dc.description.abstract
Das Umgebungsmonitoring ist für die pharmazeutische Parenteralia Produktion
ein wichtiger aber schwieriger Faktor. Zum einen sind die verwendeten
mikrobiologischen Analysemethoden nicht besonders präzise, zum anderen ist es
sehr aufwendig die Vielzahl der anfallenden Daten zu beherrschen. Für eine
Chargenfreigabe müssen definierte Reinraumbedingungen durch das Monitoring
nachgewiesen werden. Fehler bei der Interpretation können fatale Folgen haben.
Entweder kann ein Qualitätsdefizit nicht oder zu spät entdeckt werden oder ein
hoher wirtschaftlicher Schaden durch eine zu Unrecht gesperrte Charge
entsteht. Die umfassende Beherrschung des Umgebungsmonitorings ist also ein
freigabe- und businesskritischer Faktor in der Produktion von parenteralen
Arzneimitteln. Für diese Anforderung wurde im Rahmen der Arbeit ein
umfassendes Konzept zur Verwaltung, Auswertung und Optimierung eines komplexen
Umgebungsmonitorings entwickelt. Das Konzept umfasst 4 Stufen, die am Beispiel
pharmazeutische Produktion der Schering AG (Berlin Charlottenburg) in die
Praxis umgesetzt worden sind: Entwicklung einer Datenbank zur
Prozessunterstützung und Auswertung Entwicklung einer Keimlandschaft anhand
der eine Bewertung von gemessenen KBE-Belastungen erfolgen kann Analyse von
verschiedenen Einflussfaktoren auf die Keimbelastung von Reinräumen
Entwicklung von Auswertealgorithmen, mit denen die Reinraumqualität
dargestellt und Qualitätsdefizite aufgespürt werden können. Über 300.000
Einzelergebnisse bilden die Grundlage für die Untersuchung verschiedener
Faktoren auf die Reinraumqualität. So konnte gezeigt werden, dass der
Prüfungszeitpunkt in einem unter Kontrolle befindlichen Reinraum, eine
untergeordnete Rolle spielt. Es gibt also keinen worst case Zeitpunkt für die
Probenahme. Der Einfluss von Tageszeiten, Jahreszeiten oder die gleichzeitige
Partikelbelastung in einem Reinraum beeinflusst die Keimbelastung nur sehr
gering. Es ist nicht möglich von einer Partikelbelastung auf eine
Keimbelastung zu schließen oder umgekehrt. Hingegen ist der Einfluss von
Personalstunden im Reinraum sowie die Qualität der Ausstattung der Reinräume
groß. Aus diesen Erkenntnissen wurden Auswertungsmethoden entwickelt, die die
Bewertung der Reinraumqualität vereinfachen. Mit Hilfe von Qualitätskennzahlen
kann die Reinraumqualität in verschiednen Verdichtungen dargestellt werden.
Mit der Kennzahl %OoS kann eine hohe Verdichtung von Messwerten erreicht
werden. Durch die Transformation eines Messwerts in eine % Grenzwert
Darstellung lassen die unterschiedliche Messwerte in einem Diagramm
zusammenfassen. Die Prozessfähigkeitsanalyse (cpk) kann die Monitoringbereiche
identifizieren, bei denen ein Qualitätsdefizit vermutet wird. Mittels einer
CUSUM / Target Betrachtung können im Detail Trends analysiert werden.
Langfristig wird durch eine Auswertung von Fehlerklassen ein kontinuierlicher
Verbesserungsprozess eingeleitet, der zu einer stetigen Verbesserung der
getroffenen Maßnahmen führt. Mit dieser umfassenden Kontrolle des
Umgebungsmonitorings ist auch eine Unterstützung der parametrischen Freigabe
von terminal sterilisierten Chargen möglich. Durch diese Arbeit konnte eine
wesentliche Verbesserung sowohl der Qualität als auch der Effektivität des
Reinraummonitorings erreicht werden.

dc.description.abstract
Environmental monitoring is an important but difficult factor for
pharmaceutical parenteral production. Neither the analytical methods which are
not very precisely nor the huge amount of data are easy to master. Defined
clean room conditions must be ensured by monitoring in order to release
production batch. Mistakes in Interpretation could have severe consequence.
Either is a lack of quality not or too late found or causes a economical
damage due to false rejected batches. The extensive control of the
environmental monitoring is therefore a release and business critical factor
for the production of parenteral drugs. For this task was a wide concept
developed to administer, evaluate and optimise a complex pharmaceutical
environmental monitoring. It covers 4 Steps which were implemented into the
pharmaceutical production of the Schering AG (Berlin Charlottenburg):
Development of a data base to facilitate the process and evaluation
Development of a micro-organism chart to asses measured CFU-burden
Evaluation of different influence factors an the CFU-burden Development of
evaluation algorithms to demonstrate quality status and to track down weak
quality. Over 300,000 monitoring test results from the production site built
the base for the analysis of the different factors of clean room quality. So
could be demonstrated that sampling time in an clean room which is under
control plays a minor role. There is no worst case sampling time. Also
daytime, seasons and number of air born particles have only an low impact to
CFU load of clean rooms. It is not possible to infer the count of air born
particles from air CFU or the other way around. But number of working hours in
the clean room and the design of the clean room matters. Evaluation methods
were derived from all data to make the assessment of clean room quality
easier. The clean room quality could be measured with quality key performance
indicators (KPI) in different aggregation levels. The KPI %OoS is suitable for
a high aggregation of test results. Values of a different character can be
combined in one graph by a %-limit transformation. The process capability
analysis (cpk) can identify monitoring areas with potential poor quality. The
CUSUM / Target method can analyse trend in detail. A long term continues
improvement process is supported by a failure class evaluation which lead to a
permanent rising effect of corrective actions. The same KPIs can be used to
investigate out of specification results with a data drill down. This can
limit the area of an occurred error and ensure appropriate corrective actions.
The is Risk of wrong decisions is lowered. The developed methods for data
assessment could also be used to support the parametric release of terminal
sterilised pharmaceutical products. This extensive control of the
environmental monitoring can also be used to facilitate the parametric release
of terminal sterilised batches. With this work a substantial improvement of
quality and effectiveness in the environmental clean room monitoring could be
achieved.
