dc.description.abstract
Ziel dieser Arbeit war die Darstellung der Lage, der Form und der
Gestaltentwicklung des Foramen infraorbitale unter Berücksichtigung des
Canalis infraorbitalis und des infraorbitalen Gefäßnervenkomplexes.
Es wurden neun menschliche Embryonen der Scheitel-Steiß-Länge (SSL) 19 mm
250 mm, die in histologischen Schnittserien der Schichtdicke 10 µm vorlagen
und mit Hämatoxylin-Eosin oder nach modifizierter Masson-Goldner-Färbung
gefärbt waren, untersucht.
Vier dieser Schnittserien wurden mittels Zeichenmikroskop erfasst und mit
Hilfe der Software Analysis© dreidimensional rekonstruiert.
Es ergaben sich folgende Befunde:
* Der Zeitpunkt des ersten Ossifikationszentrums der Maxilla zeigte sich bei einer SSL von 19 mm. Es lag nahe kaudal des Nervus infraorbitalis, der sich dort in seine Endäste aufzweigte und entsprach der späteren Lage des Foramen infraorbitale.
Bei einer SSL von 25 mm verlief die Entwicklung der Maxilla weiterhin kaudal
entlang des Nervus infraorbitalis nach anterior, und krümmte sich dann nach
medial in Richtung der knorpeligen Spina nasalis anterior. Erst später,
ersichtlich bei SSL 117 mm, breitete sich das Wachstum der Maxilla auch nach
posterior entlang des Nervus infraorbitalis und des Nervus maxillaris aus.
Weiterhin waren auch in kranialer Richtung Wachstumsvorgänge zu verzeichnen,
die den späteren Processus frontalis der Maxilla bilden. Im rekonstruierten
Ausschnitt des Embryos der SSL 250 mm war zu erkennen, wie der maxilläre
Knochen die Zahnanlagen und den infraorbitalen Gefäßnervenkomplex in seinem
anterioren Anteil vollständig umgab.
* Erste Anzeichen zur Formgebung des Foramen infraorbitale waren bei einer SSL von 25 mm zu erkennen. An der Gabelung des Nervus infraorbitalis in seine Endäste breitete sich die Maxilla trichterförmig nach kranial aus, wodurch die Anlage des Foramen infraorbitale gebildet war. Durch das Wachstum der Maxilla nach kranial und nach posterior entlang des Nervus infraorbitalis, nahmen das Foramen infraorbitale und der Canalis infraorbitalis die Gestalt einer u-förmigen Rinne an. Diese Rinne zeigte sich bei einer SSL von 117 mm zunächst noch nach kranial offen und hatte eine in der Frontalansicht längliche, tropfenförmige Gestalt angenommen. Bedingt durch die beschriebene Wachstumsrichtung der Maxilla wurde das spätere Foramen infraorbitale nach anterior und medial verlagert und lag bei einer SSL von 117 mm kranial des ersten Milchmolaren. Später, bei einer SSL von 250 mm, verlagerte es sich weiter nach anterior auf die Höhe des Milcheckzahns.
* In der dreidimensionalen Rekonstruktion war das Foramen infraorbitale zum ersten Mal bei einer SSL von 250 mm in seiner knöchernen Circumferenz vollständig zu erkennen. Der kraniale Bereich war nun durch eine dünne Knochenlamelle geschlossen und die tropfenförmige Gestalt wurde breiter und flacher. Zu diesem Zeitpunkt war das Foramen infraorbitale fast vollständig vom infraorbitalen Gefäßnervenkomplex ausgefüllt.
* Der horizontale Durchmesser des Foramen infraorbitale nahm im Wachstumsverlauf kontinuierlich zu. Er betrug bei einer SSL von 117 mm
400 µm 500 µm. Zu einem früheren Entwicklungsstadium waren solche Messungen
aufgrund der zu geringen Ausbildung des Foramen noch nicht möglich. Bei einer
SSL von 250 mm betrug der Durchmesser dann 750 µm 800 µm.
* Ähnlich kontinuierlich nahm der laterale und der mediale Abstand des Nervus infraorbitalis zur knöchern Foramenwand ab. Er betrug bei einer SSL von 117 mm noch 170 µm 340 µm, bei einer SSL von 250 mm nur noch 50 µm 130 µm.
Der Abstand des Nervus infraorbitalis zur kaudalen knöchernen Begrenzung blieb
hingegen nahezu gleich. Er betrug bei der SSL 117 mm 90 µm 340 µm, und bei
der SSL 250 mm 130 µm 350 µm. Durch die kaudal nahe gelegenen Zahnanlagen
wurde diese Knochenschicht mitunter sehr dünn gehalten.
Das Foramen infraorbitale zeigte im Vergleich zum Foramen mentale einen im
Grunde ähnlichen Wachstumsverlauf. Bei beiden Foramina entstanden erste
Knocheninseln kaudal des Nervenverlaufs. Diese breiteten sich beiderseits des
Nerven in Form einer Rinne nach kranial aus und fügten sich schließlich
kranial des Nerven zu einem knöchernen Foramen zusammen. Im Unterschied zum
Foramen infraorbitale war der Befund des vollständigen Foramen mentale schon
bei einer SSL von 19 mm zu beschreiben. Auch der Abstand des Nervus mentalis
zur knöchernen Foramenwand war in diesem Stadium bedeutend größer (dreifache
Größe des Nervendurchmessers) als der Abstand des Nervus infraorbitalis zur
knöchernen Wand des Foramen infraorbitale bei der SSL von 250 mm.
