Bei der Strahlentherapie des Prostatakarzinoms spielt die Positionierung der
Prostata vor der Bestrahlung eine entscheidende Rolle, um dem Karzinom eine
hohe Strahlendosis zuzuführen und das umliegende Gewebe zu schonen. Dafür wird
mit Hilfe einer Bildgebung die Lage der Prostata direkt oder indirekt
detektiert und dann an den Bestrahlungsplan angepasst (bildgesteuerte
Strahlentherapie - IGRT). Häufig werden heutzutage Goldmarker in die Prostata
implantiert, welche dann zur Detektion ebendieser verwendet werden. Die
Implantation der Marker stellt dabei einen invasiven, mit Kosten und
personellem Aufwand verbundenen Prozess dar. Um die Notwendigkeit der
Markerimplantation zu evaluieren, untersuchten wir die Strahlendosisverteilung
von Prostata, Rektum, Rektumhinterwand und Harnblase bei zwei
Patientengruppen. Zusätzlich wurde die Prostatamotilität zwischen und während
den Bestrahlungen überprüft. In die Untersuchung wurden 58 Patienten mit
Prostatakarzinom aufgenommen. Den Patienten der ersten Gruppe wurden Marker in
die Prostata implantiert, bei der zweiten Gruppe wurde die Organlage passiv an
Hand der Beckenknochen ermittelt. Die Markergruppe umfasste davon 15
Patienten, die knochenkonturbasierte Gruppe 43 Patienten. Das Alter der
Patienten reichte von 49 bis 80 Jahre (Mittelwert: 70 Jahre ± 7,4 Jahre).
Insgesamt wurden 480 Bestrahlungsfraktionen analysiert, davon 337 bei
Patienten ohne Goldmarker und 143 bei Patienten mit Goldmarkern. Als
Strahlentherapiegerät wurde die Tomotherapie (Accuray, Sunnyvale, Vereinigte
Staaten) verwendet. Als IGRT-Verfahren wurde eine Megavoltage-
Computertomographie (MVCT) angewandt. Zur Auswertung der einzelnen
Bestrahlungen wurde das Softwaremodul Planned Adaptive des Tomotherapiesystems
genutzt. Der Einsatz von Goldmarkern erhöhte die Genauigkeit der Bestrahlung
entlang der y-Achse um 4,5mm gegenüber der knochenbasierten Methode. Für die
Markergruppe ergab sich eine signifikant höhere Abdeckung der Prostata um ~
2Gy bei gleicher Belastung des Rektums und der Rektumhinterwand im
Hochdosisbereich, geringer Absenkung im Niedrigdosisbereich und verringerter
Strahlenexposition der Harnblase im Vergleich zur knochenkonturbasierten
Gruppe. Ebenso ließ sich zeigen, dass die intrafraktionelle Prostatamotilität
keinen signifikanten Einfluss auf die Abdeckung der Prostata ausübte. Als
wichtigster Einflussfaktor auf die Prostatamotilität hat sich die Differenz
von Plan- und online Rektumvolumen ergeben. Der Hauptvorteil der Marker liegt
in der Reduzierung von Ausreißern in vertikaler Richtung mit einer geringfügig
verbesserten Strahlenabdeckung der Prostata. Zur Minimierung der Ausreißer bei
knochenbasierter IGRT schlagen wir eine zusätzliche Korrektur mit Hilfe der
Rektumvorderwand vor, welche gut mit der vertikalen Prostatalage korreliert.
Ob die Verbesserung der Abdeckung durch Marker einen Benefit für das klinische
Outcome hat und die Nutzung der Rektumvorderwand zur Registrierung eine
Alternative für den Markereinsatz darstellt, bedarf weiterer Forschung.

dc.description.abstract
For radiotherapy of prostate cancer positioning of the prostate plays an
important role for exposing the carcinoma to a high radiation dose while
protecting the surrounding tissue. Therefore imaging is used to detect
directly or indirectly the prostate position and adapt it to the radiotherapy
plan (image guided radiotherapy - IGRT). For improved detection of the
prostate, intraprostatic goldmarkers are often used. To evaluate the need of
the markers, we analysed the radiation dose of the prostate, rectum, rectum
wall and bladder in two groups of patients. Additionally the prostate motility
was examined between and during the radiotherapy sessions. 58 patients with
prostate cancer participated in our study. The first group was provided with
intraprostatic markers, the second group had no markers, instead the pelvis
bones were used for indirectly detecting the prostate. The marker group
contained 15 patients, the bone based group 43 patients. The patients age
ranged from 49 to 80 years (average of 70 years ± 7,4 years). Totally 480
radiotherapy sessions were analyzed, 337 without markers and 143 in the marker
group. The radiotherapy system in our study was the Tomotherapy (Accuray,
Sunnyvale, USA). As IGRT-method we used a megavoltage-computertomography
(MVCT). For the analysis of the single radiotherapy sessions the software
“Planned Adaptive” was used. There was a significant higher dose coverage of
the prostate of ~ 2Gy in the marker group with no difference in the radiation
exposure of the rectum and rectum wall in the high dose range, low decrease of
exposure in the low dose range and a significant lower exposure of the bladder
in contrast to the group without markers. Furthermore the intrafractional
prostate motility had no significant influence on the radiation dose of the
prostate. The most important factor for the vertical prostate motion was the
difference between the planning- and the online rectum volume. The main
advantage of the markers lies in reducing the outliers in vertical direction
with low improved radiation exposure of the prostate. For decrease of the
outliers in bone based IGRT we suggest an additional matching with the use of
the ventral rectum, which correlates well with the vertical position of the
prostate. The evaluation of the clinical benefit of the usage of markers and
the additional matching with the ventral rectum as alternative to the markers
needs further investigation.
