Ziel dieser Arbeit war es zu klären, ob die Konzentration der
Wachstumsfaktoren TGF-ß1 und IGF-1 sowie des Bindungsproteins IGFBP-3 im
subchondralen Knochen bei Arthrose verändert ist. Weiter wurde die Korrelation
zwischen deren Konzentrationen und der mechanischen Belastung infolge der
Achsenfehlstellung untersucht. Es wurde bei 55 Patienten mit Gonarthrose kurz
vor der Versorgung mit einer Endoprothese eine osteodensito-metrische
Untersuchung des distalen Femurs und der proximalen Tibia durchgeführt (DEXA).
Im gleichen Sin-ne wurde in einer Kontrollgruppe von 15 Verstorbenen ohne
Arthrose die Knochendichte am Kniegelenk gemes-sen. Im subchondralen
Knochengewebe der Femurkondylen wurden die Konzentrationen von IGF-1, IGFBP-3
und TGF-ß1 getrennt medial und lateral bestimmt (RIA;ELISA). Bei
Varusgonarthrose lag im medialen Fermurkondylus die Knochendichte (1,43g/cm²)
um 33% höher als in einer alters- und geschlechtsgepaarten
Referenzgruppe(0,98g/cm²; P<0,01; ANOVA) Die IGF-1 (0,27ng/mg) und TGF-ß1
(60,9pg/mg) Konzentration lag um fast die Hälfte höher als in der
Kontrollgruppe (0,19ng/mg; bzw. 30,4pg/mg; jeweils P<0,001; ANOVA). Für
IGFBP-3 ergaben sich 2,8 fach niedrigere Spiegel (4 ng/mg) als in der
Kontrollgruppe (12,38ng/mg; P<0,001; ANOVA). Es lässt sich ein Bezug zwischen
der Achsfehlstellung mit der daraus resultierenden mechanischen Belastung und
der IGF-Spiegel sowie IGFBP-3 im Knochen herstellen. Innerhalb der
Arthrosegruppe lagen die IGF-1 Spiegel bei Varusgonarthrose medial (0,25ng/mg
Knochen) um 31% höher als lateral (0,19 ng/mg Knochen; P=0,02; t-Test). Bei
Valgusgonarthrose war dagegen lateral (0,19ng/mg Knochen) die IGF-1
Konzentration um 18% erhöht (medial: 0,16ng/mg Knochen; P=0,23; t-Test). Die
IGFBP-3 Konzentration lag bei Varusgonarthrose medial (4,50ng/mg Knochen)um
25% niedriger als lateral (5,98ng/mg Knochen; P< 0,001 t-Test). Bei Valgus-
fehlstellung waren die IGFBP-3 Spiegel lateral (3,06ng/mg Knochen) um 40%
signifikant niedriger als im von Arthrose weniger betroffenen medialen
Gelenkabschnitt (5,06ng/mg Knochen; P= 0,04 ; t-Test). Für TGF-ß1 ließ sich
keine achsenabhängige Verteilung nachweisen. Zusammenfassend betrachtet konnte
in dieser Untersuchung an Kniegelenken also anhand der biochemischen Messungen
gezeigt werden, dass die Wachstumsfaktoren TGF-ß1 und IGF-1 im subchondralen
Knochen bei Arthrose in erhöhter, das IGFBP-3 aber in geringerer Konzentration
vorliegen. Weiter ließen sich für die sub-chondrale Knochendichte sowie für
IGF-1 und das zugehörige Bindungsprotein IGFBP-3 eine Korrelation zur
Achsendeformität und damit zur Belastung nachweisen.

The aim of this study was to quantify the concentration of the Transforming
Growth Factor-ß1 (TGF-ß1), the Insulin-like Growth Factor-1 (IGF-1) and its
Insulin-like Growth Factor Binding Protein-3 (IGFBP-3) in the subchondral bone
in osteoarthrosis. The correlation between these biochemical factors and the
biomechanical load because of axial deformation was determined.
Osteodensitometric determination of the distal femur and the proximal tibia of
55 patients with osteoarthrosis of the knee was undertaken before total knee
arthroplasty (DEXA). As well in 15 individuals without osteoarthrosis the bone
mineral density of the knee joint was measured. The concentration of IGF-1,
IGFBP-3 und TGF-ß1 was determined separately in the subchondral bone of the
medial and lateral femoral condyle (RIA; ELISA). The bone mineral density of
the medial femoral condyle (1,43g/cm²) in patients with varus gonarthrosis was
elevated by 33% compared to age- and sex- matched normal controls (0,98g/cm²;
P<0,01; ANOVA). The concentrations of IGF-1 (0,27ng/mg) and TGF-ß1 (60,9pg/mg)
were nearly 50% higher than in the control group (0,19ng/mg; respectively
30,4pg/mg; P<0,001; ANOVA). The determination of IGFBP-3 showed 2.8-fold lower
levels (4 ng/mg) than in the control group (12,38ng/mg; P<0,001; ANOVA). There
is a correlation between the axial deformation with the resulting mechanical
load and the levels of the IGF-1 and the IGFBP-3 in the bone. Within the
osteoarthrosis group with varus gonarthrosis the medial level of IGF-1
(0,25ng/mg bone) was 31% higher than lateral (0,19 ng/mg bone; P=0,02;
t-Test). In the group with valgus gonarthrosis however the concentration of
IGF-1 in the lateral condyle was elevated by 18% (medial: 0,16ng/mg bone;
P=0,23; t-Test). The concentration of IGFBP-3 in varus gonarthrosis was 25%
lower in the medial femoral condyle (4,50ng/mg bone) than in the lateral
(5,98ng/mg bone; P< 0,001 t-Test). In osteoarthrosis with valgus deformity the
lateral IGFBP-3 level (3,06ng/mg bone) was by 40% significantly lower than in
the less affected medial part (5,06ng/mg bone; P= 0,04 ; t-Test). There were
no differences in the axial dependent determination of the TGF-ß1
concentration. In summary our study of knee joints suggests that
osteoarthrosis is associated with higher concentration of the biochemical
growth factors TGF-ß1 and IGF-1 and lower concentration of IGFBP-3 in the
subchondral bone. The subchondral mineral bone density and the level of IGF-1
with its binding protein IGFBP-3 correlate with the axial deformity of the
knee and therefore with the mechanical load.
