Postoperative Infektionen, Sepsis und Multiorganversagen sind direkt
assoziiert mit einer überschießenden inflammatorischen Reaktion und stellen
ein Problem bei Leberteilresektionen und anderen größeren
abdominalchirurgischen Eingriffen dar. Gekennzeichnet ist diese Reaktion durch
ein Missverhältnis von pro- und antiinflammatorischen Zytokinen. Ziel dieser
Arbeit war es, den Einfluss der präoperativen Hochdosis-Steroidgabe auf die
postoperative Immunfunktion zu untersuchen. Hauptzielkriterium war der
postoperative IL-6-Plasmaspiegel. Gemessen wurden die proinflammatorischen
Zytokine IL-6, IL-8 und TNF-a, das antiinflammatorische Zytokin IL-10, die
monozytäre HLA-DR-Expression und die TNF-a-Synthese nach LPS-Stimulation. Die
Bestimmung von ALT, AST, Albumin und TPZ-INR sowie t-Bilirubin, AP und ?-GT
diente zur Beurteilung der postoperativen Leberfunktion, Synthesestörungen
bzw. Cholestase. CRP und die Leukozytenanzahl als Entzündungsparameter sowie
Kreatinin zur Beurteilung der postoperativen Nierenfunktion wurden ebenfalls
bestimmt. Der postoperative klinische Verlauf wurde anhand der postoperativen
Komplikationen, der Rekonvaleszenz und der Dauer des postoperativen
Krankenhausaufenthaltes bewertet. In die Studie wurden 20 Patienten
eingeschlossen und im Verhältnis 10:10 doppelblind randomisiert auf eine
Steroid- bzw. Kontrollgruppe verteilt. Die Patienten der Steroidgruppe
erhielten präoperativ 30 mg/kg KG Methylprednisolon, in der Placebogruppe
wurde NaCl 0,9% verabreicht. Alle Leberteilresektionen wurden ohne Pringle-
Manöver durchgeführt. Die Ergebnisse zeigen in der Steroidgruppe signifikant
niedrigere IL-6 und IL-8-Spiegel, sowie leicht supprimierte TNF-a- und
IL-10-Konzentrationen. Die TNF-a-Konzentration nach LPS-Stimulation war in der
Steroidgruppe 4 Stunden postoperativ signifikant erniedrigt, die monozytäre
HLA-DR-Expression zeigte keine signifikanten Unterschiede. CRP war in der
Steroidgruppe vom 1.-4. postoperativen Tag signifikant niedriger als in der
Kontrollgruppe. Durch die Steroidgabe unbeeinflusst zeigten sich ALT und AST
sowie Albumin und TPZ-INR. t-Bilirubin war in der Steroidgruppe am 6.
postoperativen Tag signifikant niedriger. Der Krankenhausaufenthalt war in der
Steroidgruppe signifikant kürzer, doch wird dieses Ergebnis beeinflusst durch
die komplikationsbedingt verlängerte Aufenthaltsdauer einer Patientin in der
Kontrollgruppe. Bei den postoperativen Komplikationen und der Rekonvaleszenz
gab es keinen signifikanten Unterschied. Die Ergebnisse dieser Studie zeigen,
dass durch die einmalige hochdosierte Steroidgabe eine überschießende
postoperative inflammatorische Reaktion effektiv gehemmt wird und die
zelluläre Immunität dabei unbeeinträchtigt bleibt.

Postoperative infections, sepsis and multi organ failure are directly
associated with a hyperinflammatory reaction and mean a problem in
hemihepatectomies and other extended abdominal operations. This reaction is
characterized by a disproportion of pro- and anti-inflammatory cytokines.
Purpose of this study was to examine the influence of preoperative high-dose
methylprednisolone on postoperative immunofunction. Main criterion was the
postoperative IL-6-plasma level. The pro-inflammatory cytokines IL-6, IL-8 and
TNF-a, the anti-inflammatory cytokine IL-10, HLA-DR expression on monocytes
and TNF-a-synthesis after stimulation with LPS were measured. The
determination of ALT, AST, albumin and TPZ- INR as well as t-bilirubin, AP and
?-GT reflected postoperative liver function, synthesis disorders or
cholestasis. Also measured were the inflammation parameters CRP and leucocytes
as well as creatinine as a marker for postoperative kidney function. The
postoperative clinical outcome was assessed by postoperative complications,
reconvalescence and duration of postoperative hospital stay. In this study 20
patients were included and they were double-blind randomized distributed in a
ratio of 10:10 into a steroid and a control group. The patients of the steroid
group received a preoperative dose of methylprednisolone (30 mg/kg body
weight) and the patients of the control group got saline. All
hemihepatectomies were done without pringle-maneuver. The results of this
study showed significant lower levels of IL-6 and IL-8 in the steroid group as
well as low suppressed TNF-a- and IL-10- concentrations compared to the
control group. The TNF-a- concentration after LPS-stimulation was 4 h
postoperative significant lower in the steroid group. The HLA-DR expression on
monocytes showed no significant differences. CRP was significant lower from
the 1st to the 4th postoperative day in the steroid group compared to the
control group. ALT, AST, albumin and TPZ-INR were not influenced by
preoperative high-dose methylprednisolone; t-bilirubin was significant lower
in the steroid group on the 6th postoperative day. The duration of hospital
stay was significant shorter in the steroid group, but this result was
influenced by a prolonged stay of one patient in the control group because of
postoperative complications. There was no significant difference between both
groups relating to postoperative complications and reconvalescence. The
results of this study show that a hyperinflammatory reaction is effectively
inhibited by preoperative high-dose methylprednisolone and that the cellular
immunity is not impaired.
