Für viele Patienten mit skelettalen Kieferfehlstellungen bietet das Konzept „Surgery First“ (SF) Vorteile bezüglich des Therapieerfolgs und der Behandlungsdauer, sodass sich diese Methode immer weiter etabliert. Dank des technischen Fortschritts lassen sich heute SF-Therapien auch virtuell planen und die notwendigen OP-Splinte in einem Computer-aided design/computer-aided manufacturing-(CAD/CAM)-Verfahren herstellen (VSF-Virtual Surgery First). Im Vorfeld dieser Arbeit wurde von 24 Patienten mit Hilfe eines Dentalen Volumentomografie-(DVT)-Scans eine virtuelle Planung nach VSF mit der Software SimPlant Pro OMS (Materialise Dental, Leuven, Belgien) erstellt. Diese konnte über in einem 3D-Druckzentrum (Materialise Dental® NV, Leuven, Belgien) hergestellte OP-Splinte erfolgreich umgesetzt werden. Mit einem Dental-Scanner (Orthodontic 3D Scanner R700™ der Firma 3Shape) wurden die Anfangs- und Abschlussmodelle digitalisiert. Aus den digitalisierten Daten wurde, unter Verwendung der Software 3Shape OrthoAnalyzer™, der Peer Assessment Rating Index (PAR-Index) bestimmt. Das Patientengut umfasste Fälle mit symmetrischen und komplexeren asymmetrischen Kiefer-fehlstellungen. Ziel der Studie war es, die Behandlungsdauer und den Behandlungserfolg (PAR-Index-Veränderung) des VSF-Konzepts zu evaluieren. Durch die Behandlung konnte eine signifikante Verringerung des PAR-Wertes um 33,67 Punk-te bzw. 86,42% erreicht werden (p<0,001). Damit liegt die Verbesserung des PAR-Wertes deutlich über der in konventionell durchgeführten modellbasierten Therapien ohne SF (PAR- Veränderungen 72 - 77%). Insgesamt betrug die durchschnittliche Behandlungsdauer 14,8 Monate. Im Vergleich zu konventionellen Therapiemethoden konnte eine zeitliche Reduktion von 32,4 - 56,2% erreicht werden. Signifikante Unterschiede bezüglich des PAR-Index oder der Behandlungsdauer bei verschiedenen Subgruppen (prätherapeutische skelettale Klassen, bi- und monomaxilläre OPs) traten nicht auf. Mit VSF-Konzepten können verschiedene skelettale Dysgnathien mit einem exzellenten Erfolg in deutlich kürzerer Behandlungszeit korrigiert werden. Die virtuelle Planung zeigt vor allem bei komplexeren asymmetrischen Dysgnathien große Vorteile. Es ist zu erwarten, dass sich das VSF-Konzept in Zukunft im dento-fazialen Bereich etablieren wird.
For many patients with the skeletal malposition of the jaw, the concept “Surgery First” provides many advantages regarding the therapeutic success and the duration of treatment. Therefore, this method is gaining popularity. Due to technological advances, treatment planning of SF can nowadays be done virtually on a computer and the necessary operation splints can be produced via a CAD/CAM-process (VSF-Virtual Surgery First). Prior to this step, virtual planning after VSF was rendered with the help of DTV-Scans for 24 patients using the software SimPlant Pro OMS (Materialise Dental, Leuven, Belgium). This planning was able to be treated successfully by means of the 3D-printing centre (Materialise Dental® NV, Leuven, Belgium) produced OR-splints. The dental scanner –Orthodontic 3D Scanner R700™ by 3Shape – digitalized initial and final models, and the Peer Assessment Rating Index (PAR-Index) was collected with the help of the software 3Shape OrthoAnalyzer™. The patient material included symmetric as well as more complex asymmetric malpositions of the jaw. The aim was to demonstrate the therapeutic success of VSF via the PAR-Index as well as to clarify and discuss the advantages of the method with regard to the duration of treatment. A significant reduction of the PAR-value by 33.67 points respectively 86.42% was able to be achieved (p<0,001). Therefore the improvement of the PAR-value lies significantly above the conventionally conducted model-based methods without SF, which are only able to reach an averaging PAR-modification of 72 to 77%. Overall, an average duration of treatment of 14.8 months was able to be achieved, which is an average timely reduction of 32.4 to 56.2% compared to conventional therapeutic methods. Through VSF different kinds of skeletal dysgnathia can be treated and corrected at a really high standard in significantly shorter duration of treatment. The virtual planning is especially advan-tageous regarding more complex asymmetric dysgnathia. Because of this and of many more advantages, the virtual surgery within the dento-facial segment should be seen to gain influence in the future. Nevertheless, the concept VSF still calls for a thorough selection of patients and an experienced orthodontist.
