Die Parodontitis ist eine entzündungsbedingte Krankheit, die durch
verschiedene Bakterien verursacht wird. Eine übermäßige Immunantwort führt bei
einem geringen Teil der Weltbevölkerung, der ca. 10 % ausmacht, zu einer
seltenen Form der Parodontitis, der aggressiven Form (lokalisiert oder
generalisiert ausgeprägt). Eine Blutung ist das terminale Ereignis einer Kette
von Gewebe- und Gefäßveränderungen, das aus dem Zusammenwirken von
verschiedenen bakteriellen und zellulären Entzündungsmediatoren entsteht. Im
Fall einer Gingivitis – einer Entzündung des Zahnfleisches – ist eine
Graduierung der Blutungsintensität längst ein Beurteilungsparameter für den
Schweregrad. Eine parodontale Entzündung geht mit dem Verlust des
Zahnhalteapparates einher und ist charakterisiert durch die Entstehung von
Taschendefekten im Alveolarknochen. Der Schweregrad einer Parodontitis wird
aktuell durch die Bestimmung der Sondierungstiefen (ST) beurteilt. Der Befund
einer Sondierungsblutung wird lediglich dichotom als ein Zusatzmerkmal
gehandhabt. Diese Studie sollte prüfen, ob eine visuelle Graduierung der
Sondierungsblutung in Form eines Pocket Bleeding Index (PoB-Index) den
tatsächlichen Entzündungszustand einer Parodontaltasche widerspiegeln kann.
Die visuelle Betrachtung und Einschätzung wurde mit objektivierbaren
Parametern wie der Blutungszeit (Dauer mit einer Stoppuhr gemessen) und der
Blutmenge (Länge der Blutsäule im Kapillarröhrchen) in Beziehung gebracht.
Weitere Bezugsparameter waren die Sondierungstiefe und die Menge an
Sulkusflüssigkeit (SF), die als Exsudat einem natürlichen Gradienten folgt und
mithilfe einer Filterpapiertechnik am Zahnfleischsaum gesammelt und
ausgewertet wird. Eine Gefäßveränderung geht allen klinischen Symptomen voran
und führt zur messbaren Erhöhung der SF im Sulkusbereich. Da die Parodontitis
einen schubweisen Verlauf hat, bei dem sich die akuten Phasen mit den
Ruhephasen abwechseln, wird im Gewebe auch eine wellenartige Gefäßveränderung
beobachtet. Das durchschnittliche Alter der 17 rekrutierten Patienten lag bei
32 Jahren, davon zehn Nichtraucher und sieben Raucher. Die Patienten hatten
keine parodontale und keine Antibiotikatherapie während der letzten zwölf
Monate erhalten. Die Patienten erhielten wiederholt intensive
Mundhygieneinstruktionen, so dass die API-Werte während der Studiendauer unter
25 % blieben. Die Therapie wurde gemäß internationalen Standardkriterien
durchgeführt. Alle Patienten erhielten eine Antibiotikatherapie aus
Amoxicillin (500 mg) und Metronidazol (250 mg) dreimal täglich für zehn Tage,
unmittelbar nach Abschluss der geschlossenen Kürettage. Alle Studienparameter
wurden nach der Initialtherapie und 3, 6 und 9 Monate nach Abschluss der
Therapie erhoben. Insgesamt wurden 575 Messstellen untersucht und verglichen.
Die wiederholten Messungen wurden zur besseren Reproduzierbarkeit bei allen
Patienten nach gleichen Gesichtspunkten festgelegt. Die statistisch
signifikante Korrelation der Werte deutete auf einen zum Zeitpunkt der
Eingangsuntersuchung vorhanden Zusammenhang zwischen der SF-Menge und dem PoB-
Index hin. Nicht nur der subjektive PoB-Index (R2 = 0,19), sondern auch die
Blutmenge korrelierten eng mit der SF-Menge (R2 = 0,417). Bei der
Eingangsuntersuchung wurden keine Assoziationen zwischen ST und anderen
Parametern beobachtet. Die Arbeitshypothese wurde bestätigt, als tiefe
Messstellen z. T. eine leichte oder gar keine Sondierungsblutung aufwiesen und
demgegenüber die flachen Messstellen einen viel höheren PoBI-Wert und die
korrespondierende SF-Menge zeigten. Beides gibt einen Hinweis auf eine zu
diesem Zeitpunkt aktive Entzündungsphase an diesen Stellen. Die durchgeführte
parodontale Therapie resultierte in einer Stabilisierung der parodontalen
Gewebe und in einer signifikanten Reduktion der ST, der Abnahme der SF-Menge
und der Reduktion aller Blutungsparameter. So konnte nach Abschluss der
Behandlung zwischen der SF-Menge und allen Blutungsparametern keine
statistisch signifikante Korrelation hergestellt werden. Allerdings wurde erst
nach der Therapie die Korrelation zwischen SF-Menge und der ST zunächst hoch
signifikant und später signifikant. Diese Korrelation kann durch ein
Gleichgewicht zwischen einem „inneren Taschenvolumen“ und der Menge an
produzierter SF nach der Therapie erklärt werden. Während der Studien wurde
zwischen der ST und den Blutungsparametern keine Assoziation beobachtet. Die
aktuelle Studie stellt einen Zusammenhang zwischen der
Sondierungsblutungsintensität und den anerkannten subklinischen
Entzündungsparametern her. Es konnte gezeigt werden, dass auf der Basis einer
visuellen Einstufung des parodontalen Symptoms „Blutung“ die
entzündungsaktiven Stellen effektiv eingeteilt werden können. Erst nach einer
Parodontaltherapie ist eine komplette Abwesenheit der Sondierungsblutung ein
geeigneterer Parameter zur Beurteilung der parodontalen Stabilität.

