The effect of sonic and ultrasonic activation on bacterial reduction in dual-species biofilm ex vivo

Abstract

Introduction: The removal of microorganisms by chemomechanical debridement is critical for root canal treatment success, although full eradication of bacteria remains challenging. Improvement and comparison of techniques to increase disinfection is clearly warranted.

Aim: This ex vivo study aimed to evaluate the effects of sonic and ultrasonic activation on reducing bacteria in an artificial dual-species biofilm within the root canal of extracted single-rooted teeth with straight root canals.

Methodology: 240 extracted human front teeth were selected, crowns removed, lengths were standardized to 19 mm, and embedded into resin. 120 roots were instrumented up to size 25, .06 taper (G1), the other 120 were instrumented up to size 40, .06 taper (G2), using rotary files (ProTaper Next, Dentsply Sirona), and then subsequently plasma sterilized. A dual-species biofilm with Enterococcus faecalis and Streptococcus oralis was established inside the root canal within 5 days, confirmed using Fluorescence-in-situ-hybridization (FISH). In group G1, teeth were instrumented up to size 40, .06 taper and received irrigation and activation, while group G2 only received irrigation and activation. Subgroups were treated with either non-activated manual irrigation, sonic activation (2 x 30 s), or ultrasonic activation (2 x 30 s), using sodium hypochlorite (NaOCl 1%) or saline (NaCl 0.9%) as irrigants. Dentin-adherent and planktonic samples were taken to determine bacterial concentrations before treatment (T0), immediately after treatment (T1), and 5 days after further incubation (T2). Logarithmic reduction factors (LRF) were calculated, and statistical analysis was performed using ANOVA and post-hoc Tukey-HSD tests, stratified by sampling time and bacterial location (dentin-adherent and planktonic).

Results: Dentin-adherent and planktonic bacterial counts were highly correlated (r(LRF1) = 0.91, r(LRF2) = 0.8). At T1, significant bacterial reductions were observed due to instrumentation, irrigant, and activation (p < 0.0001 each), with significant interactions between these factors (p < 0.05). In group G2, the combination of irrigant and activation method significantly reduced bacteria (p < 0.05). Both sonic and ultrasonic activation significantly enhanced bacterial reduction compared to non-activated groups (p < 0.0001 and p = 0.002, respectively), with sonic activation leading to higher LRFs. At T2, despite significant reductions from instrumentation, irrigants, and activation (p < 0.0001 each), substantial bacterial regrowth was observed across all groups.

Conclusion: In this ex vivo study, sonic activation was more effective in reducing bacterial counts than ultrasonic activation, particularly in groups treated only with irrigation and activation. Both sonic and ultrasonic activation significantly enhance the effect of irrigants compared to non-activated groups, emphasizing the importance of activation even in straight root canals.

Einleitung: Die weitestgehende Entfernung von Mikroorganismen im Wurzelkanal durch die chemomechanische Aufbereitung ist für den Erfolg einer Wurzelkanalbehandlung entscheidend, stellt jedoch noch immer eine Herausforderung dar. Die Wirksamkeit verschiedener Techniken zur Desinfektionssteigerung im Wurzelkanal wird kontrovers diskutiert.

Ziel: Ziel dieser Laborstudie war es, die Wirksamkeit von Schall- und Ultraschallaktivierung auf die Reduktion der Bakterienkonzentration eines endodontischen Biofilms in Wurzelkanälen von extrahierten einwurzeligen humanen Frontzähnen zu untersuchen.

Methode: 240 extrahierte einwurzelige humane Frontzähne wurden auf 19 mm standardisiert und in Kunststoff eingebettet. 120 Kanäle wurden bis ISO 25, .06 Konizität (G1) und 120 Kanäle bis ISO 40, .06 Konizität (G2) mit ProTaper Feilen (ProTaper Next, Dentsply Sirona) instrumentiert und anschließend plasmasterilisiert. Ein Dual-Spezies-Biofilm aus Enterococcus faecalis und Streptococcus oralis wurde über 5 Tage etabliert und mit Fluoreszenz-in-situ-Hybridisierung (FISH) nachgewiesen. Die Proben wurden in G1 und G2 aufgeteilt, wobei G1 eine kombinierte Aufbereitung und Desinfektion und G2 nur die Desinfektionsmethoden erhielt. Verwendete Desinfektionsmethoden waren NaOCl 1% oder NaCl 0.9% sowie Schall- oder Ultraschallaktivierung im Vergleich zu manueller Spülung. Die Bakterienkonzentration wurde vor der Behandlung (T0), unmittelbar danach (T1) und 5 Tage später (T2) gemessen. Planktonische und dentinadhärente Proben wurden entnommen, logarithmische Reduktionsfaktoren (LRF) ermittelt und die Daten statistisch mit ANOVA und Tukey-HSD Tests analysiert.

Ergebnisse: Dentinadhärente und planktonische Keimzahlen korrelierten stark (r(LRF1) = 0,91, r(LRF2) = 0,8). Zum Zeitpunkt T1 zeigten Instrumentierung, Spülflüssigkeit und Aktivierung (p < 0,0001 jeweils) sowie deren Interaktion (p < 0,05) signifikante Effekte auf die Reduktion der Bakterienkonzentration. In G2 hatten Spülflüssigkeit und Aktivierung signifikante Auswirkungen auf den LRF (p < 0,05). Schall- und Ultraschallaktivierung führten im Vergleich zu nicht aktivierten Gruppen zu signifikant höheren LRF-Werten (p < 0,0001 bzw. p = 0,002), wobei die Schallaktivierung effektiver war. Bei T2 wurde trotz signifikanter bakterieller Reduktion durch Instrumentierung, Spülung und Aktivierung (p < 0.0001 jeweils) ein erneutes Bakterienwachstum in allen Gruppen festgestellt.

Schlussfolgerung: In dieser In-vitro-Studie mit extrahierten humanen Frontzähnen führte die Schallaktivierung zu einer stärkeren Reduktion der Bakterienzahl im Vergleich zur Ultraschallaktivierung, insbesondere in Gruppen ohne Instrumentierung. Sowohl Schall- als auch Ultraschallaktivierung verbesserten die Wirkung der Spüllösungen im Vergleich zu nicht aktivierten Gruppen signifikant, was die Bedeutung der Aktivierung auch in geraden Wurzelkanälen unterstreicht.
