesunden Tieren sehr hoch; da sich der Knochen ständig der
Belastung anpasst und zudem als Mineralienreservoir dient. Die relativ kleine
Osteotomie der Tibia hatte keinen sichtbaren Einfluss auf diesen
Gesamtstoffwechsel bzw. die Erhöhung des Knochenstoffwechsels durch die
Fraktur ging darin unter. Nur im Vergleich mit der Kontrollgruppe und in
Relation zu dem präoperativen Wert war ein geringer Anstieg bei einigen
Knochenmarkern zu ermitteln. Die Ergebnisse der Knorpelmarker zeigten eine
gute Korrelation mit dem im histologischen Päparat sichtbaren Heilungsverlauf
und wurden weniger vom allgemeinen Körperstoffwechsel maskiert. Aber auch hier
gab es einen Abfall der Knorpelparameter während des Studienverlaufs; sowohl
bei den operierten Tieren als auch bei denen der Kontrollgruppe. Die Serum-
Werte der juvenilen Tiere lagen allerdings deutlich über denen der operierten
Gruppe; was durch den Abbau der Wachstumsfugen begründet werden kann. Eine
klinische Nutzung der in dieser Studie untersuchten Serumparameter zur
Beurteilung der Frakturheilungsprozesse ist nicht möglich; da nur in
Verbindung mit dem präoperativen Wert und den Werten der Kontroll¬gruppe eine
Aussage über den Verlauf der Werte möglich war; was im klinischen Alltag nicht
durchzuführen ist. Es ist festzuhalten; dass eine Bewegungseinschränkung nach
einer Osteotomie großen Einfluss auf die Serumparameterspiegel zu haben
scheint und dass die individuellen Unterschiede einen klinischen Nutzen in der
Frakturdiagnostik zur Zeit unmöglich machen.

A fracture of the tibia demands an intensive monitoring of the fracture
healing processes; because in many cases these fractures are combined with a
loss of bone and; therefore; the rehabilitation can be delayed. As a matter of
fact; up to twenty percent of the fracture therapies interfer with the healing
process; despite taking into account the advances in orthopedic surgery; and
this demands ongoing medical research in this area (Haas; 2000). The analysis
of the serum parameters examined in this study is used for the diagnosis of
metabolic diseases for a long time. The analyses of the same serological
parameters to monitor the progressing fracture healing gave controversial
results (Delmas et al.; 1997; Eriksen et al.; 1995; Kurdy et al.; 1998; Yu-
Yahiro et al.; 2001). To elucidate this contradiction; seven serum parameters
were compared in this study in a standardized model of a fracture; the
osteotomy of the tibia in sheeps; to monitor the fracture healing processes.
The fracture; i.e; the osteotomy of the tibia with an artificial gap of 3 mm;
was taking care of using an external fixateur. The tests were conducted over a
period of nine weeks; whererin the tibia healed completely. Blood samples were
taken from the sheep one time before the osteotomy and afterwards on a weekly
basis. The serum parameters found; were compared with the histologic structure
of the regenerating fracture two; three; six; and nine weeks after the
orthopedic surgery. All serum parameters showed a big inter-individual
variation and reduction of the serum level during the nine weeks of the study.
This was valid for the sheep in the reference group too. The systemic
metabolism of bones is very active even in healthy animals; because the bones
permanently react to changes of the mechanical forces and; in addition; are
the reservoir for various minerals. The relatively small fracture; i.e.; the 3
mm gap osteotomy; had no identifiable impact on the overall metabolism or the
increase of bone metabolic processes were too small to be detected. Only in
comparison to the reference group and in relation to the preoperative values;
a small increase in some of the bonemarkers was detected in the serum of the
sheep. The results of the serum markers indicative for metabolic processes in
cartilage tissue showed a good correlation with the histological structure in
the healing process and were only partially masked by the overall metabolism
of the body. A reduction of the serum levels of the chondral parameter during
the study was found in the reference group as well as in the group of treated
animals. The serum levels of these parameters indicative for the metabolism in
cartilage were significantly higher in juvenile sheeps than in the sheeps
after orthopedic surgery. This may be explained by the degeneration of the
epiphyseal cartilages in the juvenile sheeps. Based on the results of this
study; the clinical application of the serum parameter to assess the progress
of fracture healing processes is not advisable; because only the comparison of
the preoperative serum levels with those in the reference group provided
confidence to predict the progress of the fracture healing process. In
addition it has to be stated; that the reduction of mobility after the
osteotomy of the tibia seems to have a major impact on the level of the serum
parameters and that the individual differences of the levels prohibt a
clinical application of this kind of diagnostics of the fracture healing.