dc.description.abstract
The periodontitis is an inflammatory disease condition caused by a variety of
oral microorganisms. A hyper reactive host response regulation is triggering
the disease progression in a small part of the world wide population – around
10% at most – resulting either in a local or generalized aggressive form of
manifestation. Bleeding is a final act in a process of tissue and capillary
damage initiated by bacterial presence and modulated by pro-inflammatory
cytokines. Calibrated assignment of bleeding intensity as a parameter for the
severity of the disease is considered golden standard for diagnosing
gingivitis – an inflammatory condition restricted to the soft tissues.
Periodontal inflammation regularly causes attachment loss and is accompanied
by loss of alveolar bone and a phenomenon named “pocketing”. The severity of
this disease condition is currently characterized by assessing either the
probing pocket depths (PPD) or the attachment loss (AL). The evaluation of
bleeding tendency as Bleeding on Probing (BoP) is currently an additional
dichotomous finding. The aim of the present study was to assess the
suitability of a visual calibration of a Pocket Bleeding Index (PoB-Index) to
reflect the real inflammatory status of each single probed periodontal pocket.
By matching single grades of the index suggested – i.e. the single visually
assigned bleeding grades - with parameters such as bleeding duration (stop
watch) and amount of blood sampled by capillary tube correlation coefficients
were calculated. Correlations between PoB-Index and the probing depth and PoBI
and the amount of gingival crevicular fluid (GCF) sampled as exudates that
follow a gradient were calculated also to objectify the visual calibration of
bleeding intensity. The capillary dilatation is initiated as first step in a
cascade of inflammatory reactions in front of all clinical signs and results
in a measurable increase of the GCF. Since periodontitis progresses
episodically, i.e. phases of activity are followed by phases of inactivity the
capillary dilatation follows the same pattern. The mean age of 17
participating patients recruited was 32 years; 10 of them were non-smokers
whereas 7 were smoking. All patients enrolled did not have any periodontitis
therapy or received any antibiotics prescribed for the last 12 months. All
patients underwent intensive instructions in oral hygiene repeatedly, thus the
allocated plaque index remained for the duration of the study < 25% (API <
25%). After completion of instrumentation (S/RP) all patients received
Amoxicillin (500 mg) and Metronidazol (250 mg) x 3 daily for 10 days per os
prescribed. The assessment of the study parameters was carried out at baseline
after completion of the initial therapy (before S/RP), at month 3, 6 and 9
past active therapy. In total 575 sites were evaluated and monitored. For
improving the reproducibility, all measurements were undertaken under equal
conditions in all patients. The statistically significant correlation between
the GCF amount and the PoB-Index grades at baseline evaluation (R2 = 0.19)
indicates that the calibration of bleeding grades can reflect the severity of
inflammatory status of a site before treatment. Respectively, the amount of
blood sampled at baseline correlated with the GCF amount significantly (R2 =
0.417). None correlation between PPD and any other parameter assessed was
established at the level of statistical significance at baseline. Our
hypothesis was confirmed by the data, since the bleeding intensity in
shallower sites was recorded greater than in deeper sites and thus the higher
PoB-Index value was assigned for at baseline. The assignment of the PoB-Index
calibration was in accordance with the level of GCF determined for the same
site at the same evaluation step (baseline) regardless the PPD. Our
observations confirm that at baseline the activity grade within periodontal
pockets may vary disregarding the probing pocket depth of the site. The
periodontal therapy offered resulte in an improved periodontal situation
characterized by a significantly reduced PPD; a decline in the amount of GCF
detectable at the recall visits at sites of interest and reduced values for
the bleeding parameters. Therefore, at control visits the correlation between
the bleeding parameter and the amount of GCF did not reach the level of
statistical significance. However, at the control visit 3 and 6 months the
correlation between the GCF amount and PPD became statistically significant.
This correlation may rely on the homeostasis between the newly formed GCF
amount and the residual volume of the periodontal pocket. The correlation
between PPD and the bleeding parameter was not statistically significant
throughout the duration of the study. This study establishes an association
between subclinically detectible parameters considered occurring prior to the
clinical manifestation of inflammation with the intensity of bleeding on
probing in periodontal pockets. The study demonstrates effectively that the
visial calibration of bleeding intensity ingrades is rliable tool to quantify
the severity of the inflammation stage. First during the supportive
periodontal therapy the complete absence of bleeding becomes a suitable
parameter for documenting periodontal stability.
