Um das Verständnis über die Pathologie der Osteoarthrose zu verbessern, wurden
die relativen Genexpressionshöhen von proinflammatorischen Zytokinen (IL-1
beta und TNF alpha) und den Matrix Metalloproteinasen untersucht. Dabei wurden
diese Faktoren in der Synovialmembran von Patienten mit Kniegelenkosteoarthose
und bei Patienten mit Kniegelenkverletzungen bestimmt. Ferner wurde überprüft,
ob eine Korrelation zwischen diesen Faktoren und dem des Body Mass Index und
dem C-reaktiven Protein (CRP) besteht. Ausserdem wurde das subjektiv-objektive
Patientenempfinden erhoben. MMP-1, MMP -3, IL-1 beta and TNF alpha
Genexpressionshöhen wurden mittels der Taqman-basierten RT PCR in der
Synovialmembran von 12 Patienten mit Osteoarthrose und 13 Patienten mit
Kniegelenkverletzungen untersucht. Der BMI aus Körpergröße und Gewicht und das
C-reaktive Protein (CRP) wurden über das reguläre präoperative Labor erhoben.
Die quantitative Analyse zeigte keine signifikanten Unterschiede zwischen
MMP-1, MMP-3, IL-1 beta and TNF alpha mRNA Genexpressionshöhe in der
Synovialmembran von Patienten mit Osteoarthrose und Kniegelenkverletzungen.
Das Serum CRP war signifikant bei Patienten mit Osteoarthrose erhöht,
verglichen mit der Gruppe von Patienten mit Kniegelenkverletzungen. Eine
positive signifikante Korrelation zeigte sich zwischen mRNA
Genexpressionshöhen von MMP-1 und MMP-3 in beiden Patientengruppen, sowie
zwischen CRP und MMP-1, MMP-3 bei den Patienten mit Osteoarthrose. Ausserdem
zeigte sich eine signifikante Korrelation zwischen TNF alpha und MMP-1, MMP-3
in der Synovialmembran von Patienten mit Osteoarthrose. Es wurde u.a. eine
positive signifikante Korrelation zwischen der TNF alpha mRNA
Genexpressionshöhe und der Zeit nach der Verletzung beobachtet. Diese Studie
zeigt ähnliche Entzündungsmuster in der Synovialmembran von Patienten mit
Osteoarthrose und Patienten mit Kniegelenkverletzungen. Interessanterweise
deuten unsere Daten daraufhin, dass TNF alpha eine Schlüsselrolle in beiden
Krankheitsprozessen spielt und einen therapeutischen Ansatzpunkt darstellen
könnte. Ausserdem könnte TNF alpha ein individueller Risikofaktor für die
Entstehung einer sekundären Osteoarthrose darstellen.

To gain further insight into the pathology of osteoarthritis (OA), the
expression of pro-inflammatory cytokines and matrix metalloproteinases (MMPs)
in the synovial tissue of patients suffering from OA or non-inflammatory knee
disorders (NKD) has been investigated. Furthermore, to assess the influence of
clinical parameters such as CRP and BMI in disease pro-gression, we sought to
correlate the expression profile of the cytokines and proteases tested. MMP-1,
-3, IL-1 β and TNF-α gene expression was quantified using Taqman-based real-
time polymerase chain reaction (PCR) in synovial tissue samples obtained from
12 patients with OA and 13 with NKD. In addition, the levels of serum
inflammatory parameter C reactive protein (CRP) were recorded. Quantitative
analysis revealed no significant differences in the expression levels of MMPs,
IL-1β or TNF-α mRNA between the synovial tissues of patients with OA and NKD,
but CRP was sig-nificantly increased in the OA group. A significant
correlation was also seen regarding the gene expression levels between MMP-1
and -3, and between the CRP and MMPs tested. Furthermore, a significant
relationship between TNF-α and MMP-1 and MMP-3 was observed in osteoarthritic
synovial tissue. In addition, the relative mRNA level of TNF-α in the synovial
tissue correlated to the time after injury in patients with NKD. This study
demonstrates similar changes in the inflammatory pattern of synovial tissue of
NKD and OA. Interestingly these results suggest that TNF-α is a key factor in
both disease processes, and could provide a therapeutic target and an
individual risk factor for the development of OA.
