Die chirurgische Resektion zur kurativen Behandlung primärer und sekundärer Lebermalignome strebt eine maximale Radikalität zur Erreichung eines tumorfreien Zustandes an. Hieraus ergibt sich eine Gratwanderung zwischen möglichst großem zu resezierenden Volumen auf der einen und dem Verhindern eines postoperativen Leberversagens auf der anderen Seite. Die präoperative radiologische Diagnostik, die Konditionierung durch die interventionell- radiologische Pfortaderembolisation und die Vorhersage des postoperativen Leberversagens sind daher von großer Bedeutung. Ziel der vorliegenden Studien war eine Optimierung der Pfortaderembolisation zur Erhöhung der Hypertrophierate bei gleichzeitig gutem Sicherheitsprofil und die Evaluation des Gd-EOB-verstärkten MRT als bildgestütztem Leberfunktionstest. In Originalarbeit 1 wurde die Pfortaderembolisation (PVE) mittels Polyvinylalkohol-Partikeln (PVA-Gruppe) mit einer zusätzlichen zentralen Okklusion durch einen Amplatzer Vascular Plug (AVP) und/oder Coils (AVP/Coil- Gruppe) hinsichtlich der Hypertrophierate des links-lateralen Sektors (Lebersegmente II + III) verglichen. Es konnten 40 Patienten in die PVA-Gruppe und 35 Patienten in die AVP/Coil-Gruppe eingeschlossen werden. Der prozentuale Volumenzuwachs in der PVA-Gruppe betrug 30,1 % und 53,3 % in der AVP/Coil- Gruppe-Gruppe und lag damit bei zusätzlicher zentraler Okklusion signifikant höher (p = 0,003). Die Komplikationsrate in der AVP/Coil-Gruppe war höher als in der PVA-Gruppe, es kam jedoch zu keiner Komplikation, die die anschließende Operation verzögerte oder unmöglich machte. In Originalarbeit 2 wurden die in der MRT und CT entstehenden Artefakte durch Coils mit denen des AVP vergleichen. Da das Embolisationsmaterial im Leberhilus an einer kritischen Stelle liegt, können Artefakte durch das Embolisationsmaterial die Beurteilung einer postinterventionellen Bildgebung erschweren. Bei 13 Patienten wurde eine Embolisation mit einem AVP und bei 7 Patienten eine Embolisation mit Coils durchgeführt. Die Artefakte durch Coils waren in der CT deutlich ausgeprägter als in den T1- und T2-gewichteten MRT-Sequenzen (p < 0,05). Der Artefaktdurchmesser durch den AVP war in den T1-Sequenzen höher als in den T2-Sequenzen und in der CT. Insgesamt war der Grad an Artefakten durch AVP jedoch so gering, dass keine Beeinträchtigung der Diagnosequalität bestand. Bei Coils bestand in der CT zum Teil eine eingeschränkte Beurteilbarkeit. Nun wurde der bisherige Goldstandard an bildgestützten Leberfunktionstests – die hepatobiliäre Sequenzszintigraphie – untersucht. Ein neues Untersuchungsprotokoll, bei dem der szintigraphische Detektor in rechts anterior-obliquer Position ausgerichtet war, wurde in Originalarbeit 3 evaluiert. Der Ausrichtungswinkel hierfür wurde direkt auf dem Untersuchungstisch im vorherigen Niedrigdosis-CT bestimmt. Es wurden 14 Patienten in die Studie eingeschlossen. Für die strenge ap-Orientierung wurde ein relatives Überlappungsvolumen bezogen auf das Gesamtlebervolumen von durchschnittlich 31,3 % ermittelt, bei dem rotierten Detektor mit dem Winkel aus dem vorherigen Schwächungskorrektur-CT betrug es 3,6 %. Eine seitengetrennte szintigraphische Leberfunktionsmessung sollte daher mit rechts anterior-oblique rotiertem Detektor durchgeführt werden. Um die Validität der Gd-EOB-verstärkten MRT als bildgestütztem Leberfunktionstest zu evaluieren, wurde in Originalarbeit 4 ein Vergleich mit der hepatobiliären Sequenzszintigraphie durchgeführt. Verglichen wurde die Bestimmung der seitengetrennten Funktion 14 Tage nach Pfortaderembolisation. Insgesamt konnten 14 Patienten eingeschlossen werden, die beide Untersuchungen innerhalb von 24h absolvierten. Der Mebrofenin Uptake korrelierte statistisch signifikant mit dem Relativen Enhancement (RE) und dem Hepatocellular Uptake Index (HUI) für beide Leberlappen. Die aus den jeweiligen Verfahren berechneten prozentualen Funktionsanteile aus den jeweiligen Verfahren korrelierten ebenfalls statisch