Zusammenfassend kann gesagt werden, dass trotz der doch ähnlichen Morphogenese
beider Foramina, noch andere Faktoren auf die individuelle knöcherne
Formgebung Einfluss ausüben müssen.
Um diese individuellen und regionsspezifischen Unterschiede der formgebenden
Knochenentwicklung verifizieren zu können, müssen nun weiterführend Studien
auf molekularbiologischer Ebene durchgeführt werden.

dc.description.abstract
The aim of this investigation was to present the morphogenesis, the
localisation and the form of the infraorbital foramen with consideration of
the infraorbitalis canal and its vessels and nerves.
Nine human embryos and fetuses ranging from 19 mm 250 mm crown-rump-length
(CRL) have been prepared as histological serial sections of 10 µm thickness
and were stained by haematoxylin-eosin or modified Masson-Goldner staining.
Four of these serial sectioned embryos were microscopically enlarged and
reconstructed as three-dimensional graphics with a the software (Analysis©).
Results:
* The first condensation of osteocytes of the maxilla occurred at the stage of 19 mm CRL. They were located caudally, close to the infraorbital nerve, where the infraorbital foramen will develop and where the infraorbital nerve ends with its terminal branches.
At the stage of 25 mm CRL the further development of the maxilla proceeded
caudally of the infraorbital nerve in anterior direction and bent midwards to
the chondral anterior nasal spine. Not before the reconstructed stage of 117
mm CRL it has been obvious that the maxilla developed along the infraorbital
and maxillary nerve in posterior direction. It was further recognizable that
the maxillary bone-growth went in cranial direction where the frontal process
will develop later. At the developmental stage of 250 mm CRL the infraorbital
nerve and the anterior tooth buds were completely encircled by bone.
* First obvious signs of bone-formation of the infraorbital foramen have been seen at a stage of 25 mm CRL. At the bifurcation where the infraorbital nerve divided into its terminal branches the maxillary bone spreaded gutter-like in cranial direction. At this location the formation of the infraorbital foramen started to develop. Because of the maxillary development in cranial and in posterior direction surrounding the infraorbital nerve, the infraorbital foramen and the infraorbital canal became to be a u-shaped gutter. At the stage of 117 mm CRL the gutter still was cranially open and had a long and drop-shaped form if viewed in the frontal plane. Conditioned by the described growth direction of the maxillary bone, the formation of the infraorbital foramen was displaced in anterior and medial direction to be located cranial of the first upper deciduous molar at the stage of 117 mm CRL. Later, at the stage of 250 mm CRL, it further shifted to the level of the upper deciduous canine.
* The first time where the infraorbital foramen was completely encircled by bone in the three-dimensional reconstruction was at a size of 250 mm CRL. The cranial bony surface was quite thin and the drop-shaped formation of the foramen became wider and more flat. At this stage of development, the infraorbital foramen was almost completely occupied by the infraorbital nerve and vessels.
* The horizontal diameter of the infraorbital foramen grew continuously during the developmental process. At the stage of 117 mm CRL the diameter measured 400 µm 500 µm. No vertical measurements were possible in less advanced developmental stages, because the cranial vault of the infraorbital foramen had not been formed. At the size of 250 mm CRL the diameter showed 750 µm 800 µm.
* Also the distance between the infraorbital nerve and the medial and lateral bony border of the infraorbital foramen decreased continuously. The distance measured 170 µm 340 µm at the stage of 117 mm CRL and at the stage of 250 mm CRL it was reduced to 50 µm 130 µm.
The distance between the infraorbital nerve and the caudal bony border seemed
to stay stable. It showed 90 µm 340 µm at the size of 117 mm CRL and 130 µm
350 µm at the size of 250 mm CRL. Because of the close relation to the
caudal tooth buds the bony layer could be very thin in this region.The
developmental formation of the infraorbital foramen in comparison to the
mental foramen seemed to be similar. At both foramina the first osseous
condensations have been seen close caudal the nerve fibers. Those ossification
centers spread out gutter-like in cranial direction on both sides of the
nerve-bundle and finally united cranially of the nerve to build a completely
encircled bony foramen. In contrast to the infraorbital foramen a completely
formed mental foramen was already recognizable at the stage of 19 mm CRL. The
distance between the mental nerve and the mental foramen at this stage seemed
to be significantly larger (the foramen was three times bigger than the
diameter of the nerve) than the distance between the infraorbital nerve and
the infraorbital foramen at the stage of 250 mm CRL.
It can be concluded that the infraorbital foramen develops in a different
albeit similar way, and it is not clear, if there are any other factors beside
the ones we no, which might control the development of foramen formation.
Therefore, further studies have to be done on the microbiological way to study
and to verify those individual differences of boneformation in specific
anatomical regions.
