Bei der Behandlung schizophrener Patienten ist eine durch Antipsychotika
ausgelöste Gewichtszunahme eine häufige Nebenwirkung, die oft zu einer
Noncompliance hinsichtlich der Medikation führt. Die Entwicklung eines
Testverfahrens zur Vorhersage des individuellen Risikos für die
antipsychotika-induzierte Gewichtszunahme wird mit Nachdruck verfolgt. Ein in
diesem Zusammenhang durch seine zentrale Rolle im Energiestoffwechsel
interessantes Gen ist das Neuropeptid Y-Gen. Es sind bereits diverse
Veränderungen im Neuropeptid Y-System unter einer Antipsychotikatherapie
beobachtet worden. Die hier zu beantwortende zentrale Frage ist, ob
Polymorphismen des Neuropeptid Y-Gens einen Einfluss auf eine antipsychotika-
induzierte Gewichtszunahme haben. Hierzu wurden fünf Neuropeptid
Y-Polymorphismen (rs10551063, rs16147, rs5573, rs5574 und rs16475) in
Blutproben schizophrener Patienten (n=93) typisiert. Die Analyse möglicher
Kovariaten/Faktoren zeigte signifikante Befunde für das Ausgangsgewicht, die
Medikation und die Teilnahmedauer an der Studie. Unter Berücksichtigung der
gefundenen Zusammenhänge wurden anschließend Genotyp-, Allel- und
Haplotypanalysen durchgeführt. Es ergaben sich signifikante Befunde für die
Patientensubgruppen mit einer Risperidon-/Quetiapintherapie (n = 30) und mit
einer Clozapin-/Olanzapintherapie (n = 26). Bei Patienten mit einer
Risperidon-/Quetiapintheapie nahmen Träger das C-Allels des
rs16147-Polymorphismus im Vergleich zu homozygoten T-Allelträgern weniger zu.
Analoge signifikante Ergebnisse ergaben sich auch für den C-A-T- und den
GT-C-A-T-Haplotyp. Konträr hierzu nahmen Träger des –-Allels des
rs10551063-Polymorphismus, des T-Allels des rs16147-Polymorphismus und des
G-Allels des rs5573-Polymorphismus, die eine Clozapin-/Olanzapintherapie
erhielten, im Vergleich zu den jeweils korrespondierenden homozygoten
Genotypen signifikant weniger an Gewicht zu. In den Haplotypanalysen wiesen
der C-A-T- bzw. GT-C-A-T-Haplotyp im Vergleich zum T-G-C-Haplotypen eine
signifikant höhere GWZ auf bzw. der T-G-C-Haplotyp im Vergleich zum
C-A-T-Haplotyp eine signifikant geringere. Für die dargestellten
Untersuchungsergebnisse werden verschiedene Erklärungsansätze diskutiert. So
erbrachte ein in Zusammenarbeit mit dem Centre for Addiction and Mental
Health, Toronto, Kanada erzeugtes Sample gleichsinnige Ergebnisse in Bezug auf
die Clozapin-/Olanzapingruppe. Bezüglich der Fragestellung ist festzuhalten,
dass es für den Einfluss von Polymorphismen des Neuropeptid Y-Gens auf die
antipsychotika-induzierte Gewichtszunahme tragfähige Hinweise gibt. Um die
klinische Relevanz der Erkenntnisse klarer einordnen zu können, sind weitere
Studien unumgänglich.

dc.description.abstract
Antipsychotic-induced weight gain is a common side effect in the treatment of
schizophrenia patients often resulting in non-compliance towards medication.
The development of a predictive tool for the patient-specific risk for this
antipsychotic-induced weight gain is emphatically pushed forward. In this
context, Neuropetide Y is an interesting gene because of its central role in
energy metabolism. Diverse changes in the Neuropetide Y system have already
been observed in patients under antipsychotic treatment. The focus of this
study is to detect if polymorphisms in the Neuropetide Y gene have an
influence on antipsychotic-induced weight gain. Therefore five polymorphisms
in the Neuropeptide Y gene (rs10551063, rs16147, rs5573, rs5574, rs16475) were
determined in blood samples of schizophrenia patients (n=93). Analyses of
covariates/factors have shown significant associations for baseline weight,
medication and participation length in the study. Considering these
associations, analyses for genotypes, alleles and haplotypes were performed.
Significant associations were observed in patients who had received
risperidone/quetiapine (n=30) or clozapine/olanzapine (n=26). In patients with
a risperidone/quetiapine treatment, carriers of the c-allele of the rs16147
polymorphism gained less weight compared to individuals with a TT-genotype.
Analogously, significant results were observed for the C-A-T-haplotype and the
GT-C-A-T-haplotype. In contrast, carriers of the –-allele of the rs10551063
polymorphism, the T-allele of the rs16147-polymorphism and the G-allele of the
rs5573-polymorphism, who received clozapine/olanzapinetreatment, gained
significantly less weight compared to individuals with the corresponding
homozygous genotype. Haplotype analyses showed that patients with the C-A-T or
the GT-C-A-T haplotype compared to the T-G-C-haplotype had a significant
higher weight gain, whereas patients with the T-G-C-haplotype had a
significantly lower weight gain than patients with the C-A-T-haplotype.
Different explanations for the research results are discussed. In cooperation
with the Centre for Addiction and Mental Health, Toronto, Canada, further
patient data could be obtained and the thus merged sample yielded accordant
results for patients treated with clozapine/olanzapine. Relating to the issue
of this study there are sustainable indications that the polymorphisms of the
Neuropeptide Y gene have influence on antipsychotic-induced weight gain.
Before drawing conclusion regarding the clinical applicability of these
findings, further studies are indispensable.