signifikant miteinander (r2 = 0,775; p = 0,002). Basierend auf diesen Ergebnissen kann die Gd-EOB-verstärkte MRT die hepatobiliären Sequenzszintigraphie mittels 99mTc-Mebrofenin zur Bestimmung der seitengetrennten Leberfunktion ersetzen. Die folgenden beiden Studien beziehen sich auf die Gd-EOB-verstärkte MRT. In Originalarbeit 5 wurde der Effekt einer Flipwinkelerhöhung beim Gd-EOB-verstärkten MRT nach Pfortaderembolisation evaluiert. Hierfür wurden zwei vergeichbare T1-VIBE- Sequenzen mit 10° und 30° Flipwinkel jeweils vor und 20 Minuten nach intravenöser Applikation von Gd-EOB gemessen. Anschließend wurde für jedes Flipwinkelpaar das relative Enhancement (RE) ermittelt. In dieser Studie konnten zehn Patienten nach rechtsseitiger Pfortaderembolisation ausgewertet werden. Das RE des rechten Leberlappens betrug 0,52 bei 10° Flipwinkel und 1,41 bei 30°. Das RE des linken Leberlappens betrug 0,58 bei 10° und 2,05 bei 30°. In beiden Leberlappen war das RE bei 30° Flipwinkel signifikant höher als bei 10°. Die Flipwinkelerhöhung auf 30° führt zu einem verbesserten Kontrastmittelsignal und dadurch zu einer Erhöhung der Sensitivität bei der Detektion kleinerer Unterschiede in der Kontrastmittelaufnahme. Zuletzt konnte in Originalarbeit 6 der Funktionszuwachs des nicht-embolisierten Leberanteils nach der Pfortaderembolisation gezeigt werden. Dieser Funktionsanstieg kann durch die Volumetrie nicht adäquat abgebildet werden, da er die bloße Hypertrophierate übersteigt. Hierfür wurden zehn Patienten unmittelbar vor und 14 Tage nach rechtsseitiger PVE mittels Gd-EOB-verstärktem MRT untersucht. Es wurde seitengetrennt für beide Untersuchungszeitpunkte das relative Enhancement (RE) und der Hepatocellular Uptake Index (HUI) berechnet. Das RE im rechten Leberlappen (RLL) betrug 1,95 vor PVE und 1,41 nach PVE, für den linken Leberlappen (LLL) betrug es 1,18 vor PVE und 2,05 nach PVE. Der HUI im RLL betrug 1.829 vor PVE und 1.511 nach PVE, für den LLL betrug er 1.006 vor PVE und 1.477 nach PVE, damit sank er im RLL statistisch signifikant ab und stieg im LLL statistisch signifikant an (p < 0,05). Schon 14 Tage nach PVE kommt es zu einer signifikanten Funktionsverschiebung, die mittels Gd-EOB- verstärktem MRT dargestellt werden kann. Eine wesentliche Limitation der hier vorgestellten Studien sind die z.T. relativ geringen Patientenzahlen. Diese sind primär dadurch zu erklären, dass die erweiterte Hemihepatektomie eine selten durchgeführte Operation ist und die häufig körperlich eingeschränkten Patienten vielfach nicht in der Lage sind, zusätzliche Untersuchungen zu absolvieren. Die vorgestellten Studien konnten sowohl die Effektivität, die Sicherheit als auch die geringe Beeinträchtigung einer folgenden Bildgebung des Amplatzer Vascular Plug zur zentralen Okklusion bei der Pfortaderembolisation belegen. Zudem liefern die Ergebnisse zur Gd-EOB- verstärkten MRT zukunftsweisende Anwendungsfelder für einen neuen bildgestützten Leberfunktionstest, nachdem er im Modell der Pfortaderembolisation mit ihrer künstlich herbeigeführten Funktionsinhomogenität evaluiert wurde.
Extensive right liver resection is an established technique for achieving radical tumour removal and to improve recurrence-free and overall survival. To increase the number of resectable tumours, a preoperative portal vein embolisation is an accepted strategy in the case of small FRL. Embolisation of the right portal vein leads to a measurable increase in left liver lobe volume facilitating safe extended right hemihepatectomy after a growth interval of approximately 3-6 weeks. Combination of volumetry and global liver function tests are unsuitable for predicting FRL function after PVE because the function is distributed unequally between the left and right liver lobe. The following studies were performed to optimize portal vein embolization technique and to evaluate Gd-EOB-enhanced MRI as an imaging-based liver function test. Purpose of study 1 was to retrospectively analyze efficacy