Die Lebertransplantation gehört von den 12.000 benötigten Spenderorganen nach
der Nierentransplantation zu dem zweit häufigsten Eingriff in der
Transplantationsmedizin. In Europa wird die Verteilung der Organe über
nationale oder internationale Vereinigungen organisiert. So haben sich Länder
wie Deutschland mit ihrer nationalen Vergabestelle der Deutschen Stiftung
Organtransplantation (DSO) mit Österreich, den Benelux-Ländern, Slowenien und
Kroatien zu Eurotransplant (ET) schon 1967 zusammengeschlossen. Derzeit wird
die Zustimmungslösung von der Entscheidungslösung zur Vergabe der postmortalen
Organe nach Hirntod des Spenders abgelöst. Die Vergabe der Spenderleber an
einem Empfänger wird nach dem MELD-Score geregelt. Die etablierteste und
häufigste Art der Leberspende ist jene nach dem Hirntod (DBD). Deutlich
seltener wird die Lebendspende hierzulande durchgeführt. Nicht vereinbar mit
dem deutschen Transplantationsgesetz, aber durchaus zur Erweiterung des
Spenderpool beitragend, ist die Spende nach dem Herztod (NHBD/DCD). Dieses
Verfahren wird in ausgewählten Zentren auf der Welt auf Basis der statischen
kalten Organkonservierung (SCS) durchgeführt. Eine Alternative zur SCS ist die
aufwendige experimentelle Maschinenperfusion (MP) in Hypothermie (HMP),
Subnormothermie (sNMP) oder in Normothermie (NMP). Das Ziel dieser Arbeit war
der Vergleich unterschiedlicher Perfusionslösungen zur MP am NHBD am
Ratenmodell. Es gab drei Gruppen, die nach 1h Warmischämie gefolgt von 5h-SCS
+ 1h-MP zugeführt wurden. Die benutzen Lösungen waren die HTK (1. H-MPS), UW-
MP (2. B-MPS) und eine Basis-Lösung mit einem auf Hämoglobin basierten
Sauerstoffträger (3. O-MPS), dem Oxyglobin®. Die Kontrollgruppe (4.) wurde
nach 1h Warmischämie einer 6h-SCS zugeführt. Abschließend erfolgte in allen
vier Gruppen die Lebertransplantation. Das Follow-up betrug 60 Tage. Keiner
der Tiere aus zwei von drei MP-Gruppen überlebten den ersten postoperativen
Tag. Nur die B-MPS mit der modifizierten UW-Lösung konnte mit 4/5 überlebten
Tieren gegenüber der Kontrollgruppe mit 6/6 überlebten Tieren mit nicht
signifikanten Überlebensdaten überzeugen. Es gab in den Surrogatmarkern (ALT,
AST, GLDH, LDH) während der MP und postoperativ zwischen der H-MPS/O-MPS und
der B-MPS/Kontrolle keine eindeutigen signifikanten Unterschiede.

dc.description.abstract
Liver transplantation is one of the 12,000 needed donor organs right after the
kidney transplantation on the second place of the most common interference in
transplantation medicine. In Europe, the distribution of the organs is being
organized by national or international associations. Thus, countries such as
Germany with its national procurement office of the German Foundation for
Organ Transplantation (DSO) have joined forces to Eurotransplant (ET) with
Austria, the Benelux countries, Slovenia and Croatia in 1967. Currently, for
the award of post-mortem organs after brain death of the donor, the consent
solution is removed by decision solution. The allocation of donor liver at a
receiver is governed by the MELD score. The most established and most common
type of liver donation is that after brain death (DBD). Significantly less
frequent living donation liver transplant (LDLT) is carried out in this
country. Incompatible with the German Transplantation Act, but certainly
contributing to expand the donor pool is the donation after cardiac death
(NHBD/DCD). This process is carried out at selected centers of the world on
the basis of the simple static cold storage (SCS). An alternative to SCS is
the complicated experimental machine perfusion (MP) in hypothermia (HMP),
subnormothermia (sNMP) or normothermia (NMP). The aim of this study was to
compare different perfusion solutions for the MP on NHBD in rats. There were
three groups that underwent after 1h of warm ischemia 5h-SCS + 1h-MP. The used
solutions were the HTK (1. H-MPS), UW-MP (2. B-MPS) and a basic solution with
a hemoglobin-based oxygen carrier (3. O-MPS), the Oxyglobin ®. The control
group underwent after 1h warm ischemia 6h-SCS. Finally the liver
transplantation took place in all four groups. The follow-up was 60 days. None
of the animals from two of three MP-groups survived the first postoperative
day. Only the B-MPS with the modified UW-solution could convince with not
significant survival data with 4/5 animals survived compared with the control
group with 6/6 animals survived. There were no clear significant differences
in the surrogate markers (ALT, AST, GLDH, LDH) during the MP and
postoperatively between the H-MPS/O-MPS and B-MPS/controls.
