Einleitung: Läsionen der Subscapularissehne (SSC) haben eine große klinische
Bedeutung, werden in der klinischen Routine aber häufig übersehen. Daher
sollten die etablierten SSC-Tests hinsichtlich ihrer diagnostischen Wertigkeit
verglichen werden. Ein für die lange Bizepssehne (LBS) spezifischer Score (LBS
Score), der zunächst zur klinischen Nachuntersuchung Verwendung fand, wurde
prospektiv evaluiert. Für die Versorgung von Pathologien der LBS sind neben
der alleinigen Tenotomie auch verschiedene Techniken zur Tenodese beschrieben
worden. Diese wurden mittels des LBS Score untereinander verglichen. Methodik:
Der Lift-Off-Test, das Innenrotations-Lag-Zeichen, der Belly-Press-Test und
das Belly-Off-Zeichen wurden präoperativ erhoben, und um ihre diagnostische
Wertigkeit untereinander zu vergleichen, mit den intraoperativen Befunden der
Subscapularissehne korreliert (Publikation 1). Zur Beurteilung seines
diagnostischen Nutzens wurde der LBS Score prospektiv, prä- und postoperativ
bei zwei unterschiedlichen LBS Therapien sowie einer Kontrollgruppe erhoben
und mit dem Constant Score verglichen (Publikation 2). Eine Weichteiltenodese
und eine Ankertenodese der LBS wurden hinsichtlich ihres klinischen und
kernspintomographischen Outcomes verglichen. Zur klinischen Beurteilung wurde
dazu der LBS Score eingeführt und verwendet (Publikation 3). Ergebnisse: Der
Belly-Press-Test und das Belly-Off-Zeichen zeigten die höchste Sensitivität.
Das Belly-Off-Zeichen hatte die höchste Spezifität und die insgesamt höchste
Genauigkeit. Mit dem Belly-Press-Test und dem Belly-Off-Zeichen konnten die
kranialen Läsionen der SSC-Sehne (Typ I und Typ II) präoperativ gut
diagnostiziert werden (Publikation 1). Der Constant Score und der LBS Score
stiegen in allen 3 Gruppen postoperativ signifikant an. Gruppe I zeigte im
Vergleich zu Gruppe II signifikant bessere Ergebnisse im LBS Score und bei der
Bewertung der kosmetischen Deformität (Publikation 2). Der Constant Score
zeigte keine signifikanten Unterschiede zwischen der Weichteiltenodese und der
Ankertenodese. Die Ankertenodese schnitt hingegen im LBS Score, in der
objektiven Bewertung der kosmetischen Deformität sowie bei der
kernspintomographischen Bewertung des Tenodesekonstruktes signifikant besser
ab als die Weichteiltenodese (Publikation 3). Schlussfolgerung: Die Anwendung
der gegenwärtig verfügbaren Tests erlaubt in den meisten Fällen eine
präoperative Detektion von Läsionen der SSC-Sehne. Dennoch konnten einige
Läsionen auch unter Nutzung aller Tests präoperativ nicht diagnostiziert
werden (Publikation 1). Zur Verlaufsbeurteilung von Patienten mit Pathologien
der LBS sollte der LBS Score verwendet werden, da er spezifische Informationen
liefert. Als alleiniges Werkzeug zur Diagnostik ist er dagegen nicht geeignet
(Publikation 2). Ankertenodesen bieten gegenüber der Weichteiltenodese der LBS
eindeutige klinische und radiologische Vorteile (Publikation 3).

Introduction: Subscapularis (SSC) lesions have a high relevance but are often
underdiagnosed in the clinical routine. Therefore the current examination
techniques were evaluated to their diagnostic values. A specific score for the
long head of the tendon of the biceps muscle (LHB score), that has been used
to evaluate the clinical outcome after surgery, was prospectively tested in
regards to its preoperative diagnostic value. Different tenodesis-techniques
for the LHB as well as isolated tenotomy have been described. Soft tissue
tenodesis, bony fixation anchor tenodesis and isolated tenotomy were compared
using the LHB score. Methods: The lift-off test, the internal rotation lag
sign, the modified belly-press test and the belly-off sign where executed
preoperatively and then correlated with the findings in arthroscopy to assess
their diagnostic values (publication 1). The diagnostic performance of the LHB
score was tested by prospectively evaluating it in two different LHB therapies
and a control group, and comparing it with the Constant score (publication 2).
Soft tissue tenodesis and bony fixation anchor tenodesis were compared in
regards to their clinical and radiological outcome. To assess the clinical
outcome the LHB score was introduced and used (publication 3). Results: The
modified belly-press test and the belly-off sign showed the greatest
sensitivity. The belly-off sign had the greatest specificity and the highest
overall accuracy. The belly-off sign and the modified belly-press test are
most sensitive for upper SSC lesions (publication 1). The Constant score and
the LHB score improved significantly in all three groups. Compared to
tenotomy, tenodesis showed significant better results in the total LHB score
and in the cosmetic results (publication 3). The Constant score did not reveal
any significant difference between the soft tissue tenodesis and the bony
fixation anchor tenodesis. The bony fixation anchor tenodesis showed
significantly better results in the LHB score, the objective evaluation of the
cosmetic result, and in the radiological evaluation (publication 2).
Conclusion: Using the current SSC tests allows diagnosing SSC lesions in most
cases. However, some tears could not be predicted by preoperative assessment
using all the tests (publication 1). We recommend that patients with LHB
pathologies are evaluated using the LHB score, since it provides LHB specific
information and is a proper tool to assess the clinical outcome after surgery.
However, the score is not suitable to detect LHB pathologies preoperatively
(publication 2). Bony fixation anchor tenodesis is recommended over soft
tissue tenodesis because it provides significant clinical and radiological
advantages (publication 3).
