Acute myeloid leukemia (AML) is a biologically heterogeneous malignancy of the hematopoietic system. Despite the development of novel therapies, more than 50 % of patients relapse and die of the disease. For patients not suitable for intensive therapy the outcome is even worse. Here, B-Cell Lymphoma 2 (BCL2)-inhibition combined with the hypomethylating agent 5-Azacytidine represents the standard of care. However, the median overall survival in this patient subgroup remains unsatisfactory and the majority of patients, especially patients with high-risk genetics like inactivation of the tumor suppressor gene Tumor Protein 53 (TP53), experience relapse. In order to enhance patient outcomes, it is essential to understand the cellular factors that influence sensitivity to BCL2-inhibition. The objective of our study was to elucidate potential resistance mechanisms to BCL2-inhibition and identify novel drug targets for BCL2-inhibitor combination therapies to enhance efficacy in AML. To this end, we employed a functional genomics approach utilizing genome-wide Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR)-CRISPR associated protein 9 (Cas9) activation and knockout resistance screens in two AML cell lines. Our screens generated a comprehensive dataset of candidate resistance and sensitizer genes in Venetoclax treatment. Our activation screen identified 12 candidate genes that induced resistance in the TP53 mutant cell line HL60 and TP53 wildtype (WT) OCIAML5 cells. Upregulation of apoptotic regulators including B-Cell Lymphoma 2 Like 1 (BCL2L1 alias BCL-xL), B-Cell Lymphoma 2 related A1 (BCL2A1 alias BFL1) and BCL2 conferred resistance to BCL2-inhibition while their downregulation enhanced sensitivity. Further top hits were Friend Leukemia Integration 1 Transcription Factor (FLI1), Ubiquitin Associated Protein 2 Like (UBAP2L) and Colony Stimulating Factor 1 Receptor (CSF1R). The resistance conferred by the B-Cell Lymphoma 2 Homology (BH) 3 protein BCL2A1 was validated by retroviral overexpression of BCL2A1, which rescued AML cell lines from BCL2-inhibition. Furthermore, our screens revealed the deubiquitinating enzyme Ubiquitin Specific Peptidase 7 (USP7), for which selective inhibitors exist, as a potential TP53-independent target. USP7-inhibition was highly synergistic with BCL2-inhibition. Mechanistic studies revealed that this is mediated by inactivation of the NADPH Oxidase 2 (NOX2) complex, leading to reduced reactive oxygen species (ROS) generation and transcriptional downregulation of BCL2A1, providing an indirect, druggable approach to inhibit BCL2A1 and enhance BCL2-inhibitor activity. This study has unveiled potential novel resistance mechanisms associated with BCL2-inhibition and has identified strategies to target these mechanisms. If successfully translated into clinical practice, these findings may pave the way for the development of improved therapeutic approaches and enhanced outcomes for AML patients.
Akute myeloische Leukämie (AML) ist eine biologisch heterogene Erkrankung des blutbildenden Systems. Trotz neuer Therapieoptionen, werden noch immer mehr als 50 % der Patienten rückfällig und sterben an der Krankheit. Für eine intensive Therapie ungeeignete Patienten, haben eine noch schlechtere Prognose. Bei diesen Patienten stellt die Kombination aus B-Cell Lymphoma 2 (BCL2)-Inhibition und der hypomethylierenden Substanz 5-Azacytidin den Behandlungsstandard dar. Das mittlere Gesamtüberleben in dieser Patientengruppe ist nach wie vor unbefriedigend und die Mehrzahl der Patienten, besonders Patienten mit einer Mutation des Tumorsuppressorgens Tumor Protein 53 (TP53), wird rückfällig. Um Behandlungsstrategien nachhaltig zu verbessern, bedarf es eines tieferen Verständnisses der zellulären Faktoren, welche die BCL2-Inhibition beeinflussen. Ziel dieses Projektes war es, potenzielle Resistenzmechanismen gegen BCL2-Inhibition aufzudecken und neue Angriffspunkte für BCL2-Inhibitor-Kombinationstherapien zu identifizieren, um deren Wirksamkeit zu verbessern. Wir verwendeten einen funktionellen genomischen Ansatz, basierend auf genomweiten Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR)-CRISPR-assoziiertes Protein 9 (Cas9) Aktivierungs- und Knockout-Resistenzscreens in zwei AML-Zelllinien. Die Screens identifizierten das deubiquitinierende Enzym Ubiquitin Specific Peptidase 7 (USP7), welches selektiv inhibiert werden kann, als einen potenziellen TP53-unabhängigen Resistenzmodulator. USP7- und BCL2-Inhibition wirkten stark synergistisch. Weitere Untersuchungen zeigten, dass dieser Effekt durch die Herunterregulation der NADPH Oxidase 2 (NOX2) vermittelt wird. Durch eine geringere Bildung reaktiver Sauerstoffspezies (ROS), kommt es zu einer niedrigeren Expression von BCL2A1, was einen indirekten, medikamentösen Ansatz zur BCL2A1-Hemmung darstellt. Bei erfolgreicher Translation der Ergebnisse dieses Projekts in die klinische Praxis, ebnen diese den Weg für die Entwicklung besserer therapeutischer Ansätze und Ergebnisse für AML-Patienten.
