Ergebnisse nach Implantation der achsgeführten Knietotalendoprothese RT-PLUS™ Solution bei verschiedenen Indikationsstellungen

Abstract

Die gekoppelte Rotations-Knietotalendoprothese zur zementierten Implantation (RT-PLUS™ Solution) wird bei hochgradiger Destruktion des Kniegelenkes mit Knochendefekten, Kontrakturen, Instabilitäten, Osteopenie und Arthrofibrose verwendet. Der Kopplungsmechanismus der Prothese erlaubt eine begrenzte Rotation im Gelenk, was sich der physiologischen Artikulation nähert. In einer retrospektiven Studie wurden 136 Patienten mit 155 Primär- und Revisionsimplantationen und einem mittlerem follow-up von 22,8 Monaten klinisch und röntgenologisch nachuntersucht. Der postoperative Punktzuwachs betrug beim HSS-Score 53,65 Punkte, beim KSS-Score 118,97 Punkte. Die postoperative Flexion lag bei durchschnittlich 126,5°. 94,2% der Patienten mit Primärimplantation waren subjektiv zufrieden und sehr zufrieden. Die Gesamtkomplikationsrate lag bei 13,5%. 47,6% aller Komplikationen betrafen den Streckapparat. Die Anwendung der RT-PLUS™ Solution- Knie-TEP ermöglicht durch ihre Konstruktion eine zuverlässige Achskorrektur, eine Stabilisierung des Gelenkes auch bei sehr kritischen Weichteilinsuffizienzen und erlaubt gleichzeitig einen überdurchschnittlich hohen Bewegungsumfang. Die Infektionsrate lag nicht höher, als in der Literatur beschrieben. Die Patellakomplikationen können durch Prothesenpositionierung und Weichteilmanagement sicher vorgebeugt werden.

The hinged rotational knee system for cemented implantation (RT-PLUS™ Solution) is used in highly destructed knee joints with osseous defects, contractures, instabilities, osteopenia and arthrofibrosis. The hinge mechanism allows a limited joint rotation which comes close to physiological articulation. In a retrospective study 136 patients including 155 primary and revisional implantations were clinically and radiologically evaluated with a mean follow-up of 22,82 months. The postoperative HSS-score improved by 53,65 points, the KSS-score by 118,97 points. The postoperative knee flexion averaged 126,5°. 94,2% of the patients with primary implantation were subjectively content and very content. The overall complication rate was at 13,5% of which 47,6% referred to the extender tendon complex. Due to its construction, the implementation of the RT-PLUS™ Solution knee system provides at the same time a reliable axis correction, joint stabilisation even in critical soft tissue insufficienties and a range of motion above average. Infection rate was not higher than described in literature. Patella complications can be securely avoided by protheses positioning and soft tissue balancing.
