Bei der Anwendung des monoklonalen Antikörpers Bevacizumab zusätzlich zu
aktuellen multimodalen Therapieverfahren beim lokal fortgeschrittenen
Rektumkarzinom stellt sich die Frage, ob sich aufgrund des spezifischen
Toxizitätsprofils von Bevacizumab die Häufigkeit von Komplikationen bei
anschließenden Operationen erhöht. Es wurden die chirurgischen Ergebnisse von
62 Patienten mit Rektumkarzinom im UICC-Stadium II/III in einer retrospektiven
Studie untersucht, die eine neoadjuvante Radiochemotherapie mit Capecitabin,
Oxaliplatin unter Hinzunahme von Bevacizumab erhielten. Sphinktererhaltende
Operationen wurden bei 40 Patienten (64,5%) durchgeführt. Postoperative
Komplikationen traten bei 27 Patienten (43,5%) auf. Anastomoseninsuffizienzen
wurden bei 7 Patienten (17,5%), Wundinfektionen und Wundheilungsstörungen bei
jeweils 8 Patienten (12,9%) und operationspflichtige Nachblutungen in zwei
Fällen (3,2%) beobachtet. Diese Ergebnisse liegen in dem Bereich, in dem sie
bei anderen Studien, mit operativen Eingriffen am Rektum, unter Einsatz von
Bevacizumab beobachtet worden sind. Gegenüber Studien mit konventioneller
neoadjuvanter Radiochemotherapie ohne zielgerichtete Antikörper ist, bei der
Betrachtung der Ergebnisse dieser Untersuchung, weder das Risiko für das
Auftreten von perioperativen Komplikationen erhöht noch das
Komplikationsspektrum verändert.

Addition of the monoclonal antibody bevacizumab to actual multimodal therapy
in local advanced rectal cancer leads to the discussion, whether the specific
profile of toxicity of bevacizumab results in an increased risk of
complications in following operations. In this retrospective study the
surgical results of 62 patients with UICC-stage II/III rectal carcinoma,
treated with preoperative chemoradiation with capecitabine and oxaliplatin
plus bevacizumab, were analyzed. Sphincter-sparing surgical procedures were
done in 40 patients (64.5%). Postoperative complications occured in 27
patients (43.5%). Leak of anastomosis was seen in 7 patients (17.5%), wound
infections and delayed wound healing in 8 patients (12.9%) respectively and
bleeding with requirement of re-operation in 2 cases (3.6%). These results are
in line with results documented in studies observing rectal cancer surgery in
combination with bevacizumab containing therapy. Regarding the results of
studies with conventional preoperative chemoradiation without targeting
antibody therapy the results of this study show no increased risk for
perioperative complications and no change in the spectrum of complications.
