HIV-1-Variabilität als Antwort auf den Selektionsdruck durch zytotoxische Lymphozyten: eine Genanalyse

Abstract

Die Entdeckung von HIV-1 und HIV-2 als den Erregern von AIDS liegt nun mehr als 20 Jahre zurück und noch immer ist diese neuzeitliche Infektionskrankheit nicht heilbar oder mit einem Wirkstoff prophylaktisch zu bekämpfen. Das HI- Virus ist ein in hohem Maße optimiertes System, das mit nur wenigen Genen absolut effizient funktioniert. Dabei erfüllt jedes virale Protein in vivo eine für das Virus bedeutsame Funktion und bietet einen potentiellen Ansatz für antiretrovirale Therapien. Da akzessorische und regulatorische Proteine im Replikationszyklus des HIV früh entstehen – damit also in die Virusvermehrung zeitig eingegriffen werden könnte – stellen sie darüber hinaus ein attraktives Ziel für zellvermittelte antivirale Impfstrategien dar. Nach dem bisher vergeblichen Versuch, einen Impfstoff zu entwickeln, der mittels Induktion einer humoralen Abwehr vor der Infektion mit dem Retrovirus schützt, konzentriert sich die Forschung zunehmend auf die Induktion einer zellulären Abwehr. Diese wird durch zytotoxische T Lymphozyten (CTL) vermittelt, die infizierte Zellen anhand der auf der Zelloberfläche in den Bindungstaschen der MHC-Moleküle präsentierten viralen Proteine spezifisch erkennen und abtöten. Zur Entwicklung eines auf zellulärer Abwehr basierenden Impfstoffes ist also sowohl der HLA-Typ des Wirtes von Bedeutung, der die Form und Bindungseigenschaften der MHC-Moleküle bestimmt, als auch die entsprechend präsentierten viralen Peptide. Als Reaktion auf den antiviralen Druck durch die CTL verändert sich das Virusgenom im Bereich der Epitope, um von dem Immunsystem nicht mehr erkannt zu werden. Diese als „Fluchtmutationen“ bezeichneten Veränderungen des HIV-Genoms erhöhen dessen Variabilität und erschweren die Generierung eines nachhaltig wirksamen und effizienten Impfstoffes. Die vorliegende Arbeit ist thematisch einzuordnen in die Entwicklung eines prophylaktischen oder therapeutischen Wirkstoffes, der auf der Induktion einer zellulären Immunantwort basiert. Ziel war die genaue Analyse von CTL-Epitopen in den akzessorischen Proteinen Vif, Vpr, Vpu und den regulatorischen Proteinen Rev, Tat vor dem Hintergrund des individuellen HLA- Typs des Wirtes. Die Hypothese war, dass aufgrund des antiviralen Drucks durch spezifische CTL, die Aminosäurevariabilität innerhalb der CTL-Epitope erhöht ist. Dazu erfolgte die HLA-Typisierung von HIV-infizierten Patienten mit anschließender Auswahl von 11 HLA A2-positiven und sechs HLA A2-negativen Patienten (zehn Langzeitinfizierte, sieben akut Serokonvertierte). Es wurden von 19 Patientenproben (17 Patienten) 180 HIV-Klone sequenziert und die resultierenden 900 Proteinsequenzen qualitativ und statistisch analysiert. In der qualitativen Analyse der CTL-Epitope und der örtlichen Variationen der Proteinsequenzen konnte keine eindeutige Korrelation zwischen Variabilität und Epitoplokalisation festgestellt werden. Die HLA A2-spezifische Auswertung zeigte nur singulär auffallende Ergebnisse: so weist z.B. ein CTL-Epitop in Vpr eine eindeutig erhöhte Variabilität in den A2-positiven Proben von Langzeitinfizierten (LTNP) im Vergleich zu den A2-negativen auf, was auf Fluchtmutationen hinweist. Ein umfassendes Muster der Variationen im Sinne von gezielten Fluchtmutationen lässt sich jedoch nicht erkennen. Bei Gegenüberstellung der HIV-Sequenzen von Proben nach unterschiedlicher Infektionsdauer war zu erkennen, dass die Zahl der Quasispezies in einigen Proteinsequenzen bei der Gruppe der akut Serokonvertierten erhöht war. Dies deutet darauf hin, dass sich die dominante Virusform nach kurzem Infektionsverlauf noch nicht durchsetzen konnte. Auffällig war außerdem das oben bereits erwähnte CTL-Epitop in Vpr, das unter den akut Serokonvertierten ob A2-positiv oder A2-negativ – keine Differenzen zeigte. Hier haben spezifische CTL möglicherweise noch keinen effizienten Selektionsdruck ausgeübt oder die Zeit zur Ausbildung von Escape-Varianten war nicht ausreichend. Bei Analyse von mehreren Blutproben eines Patienten im Infektionsverlauf gelang die Dokumentation der Selektion der dominanten Quasispezies, sowie die des Auftretens einer möglichen Fluchtmutation innerhalb eines CTL-Epitops. Ebenso zeigte sich eine stets gekoppelt auftretende Aminosäurevariation als möglicher Hinweis auf eine kompensatorische Mutation. In der statistischen Auswertung konnte die Hypothese, die von einer erhöhten Aminosäurevariabilität innerhalb der CTL- Epitope ausging, nicht bestätigt werden. Statistisch signifikante Ergebnisse konnten nicht durchgehend erhoben werden. Die Zahlen deuten darauf hin, dass die Variabilität innerhalb der Epitope im Vergleich der außerhalb der Epitope in den hier untersuchten Genen und Genabschnitten nicht erhöht ist. Für eindeutigere Aussagen sind Untersuchungen mit einer größeren Anzahl an analysierten HIV-Sequenzen notwendig. Die erhobenen Daten bieten jedoch bereits eine gute Grundlage für weitere Studien, insbesondere für solche, die die zelluläre Ebene in die Analyse einbeziehen.

