Einleitung: Die Fontanoperation ist der endgültige palliative Schritt zur
Volumenentlastung und Zyanosebeseitigung im Zuge der Kreislauftrennung bei
Patienten mit univentrikulären angeborenen Herzfehlern. Da der pulmonale
Blutfluss nach der Fontanoperation passiv ist, hängt die pulmonale Perfusion
von einem niedrigen pulmonalen Gefäßwiderstand (PVR) ab. Eine
frühpostoperative Erhöhung des PVR führt über einen verringerten pulmonalen
Blutfluss und somit eine reduzierte Vorlast zu einem herabgesetzten
Herzzeitvolumen und eingeschränkter Systemperfusion mit darauffolgenden
Komplikationen im frühpostoperativen Verlauf. Daher wurden die
Therapieprinzipien zur Senkung des PVR als kausale Therapie des frühen
Fontanversagens untersucht. Methodik: In die retrospektive Datenanalyse wurden
151 Patienten eingeschlossen, die von 1995 bis 2012 eine extrakardiale
Fontanoperation am DHZB erhalten haben. Die Patientenpopulation wurde je nach
Dauer der mechanischen Ventilation in drei Gruppen (A ≤ 10h, B 11-72h, C >
72h) und je nach Bedarf an zusätzlicher Verabreichung von Stickstoffmonoxid
(NO) in zwei Gruppen (mit und ohne NO) aufgeteilt. In der Phase der
Beatmungsentwöhnung wurde die Verabreichung von inhalativem NO durch
zusätzliche Gabe eines PDE-5-Inhibitors (Sildenafil) ergänzt und die Wirkung
in Gruppen analysiert. Untersucht wurde die Senkung des PVR anhand folgender
Parameter: Mittlerer pulmonalarterieller Drück (mPAP), mittlerer arterieller
Drück (MAD), Beatmungs- und Aufenthaltsdauer sowie Inzidenz von verlängerten
Pleuraergüssen. Ergebnisse: Den optimalen postoperativen Verlauf (jeweils
signifikant niedrigere mPAP-Werte, höhere MAD-Werte, kürzere Aufenthaltsdauern
auf der Intensivstation) hatten Patienten mit kurzer Beatmungsdauer. Eine
verlängerte Dauer der Herzlungenmaschine ging mit einer signifikant
verlängerten Beatmungsnotwendigkeit einher. Erhöhte mPAP-Werte, erniedrigte
MAD-Werte, längere Aufenthaltsdauern auf der Intensivstation sowie die erhöhte
Inzidenz von verlängerten Ergüssen korrelierten signifikant mit dem Bedarf an
postoperativer NO-Ventilation. Die NO-Ventilation erlaubte im Trend eine
Reduktion der für die mechanische Ventilierung notwendigen Beatmungsdrücke.
Durch die zusätzliche Gabe von Sildenafil konnten postoperative Ergüsse
reduziert werden. Schlussfolgerung: Die durchgeführten Analysen erlauben die
Schlussfolgerung, dass eine möglichst schnelle Entwöhnung von der maschinellen
Ventilation die optimale Strategie im frühen Verlauf nach Fontanoperation
darstellt. Eine verlängerte Ventilation zeigt eine deutliche Korrelation mit
frühen Fontankomplikationen. Patienten, die eine verlängerte Ventilation
benötigen, müssen eine medikamentöse Therapie zur Senkung des PVR erhalten, da
ein für die Organperfusion ausreichendes Herzzeitvolumen nach Fontanoperation
nur durch eine erleichterte Lungenperfusion und damit bessere ventrikuläre
Füllung erzielt werden kann. Der Einsatz von inhalativem NO hat sich bewährt
und ermöglicht eine Verringerung der Beatmungsdrücke. Die zusätzliche
Verwendung von oralem Sildenafil erlaubt ohne Beatmungsnotwendigkeit die
Senkung des PVR mit verringerten Pleuraergüssen. Auf der Basis unserer
Auswertungen konnten Strategien zur Optimierung des Verlaufes früh nach
Fontanoperation entwickelt werden.

Introduction: The Fontan operation is the final palliative step to resolve
volume overload and cyanosis in the process of separation of the circulation
in patients with univentricular hereditary cardiac defects. As the pulmonary
blood flow after Fontan procedure is passive, the pulmonary perfusion depends
on a low pulmonary vascular resistance (PVR). An early postoperative increase
of PVR with reduced pulmonary blood flow and hence reduced preload leads to
restricted cardiac output and limited systemic perfusion, with subsequent
complications in the early postoperative course. Hence, therapeutic principles
of PVR reduction as causal therapy of early Fontan failure were examined.
Methods: The retrospective analysis was of 151 patients who underwent an
extracardiac Fontan operation at the Deutsches Herzzentrum Berlin (German
Heart Institute Berlin) between 1995 and 2012. According to the duration of
ventilation, the patient population was divided into three groups (A ≤ 10h, B
11-72h, C > 72h). In terms of the necessity of additional administration of
nitric monoxide (NO) the total patient group was divided into two subgroups
(with and without NO medication). During weaning from ventilation inhalative
NO was supplemented by use of a PDE-5 inhibitor (Sildenafil) and the effects
were analyzed in groups. The reduction of PVR was examined by taking into
account the following parameters: mean pulmonary arterial pressures (mPAP),
mean arterial systemic pressures (MAP), duration of intensive care unit (ICU)
and hospital stay, duration of ventilation and incidence of protracted pleural
effusion. Results: Patients with short duration of ventilation had optimal
postoperative course with, in each case, significantly lower mPAP values,
higher MAP values and reduced durations of hospital and ICU stay than in those
ventilated for longer. A longer cardiopulmonary bypass time correlated
significantly with prolonged need for ventilation. Elevated mPAP values,
reduced MAP values, longer durations of hospital and intensive care unit stay
as well as elevated incidence of pleural effusions correlated significantly
with necessity of postoperative NO ventilation. The ventilation pressures
necessary for the mechanical ventilation showed a trend towards a reduction by
NO ventilation. The additional use of oral Sildenafil significantly reduced
the duration of pleural effusions. Conclusion: The results of the analysis
suggest that fast weaning from mechanical ventilation represents the optimal
strategy in the early postoperative course after Fontan operation. Prolonged
ventilation significantly correlates with early Fontan complications. Patients
with prolonged necessity of mechanical ventilation need causal drug therapy to
reduce the PVR, as sufficient cardiac output in the Fontan circulation can
only be achieved with eased lung perfusion and consequently adequate
ventricular charge. The use of inhalative NO is an approved method and enables
the reduction of ventilation pressures. Additional oral Sildenafil allows the
PVR to be lowered, with reduced duration of pleural effusions, without
ventilation being necessary. Based on our analysis, strategies to improve the
early postoperative course after Fontan operation could be developed.
