In der vorliegenden Studie sollte untersucht werden, ob die Verwendung einer rotierenden oder festen Plattform in der Endoprothetik des Kniegelenks einen relevanten Einfluss auf das postoperative Ergebnis hat. Dazu wurden 84 Patienten klinisch und radiologisch nachuntersucht, die zwischen 1998 und 2001 einen endoprothetischen Ersatz des Kniegelenks in der orthopädischen Klinik der Charité erhalten hatten. Potentielle weitere Einflussfaktoren auf das postoperative Ergebnis, wie der operative Zugangsweg, die Durchführung eines lateralen Releases sowie das Prothesenalignment, wurden ebenfalls erfasst. Das postoperative Ergebnis wurde mittels etablierter standardisierter klinischer Scores (Knee Society Score, WOMAC, SF-36) quantifiziert. Das Implantatalignment wurde an Ganzbeinaufnahmen im Stand ermittelt. Die Haupthypothese einer Beeinflussung des postoperativen Ergebnisses durch die Verwendung einer rotierenden vs. fixen Plattform konnte für alle untersuchten Parameter zurückgewiesen werden. Das Patientenalter hingegen hatte einen Einfluss auf das klinische Ergebnis und die Längenänderung des Patellabandes, welche bei fast allen Patienten jedoch in unterschiedlicher Ausprägung nachgewiesen werden konnte. Nebenbefundlich wurde festgestellt, dass die Inzidenz einer Bluttransfusion mit der Implantatgröße stieg. Die Verwendung des invasiven Subvastuszuganges war, wahrscheinlich als Folge der im Vergleich zum Standardzugang reduzierten Übersicht, mit einer erhöhten Inzidenz varischer Implantationen vergesellschaftet. Ein Zusammenhang zwischen dem Implantatalignment in der Frontalebene und dem klinischen Ergebnis konnte jedoch nicht nachgewiesen werden. Ebenfalls überraschend war, dass der präoperative klinische Score kein Prädiktor des postoperativen Ergebnisses war.
In the present study should be examined whether the use of a rotary or firm platform has a relevant influence on the postal-surgical result in knee endoprosthetics. In addition 84 patients were given a further checkup clinically and radiologically who had received between 1998 and 2001 an knee endoprosthetics. Potential other factors of influence on the postal-surgical result, like the surgical access road, the realisation of lateral releases as well as the alignment of the knee endoprosthetics., were also grasped. The postal-surgical result was quantified by means of established standardised clinical scores (Knee Society Score, WOMAC, SF-36). The alignment was determined in whole leg admissions in the state. The main hypothesis of an influencing of the postal-surgical result of the use rotary versus fixed platform could be rejected for all examined parametres. Only, the patient's age had an influence on the clinical result and the length change of the patella ligament, which could be proved with almost all patients, nevertheless, in different stamping. Also was found out that the inzidenz of a blood transfusion by the implant size rose. The use of the subvastusaccess was linked, probably as a result of the overview diminished in comparison to the standard access, with an increased inzidenz of varus implantations. A connection between the alignment in the frontal level and the clinical result could not be proved. The presurgical clinical score had no influence on the postal-surgical result.