as measured by volume gain of future remnant liver (FRL) after right portal vein embolization (PVE) using particles only versus particles and additional central plug and/or coil (CP/C) embolization. All patients who underwent PVE between July 2011 and December 2012 were retrospectively analyzed. Right PVE was performed either with particle-only (PO) embolization or additional CP/C embolization. All enrolled patients underwent computed tomography or magnetic resonance imaging before PVE and surgery. The images were used for volumetry of the FRL. Of 75 patients, 40 had PO and 35 CP/C embolization. Age, sex, and tumor entities did not differ significantly between the two groups. Tumor entities included cholangiocarcinoma (n = 52), metastasis from colorectal cancer (n = 14), hepatocellular carcinoma (n = 2), and others (n = 7). Time from PVE to preoperative imaging was similar in both groups. FRL volume before PVE was 329±121 ml in the PO group and 333±135 ml in the CP/C group, and 419 ± 135 ml and 492 ± 165 ml before operation. The average percentage volume gain was significantly higher in the CP/C group than in the PO group, with 53.3 ± 34.5 % versus 30.9 ± 28.8 % (p = 0.002). Right PVE with additional CP/C emboliza- tion leads to a significantly higher gain in FRL volume than embolization with particles alone. Purpose of study 2 was to retrospectively analyze the degree of artefacts in CT and MRI scans caused by coils and vascular plugs used for portal vein embolization (PVE). All patients who underwent PVE between July 2011 and December 2012 and received either plug or coil embolization additional to particle embolization were retrospectively ana- lyzed. Artefacts causes by embolization materials were measured in CT and MRI scans following PVE. The sum of the artefact diameters caused from coils was significantly higher in CT (188.3 ± 26.1 mm) than in MRI T1 (50.5 ± 6.5 mm) or T2 imaging (39.5 ± 9.7 mm) (P < 0.05). In MRI, the diameter in T1 and T2 sequences did not differ significantly (P = 0.977). The sum of the artefact diam- eters from vascular plugs in MRI T1 sequences (62.5±8.7mm) was significantly higher than in CT (46.6 ± 4.8 mm; P < 0.05) and MRI T2 sequences (52.8 ± 3.8 mm; P < 0.05). PVE with particles and vascular plug causes significantly fewer artefacts than PVE with par- ticles and coils on CT scans following embolization, which can be important in the event of vascular complications or in follow-up scans, should the patient become unresectable. To correctly display the left and right liver lobe separately on dynamic projection scintigraphy, it is essential to adjust the collimator to the angle of the plane between the two liver lobes. We propose an optimized protocol for separating left and right liver lobe in 99mTc- mebrofenin hepatobiliary scintigraphy in a hybrid SPECT- CT device. The protocol uses the inherent attenuation correction low-dose CT (AC-CT) for individually adjusting gamma camera head angulation. The results of this proto- col are compared with hypothetical results based on pre- vious MRI, fixed angle, and traditional frontal projection. The absolute and relative degrees of overlap- ping volume between left and right liver lobe parenchyma for frontal projection, 45° right anterior oblique (RAO) projection, RAO angulation based on previously acquired MRI, and RAO based on the AC-CT were measured in 14 patients who underwent 99mTc-mebrofenin hepatobiliary scintigraphy. Relative degree of overlap was 31.3 ± 15.2 % for frontal projection, 8.2 ± 6.5 % for 45° RAO, 5.5 ± 3.5 % for RAO based on previous MRI, and 3.6 ± 2.5 % for RAO based on AC-CT. The relative overlap of RAO projections based on previous MRI was significantly lower than for frontal projection (p \ 0.05). Use of the angle from the prior AC-CT, however, resulted in an even lower degree of overlap (p \ 0.05). Performing 99mTc-mebrofenin hepatobiliary scintigraphy using RAO detector alignment with an angle derived from a prior CT obtained