In Deutschland werden derzeit jährlich etwa 48.650 PCa diagnostiziert. So ist
die Prostata mit 22,3% die häufigste Lokalisation bösartiger Neubildungen beim
Mann. Das PSA ist zzt. der mit Abstand bedeutendste Tumormarker der
urologischen Onkologie in Bezug auf Früherkennung, Therapie und Verlauf des
PCa. Der große Nachteil des PSA-Wertes ist sein Mangel an Spezifität. Das PSA
besitzt im Bereich zwischen 4,0–10 µg/L eine unzureichende Spezifität. Das oft
synchrone Auftreten von BPH und Prostatitis oder auch PCa, verbunden mit der
relativ hohen Rate falsch-positiver bzw. falsch-negativer PSA-Werte im Serum
führte zur Entwicklung weiterer Konzepte zur Spezifitätsverbesserung wie der
PSA-Anstiegsgeschwindigkeit, PSA Verdopplungszeit oder auch der PSA-Dichte.
Die Erwartung, dass diese Verfahren die Aussagekraft des tPSA-Wertes
signifikant erhöhen, hat sich jedoch nur teilweise erfüllt. Zum heutigen
Zeitpunkt ist präoperativ lediglich der %fPSA-Wert zusätzlich zum tPSA als
klinischer Routineparameter zur besseren Differenzierung zwischen einem PCa
und einer BPH geeignet. Jeder neue Parameter zur Prostatakarzinomerkennung
muss im Vergleich zum %fPSA im Hinblick auf die Verbesserung der Spezifität
zur Tumordetektion betrachtet werden. Intensive Erforschungen des PSA konnten
zeigen, dass das PSA sowohl im Plasma als auch in der Seminalflüssigkeit aus
einer heterogenen Mischung verschiedener freier und gebundener PSA-
Subfraktionen besteht. Zusätzlich besteht auch die freie Fraktion des PSA aus
weiteren verschiedenen molekularen Formen. In zahlreichen präklinischen
Studien gelang mittlerweile der Nachweis potentiell karzinomspezifischer bzw.
BPH-spezifischer Varianten der freien Fraktion des PSA. Die Einführung
kombinierter (-2) proPSA, (-4)proPSA und (-5,-7)proPSA-Assays als neue Marker
zur präoperativen Tumordetektion hatte eine Reihe an Untersuchungen zur
Bestimmung ihrer Wertigkeit in Bezug auf die Früherkennung von vor allem
aggressiven Formen des PCa zur Folge. Die vorliegende Arbeit diente der
Bestimmung der präoperativen diagnostischen Wertigkeit des (-5,-7)proPSA und
seiner Quotienten mit tPSA und fPSA zur Differen-zierung von Tumorstadium und
-grading. Hierzu wurden die Daten von 376 prostatektomierten Patienten aus der
Klinik für Urologie der Medizinischen Fakultät Charite - Universitätsmedizin
Berlin aus-gewertet. Zusammenfassend demonstrierte diese Studie die
verbesserten Möglichkeiten des Quotienten (-5,-7)proPSA/(%fPSA) in der
Unterscheidung zwischen aggressiven von weniger aggressiven Formen des PCa.
Obwohl die Bestimmung des (-5,-7)proPSA eine begrenzte Wertigkeit in Bezug auf
die Differenzierung zwischen dem PCa und nichtmalignen Prostataerkrankungen
hat, zeigte dieser Serummarker in dieser Studie die Möglichkeit zur
verbesserten Vorhersage der Aggressivität eines PCa.

dc.description.abstract
In Germany approximately 48650 new cases of PCa are diagnosed for the year
2006. Since its discovery more than 20 years ago, PSA has been established as
the most valuable tool for early detection, staging and monitoring of PCa. Low
diagnostic specifity is a major drawback of the marker prostate specific
antigen (PSA). So far, only %fPSA has been established as a clinical routine
parameter for better discrimi-nation between PCa and BPH in addition to tPSA
and every new parameter for PCa detection has to be tested in comparison to
%fPSA regarding its ability to further enhance specifity. The use of molecular
forms of PSA such as free PSA and its ratio to total PSA ((f/tPSA oder %fPSA)
have been proposed to enhance the specificity for the detection of prostate
cancer (PCa). Recently, serum free PSA has been found to be a mixture of
different subforms including proPSA, a precursor form of PSA, “benign” PSA
(BPSA), a cleaved form of PSA and “intact” (iPSA), an inactive form of free
PSA. ProPSA, the precursor of tPSA (237 amino acids) consists of 244 amino
acids and its currently termed (-7)proPSA. However, truncated forms of this
proPSA such as (-2) and (-5)proPSA were also identified in serum, and assays
were developed by two research groups. Beckmann Coulter introduced a research
assay for the combined detection of (-2)proPSA, (-4)proPSA and (-5,-7)proPSA
that has been suggested to preferentially detect aggressive PCa . The other
developed assay (Roche Diagnostics, Mannheim, Germany) for (-5,-7)proPSA was
recently used in two studies to assess the validity of these proPSA forms to
detect early PCa and to distinguish pathological grade. Data from one study
showed a limited validity of proPSA compared to %fPSA to improve PCa detection
and the power to discriminate between G1, G2 and G3 tumors. Since the results
from both studies with the combination of (-2)proPSA, (-4)proPSA and
(-5,-7)proPSA assay and with the (-5,-7)proPSA were based on small subgroups
of Gleason score greater or equal than 7 tumors or G3 tumors we conducted a
study on a larger population of PCa patients treated with radical laparoscopic
prostatectomy. Thus, the aim of our present study in patients with organ
confined and non-organ confined PCa was to investigate whether proPSA or its
ratios with tPSA and fPSA may predict final pathological stage and may be
useful to differentiate between G1, G2 and G3 PCa and to distinguish between
Gleason score<7 and Gleason score greater or equal than 7. We also measured
the (-5,-7)proPSA to estimate the possible correlations of proPSA to known
factors like PSA, fPSA and %fPSA, and furthermore to see possible advantages
of proPSA measurements in subgroups of patients with %fPSA values of <10%.
Three hundred and seventy six patients with a tPSA concentration of 1-25 µg/l
treated at the Charité Hospital Berlin with radical laparoscopic prostatectomy
between 2001 and 2004 were enrolled in this study. All patients were diagnosed
histo-pathologically by microscopic examination of the prostatic specimens.
Cancer stage was assigned according to the TNM system from 2002. Only patients
without metastases were analysed (all N0 and M0). This study could demonstrate
that measurement of (-5,-7proPSA serum concentrations enhanced the chance to
identify more aggressive forms of PCa and an advantage of (-5,-7)proPSA to
separate pT2 and pT3 PCa. In summary, this study demonstrated in a large
cohort the improved performance of proPSA/%fPSA compared with (-5,-7)proPSA
and %fPSA alone to separate aggressive from non-aggressive PCa. Thus, although
proPSA alone showed a limited capability to differentiate between malignant
and non-malignant prostate diseases, the determination of proPSA and the
above-mentioned calculation of ratios could be taken into account to predict
the aggressiveness of the tumor.
