Einleitung Als möglicher Einflussfaktor einer craniomandibulären Dysfunktion
ist die Kondylenposition im Kiefergelenk Gegenstand kontroverser Diskussionen.
Das Ermitteln der Kondylenposition erfolgt bisher überwiegend durch die
klinische Untersuchung in Kombination mit instrumenteller Funktionsanalyse.
Die digitale Volumentomographie hat bis dato einen marginalen Stellenwert in
diesem Diagnostikprozess, obwohl diese eine dreidimensionale Erfassung der
Kondylenposition ermöglicht. Das Ziel der vorliegenden Arbeit war die
Entwicklung einer Klassifikation, welche die in der digitalen
Volumentomographie dreidimensional ermittelte Kondylenposition mit klinischen
Befunden der Manuellen Strukturanalyse bezüglich des Gelenkraums kombiniert.
Material und Methoden Das Patientenkollektiv umfasste eine Gruppe von
Patienten mit craniomandibulären Dysfunktionen (CMD-Gruppe; n=101) und eine
Vergleichsgruppe, deren Patienten sich in kieferorthopädischer Behandlung
befanden (KFO-Gruppe; n=102). Retrospektiv wurden die Kondylenpositionen in
den vorhandenen digitalen Volumentomogrammen ermittelt, mit spezifischen
klinischen Befunden des Gelenkraums kombiniert und in unterschiedliche
Kondylenpositionsklassen (= CP-Klassen) eingeteilt. Sowohl die Häufigkeiten
der einzelnen Klassen im Gruppenvergleich, als auch das Auftreten weiterer
pathologischer Befunde der Manuellen Strukturanalyse in Abhängigkeit von den
CP-Klassen, wurden untersucht. Ergebnisse Radiologisch als physiologisch
bewertete Kondylenpositionen (CP0) traten in der CMD-Gruppe mit 3 % deutlich
seltener auf als in der KFO-Gruppe mit 26,5 % (p<0,001). Gelenkentlastende
Positionen (CP1) traten in der CMD-Gruppe zwar seltener auf, dieser
Unterschied war jedoch nicht signifikant (CMD-Gruppe: 9,4 %; KFO-Gruppe: 15,7
%; p=0,056). In beiden Gruppen zeigten mehr als die Hälfte aller Gelenke
radiologisch eine gelenkbelastende Kondylenposition ohne dazu passenden
klinischen Befund (CP2; CMD-Gruppe: 56,9 %; KFO-Gruppe: 55,9 %; p=0,831).
Darüber hinaus wies die CMD-Gruppe wesentlich häufiger eine radiologisch
gelenkbelastende Kondylenposition mit dazu passendem klinischen Befund auf
(CP3; CMD-Gruppe: 26,2 %; KFO-Gruppe: 2 %); dieser Gruppenunterschied war
signifikant (p<0,001). Für das Auftreten weiterer pathologischer
Funktionsbefunde der Manuellen Strukturanalyse (klinische Befunde der
Gelenkflächen, Muskeln, Knackgeräusche und Mundöffnung) ergaben sich
signifikante Abhängigkeiten zu den CP-Klassen (jeweils p<0,05).
Schlussfolgerung Die Kombination der radiologisch ermittelten Kondylenposition
mit spezifischen klinischen Befunden der Manuellen Strukturanalyse
berücksichtigt die Adaptations- und Kompensationsfähigkeit des
craniomandibulären Systems. Die CP-Klassifikation ist dazu geeignet, den
Einfluss der Kondylenposition auf die craniomandibuläre Dysfunktion
patientenindividuell zu bewerten und in der Therapieentscheidung zu
berücksichtigen.

Introduction As a possible factor influencing craniomandibular dysfunction,
condylar position is the subject of controversial discussions. The condylar
position has so far mainly been determined by clinical examination in
combination with instrumentally supported functional analysis (Instrumentelle
Funktionsanalyse). To date, cone beam computed tomography has been of marginal
use in this diagnostic process, although it allows three-dimensional detection
of the condylar position. The aim of this thesis was to develop a
classification that combines the three-dimensional evaluation of condyle
position in cone beam computed tomography with specific clinical findings from
the manual structural analysis (Manuelle Strukturanalyse; MSA) regarding the
joint space. Material and Methods The examined cohort consisted of a group of
patients with craniomandibular dysfunction (CMD group; n=101) and a comparison
group of patients in orthodontic therapy (KFO group; n=102). The condylar
positions were determined retrospectively from cone beam computed tomographs
combined with specific clinical findings regarding the joint space, and
classified into different condylar position classes (CP classes). The
frequencies of the individual classes in the groups were compared and the
occurrence of further pathological findings in the MSA in relation to the CP
class was investigated. Results Radiologically, physiological condylar
positions (CP0) were significantly less frequent in the CMD group (3%) than in
the KFO group (26.5%) (p<0.001). Joint relieving positions (CP1) also tended
to be less frequent in the CMD group (CMD group: 9.4%; KFO group: 15.7%) but
this difference was not significant (p=0.056). In both groups, more than half
of all joints showed a joint loading condylar position radiologically without
appropriate clinical findings (CP2; CMD group: 56.9%, KFO group: 55.9%,
p=0.831). In addition, the CMD group presented a joint loading condylar
position with appropriate clinical findings more frequently than the KFO group
(CP3; CMD group: 26.2%; KFO group: 2%); this group difference was highly
significant (p<0.001). The occurrence of further pathological findings in the
MSA (e.g. muscular findings, cracking noises, mouth opening restrictions) was
significantly correlated with the CP classes (in each case p<0.05). Conclusion
The combination of radiologically determined condylar position with specific
clinical findings in the MSA takes into account the adaptation and
compensatory capacity of the craniomandibular system. The CP classification is
suitable for assessing the individual influence of the condylar position on
craniomandibular dysfunction and can be used in the treatment decision.