in the SPECT-CT scan- ner can significantly reduce the degree of overlap between right and left liver lobe. If SPECT-CT is not available, previous CT or MRI or a fixed angle of 45° may be used. Objective of study 4 was to compare Gd-EOB-enhanced MRI and 99mTc- mebrofenin hepatobiliary scintigraphy (HBS) as imaging- based liver function tests for separate evaluation of right (RLL) and left liver lobe (LLL) function. Fourteen patients underwent Gd-EOB-enhanced MRI and 99mTc- mebrofenin HBS after portal vein embolization within 24 h. Relative enhancement (RE) and hepatic uptake index (HUI) were determined from MRI; and Tmax, T1/2 and mebrofenin uptake were determined from HBS, all values separately for RLL and LLL. Mebrofenin uptake correlated significantly with HUI and RE for both liver lobes. There was strong correlation of mebrofenin uptake with HUI for RLL (r2 = 0.802, p = 0.001) and RE for LLL (r2=0.704, p=0.005) and moderate correlation with HUI for LLL (r2=0.560, p=0.037) and RE for RLL (r2 = 0.620, p = 0.018). Correlating the percentage share of RLL function derived from MRI (with HUI) with the percentage of RLL function derived from mebrofenin uptake revealed a strong correlation (r2 = 0.775, p = 0.002). Both RE and HUI correlate with mebrofenin uptake in HBS. The results suggest that Gd-EOB- enhanced MRI and 99mTc-mebrofenin HBS may equally be used to separately determine right and left liver lobe function. Study 5 was designed to because the optimal sequence for Gd-EOB-DTPA uptake measurement in the liver with the purpose of liver function measurement is still not defined. Objective was to prospectively evaluate the effect of an increased flip angle (FA) of a T1-weighted fat-saturated 3D sequence for the measurement of hepatocyte uptake of Gd-EOB-DTPA magnetic resonance imaging (MRI) after right portal vein embolization (PVE).: T10 patients who received a PVE prior to an extended hemihepatectomy were examined 14 days after PVE using Gd-EOB-DTPA enhanced MRI of the liver using the standard FA of 10° and the increased FA of 30°. Relative enhancement of the right liver lobe (RLL) was 0.52 ± 0.12 for 10° and 1.41 ± 0.39 for 30°. Relative enhancement of the left liver lobe (LLL) was 0.58 ± 0.11 for 10° and 2.05 ± 0.61 for 30°. Relative enhancement of the RLL was significantly higher for 30° than for 10° (p = 0.009) and significantly higher in the 30° than in the 10° sequences (p = 0.005) for the LLL. A flip angle of 30° increases the contrast between liver partitions with and without portal venous embolization. Thereby, the sensitivity for differences in uptake intensity is increased. This could be of value for a more exact determination of differences in regional liver function and, consequently, the estimation of the future remnant liver function. The last study was designed to prospectively evaluate the early development of regional liver function after right portal vein embolisation (PVE) with Gd-EOB-DTPA- enhanced MRI in patients scheduled for extended right hemihepatectomy. Ten patients who received a PVE before an extended hemihepatectomy were examined before and 14 days after PVE using Gd-EOB-DTPA-enhanced MRI of the liver. In these sequences representative region of interest measure- ments were performed in the embolised right (RLL) and the non-embolised left liver lobe (LLL). The volume as well as hepatic uptake index (HUI) was calculated independently for each lobe. Relative enhancement 14 days after PVE decreased in the RLL and increased significantly in the LLL (P < 0.05). Average hepatic uptake index (HUI) for RLL was signifi- cantly lower 14 days after PVE than before PVE (P < 0.05) and significantly higher for LLL (P<0.05). A significant shift of contrast uptake from the right to the left liver lobe can be depicted as early as 14 days after right PVE by using Gd-EOB-DTPA-enhanced MRI, which could reflect the redirected portal venous blood flow and the rapid utilisation of a hepatic functional reserve.
