Primäre Hochdrucktherapie mit einer Fixkombination: Sicherheits- -und Wirkungsaspekte in der Akut- und Dauerapplikation

Abstract

Diese Arbeit befasst sich mit der Prüfung der Arzneimittelsicherheit bei der Behandlung der Hypertonie. In einer Pilotstudie mit 149 Patienten mit primärem Einsatz der Fixkombination Ramipril-Felodipin wurde insbesondere das Auftreten von starken Blutdrucksenkungen (<100mmHg) und das Auftreten von Blutdruckabfällen (systolisch >30mmHg) untersucht. Ferner wurden die orthostatischen Potenzen der Kombination mit denen der Einzelpräparate in der Monotherapie verglichen. Das Blutdruckverhalten während der Langzeitanwendung über 4 Wochen wurde mittels 3 verschiedener Messverfahren bewertet: Office- Messung, ambulante Selbstkontrolle und ABDM. Dabei wurde ein besonderes Augenmerk auf die frühen Morgenstunden gelegt. Es wurden der systolische und diastolische Blutdruck, die Herzfrequenz und der Pulsdruck beurteilt. Die Auswertung de Ergebnisse hat gezeigt, daß die Fixkombination signifikant stärker wirkt als die Einzelsubstanzen. Sie ist sicher und kann für den primären Einsatz bei der Behandlung der Hypertonie empfohlen werden.

This paper examines the pharmaceutical drug safety during treatment of hypertension. In a pilot study the fix combination of Ramipril-Felodipin was applied to 149 patients. In the process special attention was turned to the occurrence of strong blood pressure droppings (<100 mmHg) and decline in systolic blood pressure by more than 30 mmHg. In addition orthostatic events have been compared to the application of the single compounds in mono therapy. The development of blood pressure during long term treatment (> 4 weeks) was monitored using 3 measurement methods; Office measurement, ambulatory self measurement and ABDM. Thereby special attention was drawn to systolic and diastolic blood pressure as well as heart rate and pulse pressure in the small hours. The evaluation of the results showed a significantly better performance of the fix combination compared to the single compounds. The fix combination of Ramipril-Felodipin guarantees patient safety and can be recommended for the primary application in hypertension treatment.
