Ziel Geburten vor der 37. Schwangerschaftswoche stellen in Deutschland weiterhin eine geburtsmedizinische und neonatologische Herausforderung dar. Von diesen sind bis zu 40% der Frühgeburten infektiös bedingt. Zur Identifikation eines Amnioninfektionssyndroms (AIS) und der beteiligten Mikroorganismen werden routinemäßig eine kulturbasierte und histologische Diagnostik durchgeführt. Ziel dieser Studie ist es, ein kontaminationsarmes Studienprotokoll zu entwickeln und anschließend molekularbiologische Nachweismethoden (FISH, PCR) zur Detektion der beteiligten Mikroorganismen einzuführen und die Ergebnisse mit der konventionellen Diagnostik zu vergleichen. Methodik Die Studie setze sich aus 25 Patientinnen in der AIS- Gruppe und 25 Patientinnen der Kontrollgruppe zusammen. Zur Minimierung der mikrobiellen Kontamination während der Entnahme der Plazenta- und Eihaut- Proben nach Sectio caesarea wurde ein Entnahmeprotokoll zur Standardisierung der Vorgehensweise entwickelt. Anschließend wurden mittels 16S rRNA Sonden die Fluoreszenz-in-situ-Hybridisierung sowie die PCR des 16S rRNA-Gens durchgeführt. Zur Evaluierung dieser Methoden erfolgte der Vergleich der Ergebnisse mit den Resultaten der kulturellen und histo-pathologischen Untersuchungen. Ergebnis Die FISH und PCR konnten in vielen Fällen eine Besiedlung der Eihäute nachweisen und die besiedelnde Spezies identifizieren. Häufig stellte dies eine Ergänzung zur kulturellen Untersuchung dar, die nicht zuverlässig einen Besiedlungsnachweis bei histo-pathologisch gesichertem AIS erbringen konnte. Vereinzelt ergab sich eine durch FISH und PCR identifizierte, mikrobielle Besiedlung, die histologisch und kulturell nicht bestätigt werden konnte. Schlussfolgerung Die Ergebnisse der Studie legen nahe, dass die FISH und PCR eine wertvolle Ergänzung in der Diagnostik eines AIS darstellen und Hinweise auf deren Entstehungsmechanismus liefern. Weiterhin war es möglich, ein kontaminationsarmes Studienprotokoll zu entwickeln und eEPK als Werkzeug zur Prozessbeschreibung in der medizinischen Forschung zu etablieren. Die Untersuchung der Kontrollpatientinnen erbrachte neue Hinweise, die gegen das häufige Vorkommen klinisch unauffälliger Infektionen bei normal verlaufenden Schwangerschaften sprechen. Aufgrund der geringen Größe einer Probe ist es möglich, dass mitunter besiedelte Areale nicht erfasst wurden. Eine Weiterführung der Untersuchungen an zusätzlichen betroffenen Patientinnen ist notwendig, um die Ergebnisse zu verifizieren und ein besseres Verständnis der Pathogenese des AIS zu entwickeln.
Aim of the study Preterm birth is still an obstetrical and neonatological challenge in Germany. 40% of these deliveries are induced by infectious agents. The identification of chorioamnionitis and the causing microorganisms is currently obtained by culture-based methods and histological diagnosis. The objective of this study is to originate a contamination-free study protocol as a basis to analyse samples taken from the amniotic membrane and the placenta. Results derived from sample analysis through moleculobiological methods (FISH, PCR) are matched against conventional diagnosis. Methods The study population consists of 25 inpatients with chorioamnionitis delivering via secondary Caesarean section and 25 inpatients that delivered by a planned Caesarean section. A standarised extraction protocol has been developed to minimise microbial contamination during sample acquisition after the Caesarean section. The following working steps included Fluorescence-in-situ-hybridisation with 16S rRNA-probes and RT-PCR of the 16SrRNA-gene. The results of the moleculobiological based methods have been compared to the results of culture- based and histological methods. Results FISH and PCR identified a microbial colonisation of the amniotic membrane and the precise species. This is a useful supplement to culture-based methods since those were not able to detect a microbial colonisation in a histological ensured chorioamnionitis. Occasionally, FISH and PCR detected a microbial infection that could not be verified by culture-based and histological methods. Conclusion The results of this study suggest that moleculobiological methods are valuable additions in the diagnostic process of chorioamnionitis. They provide new indications of the formation and origin of chorioamnionitis that leads to a better understanding of this medical condition. Furthermore, we were able to provide a standardized contamination-free study protocol and implemented eEPK as a medical process description notation. Additionally, we demonstrated the absence of microbial invasion in normal gestations. The limitations of this study are the small number of inpatients and the limited number of samples derived and analysed from each patient. Further studies are required to verify the obtained results.