The discovery of HIV-1 and HIV-2 as the causative agent of AIDS occurred 20 years ago, and despite intensive efforts worldwide all attempts to develop an effective AIDS vaccine or cure have so far failed. HIV is a highly optimised system operating with only a few genes in a highly efficient manner. Each viral protein plays a significant role in vivo and therefore offers a potential target for anti-retroviral therapy. Accessory and regulatory proteins are produced early in the replication cycle of HIV and are therefore good targets for the early inhibition of virus reproduction and an attractive basis for antiviral vaccines. After the early failure of vaccines designed to stimulate a humoural immune response, research has increasingly concentrated on the induction of HIV-specific cellular immunity mediated by cytotoxic T-lymphocytes that recognise infected cells via viral protein fragments presented by MHC-molecules. The efficacy of such a vaccine in humans would depend on the HLA-type of the host as this determines the pattern of epitope recognition and binding/presentation capacity. In response to the antiviral pressure exerted by CTLs, HIV can mutate in regions coding for the presented epitopes in order to avoid recognition by the immune system. Such genomic- variants are termed “escape-mutations” and severely complicate the development of a long-term, efficient vaccine. The project described in this thesis is thematically aimed at the development of a prophylactic or immunotherapeutic agent against HIV by induction of cellular immunity. The aim was to accurately analyse CTL-epitopes within the accessory proteins Vif, Vpr, Vpu and the regulatory proteins Rev and Tat against the background of the host's individual HLA-haplotypes. The working hypothesis was that as a result of the antiviral pressure exerted by specific CTLs the variability of the amino acids sequence within such epitopes is increased compared to those regions not recognised by CTLs. To accomplish this, HIV-positive patients were HLA-typed and 11 HLA A2-positive and six HLA A2-negative patients (ten long-term infected, therapy-naïve patients and seven acute seroconverters) were selected. Altogether 180 HIV clones from 19 samples (17 patients) were sequenced and the corresponding 900 protein sequences qualitatively and statistically analysed. The analysis of CTL-epitopes and their variation in the different proteins revealed no clear correlation between variability and localisation within epitopes. The HLA-A2-specific analysis showed only individual cases of correlation. For example, the variability in one of the CTL-epitopes in Vpr was significantly higher in viruses from A2-positive long- term infected patients compared to those from A2-negative patients – suggesting possible escape-mutations. However, a clear and generalised pattern of protein variation with regard to specific escape-mutations could not be identified. HIV sequences from samples taken after a short period of infection (acute seroconverter cohort) showed a relatively elevated number of quasispecies. This suggests that the dominant quasispecies still hadn't been selected for during the short time of infection. It was also noticeable that the CTL epitope in Vpr mentioned above showed in neither A2-positive nor A2-negative patients any increase in variability. It is likely that either the specific CTLs had not expanded enough to exert an effective selective pressure or that the virus had not yet had time to develop a dominant population of escape mutants. By analysing several samples from one patient during the course of infection, it was possible to document the selection of the dominant quasispecies, as well as the appearance of a probable escape mutation within a CTL epitope. Similarly, the appearance of a variation always in tandem with a second variant suggested the forced development of a compensatory mutation. It was not possible by statistical analysis of all the data to verify the hypothesis that the amino acid variability within the CTL-epitopes of the proteins studied is significantly elevated. The statistically significant differences were limited to individual cases. Overall, there was no evidence that variability within epitopes is generally higher than in regions not recognised by CTLs. To confirm this finding it will be necessary to analyse a larger number of HIV sequences. However, the results presented here provide a solid basis for future studies, particularly for those including an actual examination of the cell-mediated immune responses in the patients.
