Letalität bei neonatologischen Intensivpatienten mit nosokomialer Infektion

Abstract

Hintergrund: Nosokomiale Infektionen bei Frühgeborenen sind eine wichtige Ursache für neonatale Letalität. Gerade bei sehr kleinen neonatologischen Intensivpatienten mit einem Geburtsgewicht unter 1.500g (very low birth weight; VLBW) ist der Erwerb einer nosokomialen Sepsis bzw. nekrotisierenden Enterokolitis (NEC) mit einer erhöhten Letalität verbunden. Bisher existieren jedoch nur wenige Untersuchungen, die sowohl Aussagen zur Gesamtletalität als auch zur „attributable mortality“ dieser beiden Krankheitsbilder bei VLBW- Kindern in Deutschland erlauben. Zielstellung: Im Rahmen dieser Studie sollten folgende Fragen beantwortet werden: (1) Wie hoch ist die absolute Letalität bei VLBW-Kindern mit einer nosokomialen Infektion und gibt es eine Assoziation zwischen Geburtsgewicht und Gestationsalter zur Letalität? (2) Wie hoch ist die „attributable mortality“ durch die nosokomiale Infektion und in welchem Maße erhöht sich die Letalität bei Auftreten einer nosokomialen Infektion? Methode: Zur Beantwortung der Fragen wurden Daten von 229 neonatologischen Abteilungen, die zwischen 2000 und 2011 an NEO-KISS, dem deutschen Krankenhaus-Infektions-Surveillance-System für nosokomiale Infektionen bei Frühgeborenen auf neonatologischen Abteilungen, beteiligt waren, untersucht. Zur Berechnung des Letalitätsrisikos wurde eine prospektive Kohortenstudie mit der Cox-Proportional Hazard Regression mit den zeitabhängigen Variablen Sepsis/NEC durchgeführt. Zur Berechnung der „attributable mortality“ wurde zusätzlich eine gematchte Fall-Kontroll-Studie durchgeführt. Ergebnisse: Insgesamt wurden Daten von 43.116 VLBW-Kindern in die Kohortenanalyse einbezogen. Die Gesamtletalität in der Kohorte (bis zum Erreichen von 1.800g, Tod oder Verlegung des Kindes) betrug 6,6%. Die Letalität bei Kindern mit mindestens einer nosokomialen Sepsis lag bei 5,6%, beim Vorliegen einer NEC betrug die Letalität 19,2%. Die adjustierte Cox-Proportional Hazard Regressionsanalyse ergab, dass die Sepsis (HR=1,83 95%KI 1,61-2,08) und die NEC (HR=6,35 95%KI 5,47-7,38) unabhängig voneinander mit einer Erhöhung der Letalität verbunden sind. In der Fall-Kontroll-Studie wurden 5.187 (75,1%) Sepsis-erkrankte Kinder 1:1 mit nicht erkrankten Kindern gematcht. Die Letalität in der Fall-Gruppe betrug 4,9%, die in der Kontroll-Gruppe 3,5%, woraus sich für die Sepsis eine „attributable mortality“ von 1,4% (95% KI 0,69-2,17) ergab. Das nach dem Geburtsgewicht adjustierte Odds Ratio der logistischen Regressionsanalyse lag bei 1,28 (95%KI 1,00-1,64). Für die 1.092 (85,9%) gematchten Kinder mit einer NEC lag die Letalität bei 18,2%, in der Kontrollgruppe betrug sie 3,6%. Die „attributable mortality“ lag demnach bei 14,7% (95% KI 12,22-17,08), das adjustierte Odds Ratio war 7,96 (95% KI 5,16-12,28). Schlussfolgerungen: Die nosokomiale Sepsis und NEC führten zu einer erhöhten Letalität bei VLBW-Kindern. Die Sepsis war mit einer relativ niedrigen „attributable mortality“ assoziiert, wohingegen die NEC mit einer hohen „attributable mortality“ von 14,7% verbunden ist.

Background: Nosocomial infections in preterm infants are an important cause of neonatal mortality. The occurrence of both primary bloodstream infection (BSI) and necrotizing enterocolitis (NEC) is potentially associated with increased mortality, especially in very low birth weight infants (VLBW). There is scant data on both mortality and attributable mortality caused by BSI and NEC among VLBW infants in Germany Aim: To conduct a study to determine crude mortality and attributable mortality associated with nosocomial infections among VLBW infants. Methods: Surveillance data from the German national surveillance system for nosocomial infections in neonates NEO-KISS, collected between 2000 and 2011 by 229 neonatology departments, were used. To determine the mortality risk a prospective cohort study with Cox Proportional Hazard Regression model with time dependent variables BSI/NEC was conducted. In addition, a matched case control study was performed to calculate attributable mortality. Results: A total of 43.116 VLBW infants were included in the cohort analysis. Overall mortality (until achieving 1.800g, transfer or death of the infant) was 6.6%. Infants with at least one BSI had a mortality of 5.6%, with at least one NEC 19.2%. The Cox Proportional Hazard Regression analysis with adjustment for all available risk factors revealed that BSI (HR=1.83; CI95 1.61-2.08) and NEC (HR=6.35; CI95 5.47-7.38) are independently associated with increased mortality. 5.187 (75.1%) BSI infants were matched 1:1 to unexposed infants. Crude mortality was 4.9% among exposed and 3.5% among unexposed infants and lower than in the entire cohort. Attributable mortality was 1.4% (CI95 0.69-2.17). The adjusted odds ratio of the conditional regression analysis was 1.28 (CI95 1.00-1.64) and lower as the adjusted HR of Cox regression in all patients. For 1.092 (85.9%) matched NEC infants crude mortality was 18.2% among exposed and 3.6% among unexposed infants and lower than in the entire cohort. Attributable mortality was 14.7% (CI95 12.22-17.08) and the adjusted odds ratio was 7.96 (CI95 5.16-12.28) and higher as the adjusted HR of the Cox regression in all patients. Conclusion: BSI and NEC increased mortality in VLBW infants. BSI was associated with a relatively small attributable mortality, whereas NEC had a high attributable mortality of 14.7%.
